REMOVED FROM MGB. A CRACK WAS DISCOVERED BY FIELD MAINTENANCE ON THE GEARBOX COVER AND THE GEARBOX WAS REMOVED AT ACFT TT: 38,523.7. IT WAS REPAIRED AND RELEASED UNDER CRS W/O 7027. THE TRANSMISSION WAS INSTALLED ON THE SAME AIRCRAFT AT ACFT TT: 38,798.9 AND IS CURRENTLY INSTALLED.
AT OVERHAUL, REMOVED HUB BEARING INNER RACES AND FOUND (1 EACH) CRACKED ABOUT 1.5 INCHES DUE TO SEVERE BRINNELING. PROBABLE CAUSE COULD BE DUE TO GREASE (MINIMAL AMOUTS). RECOMMEND DAILY PURGING OF HUB BEARING TO PREVENT ANOTHER OCCURENCE. (K)
SPINDLE WAS FOUND CRACKED IN AREA OF FIRST THREAD. SPINDLE HAS BEEN SENT TO SIKORSKY AIRCRAFT.
AT OVERHAUL, TAIL ROTOR SPINDLE WAS FOUND CRACKED IN AREA OF FIRST THREAD. SIKORSKY AIRCRAFT HAS BEEN NOTIFED AND THE SPINDLE SENT TO THEM. (X)
THE ENGINE LOST FUEL AND OIL PRESSURE IN-FLIGHT AND QUIT. THE ACCESSORY DRIVE GEARBOX PINION SHAFT BEARINGS FAILED AND ALLOWED GEAR DISSENGAGEMENT TO FAILURE POINT. SUBMITTER RECOMMENDS REPLACING BEARINGS AT 3,600 HOURS TT.
RIGHT REAR FUSELAGE FITTING HAD A SMALL CRACK ON INSIDE OF REAR EAR. CRACK WAS FOUND BY DYE CHECK OF FITTING WHILE DOING A LANDING GEAR CHANGE. RECOMMEND DYE CHECK OF FITTING ANY TIME GEAR IS REMOVED.
REAR COVER WAS FOUND CRACKED ON NR 2 GENERATOR BOSS. THIS WAS A SURFACE CRACK ON INSIDE OF GEARBOX REAR COVER, AND WAS FOUND AT OVERHAUL.
TAIL ROTOR BLADE SKIN CRACK OUTBOARD SIDE APPROXIMATELY 19 INCHES FROM ROOT END OF BLADE. SUBMITTER RECOMMENDATIONS - CLEAN AND INSPECT DAILY PER SIKORSKY INSPECTION PROGRAM.
TAIL ROTOR BLADE SKIN CRACK OUTBOARD SIDE APPROXIMATELY 20 INCHES FROM ROOT END OF BLADE. RECOMMENDATIONS - CLEAN AND INSPECT DAILY PER SIKORSKY INSPECTION PROGRAM.
