RETURNED TO BASE AFTER LOW OIL PRESSURE ANNUNCIATOR CAME ON DURING TAXI. FOUND ENGINE IDLING AT 54-59% N1. UPON OPENING ENGINE COWLS FOUND ENGINE BAY WET WITH FUEL. FOUND FUEL LINE BETWEEN FCU AND FUEL PRESSURE TRANSDUCER, P/N 3040637, CHAFFED THROUGH TO WHERE A PIN HOLE EXISTED, AND LET THE FUEL PRESSURE DROP TO WHERE MINIMUM GROUND IDLE COULD NOT BE SUSTAINED. REMOVED AND REPLACED FUEL LINE IN ACCORDANCE WITH APPLICABLE INSTRUCTIONS, LEAK CHECKED AND RETURNED TO SERVICE.
BEFORE START UP THE NR 1 COM CONTROL HEAD SHORTED OUT & STARTED TO SMOKE. POWER WAS SHUTDOWN & WE BEGAN TO TROUBLESHOOT. AFTER LOOKING AT THE WIRING DIAGRAM & CHECKING THE POWER CIRCUIT AT THE NR 1 COM CONTROL CONNECTOR, FOUND THERE WAS NO SHORT TO GND. INSTALLED AN OVERHAULED COM CONTROL HEAD & POWERED IT UP & OPERATIONS CHECKED IT WITH NO ISSUES. CONCLUDED THAT THE COM CONTROL HEAD MUST HAVE HAD AN INTERNAL SHORT. THE AVIONICS SYS WAS POWERED UP FOR APPROX 20 MINUTES BEFORE TAKEOFF WITH NO ISSUES. THE WORK WAS SIGNED OFF & THE ACFT WAS RELEASED. AFTER ABOUT 30 MINUTES OF FLIGHT TIME THE NR 1 COM CONTROL STARTED TO SMOKE AGAIN. SHUT OFF THE AVIONICS SWITCH & WERE WITHOUT COMMUNICATION & NAVIGATION SYS. RETURNED WITHOUT COM OR NAV SYS. TROUBLESHOOTING AGAIN & WHEN THE BATTERY SWITCH WAS TURNED ON, THE BATTERY RELAY DID NOT CLOSE. MX WENT TO THE BATTERY BOX & FOUND THE BURNED DIODES & WIRES. AFTER REMOVING THE DIODES, FOUND THAT BOTH WERE MISSING AN INSULATION RING. PN DF3D.
DURING INSPECTION A CRACK WAS FOUND ON THE LT WING FLAP. THE ACFT WAS REMOVED FROM SERVICE & REPAIRED.

AFTER TAKEOFF FROM TAIPEI, DURING AIRCRAFT CLIMBING, CAPTAIN REPORT FIRST OBSERVOIR'S AIRSPEED INDICATION AND ALTITUDE INDICATION BLANK, CAPTAIN REQUEST AIR RETURN. ON GROUND CHECK FOUND NR 2 AIR DATA COMPUTER INOPERATIVE, AFTER REPLACED COMPUTER AND PERFORM PITOT-STATIC TEST, SYSTEM BACK TO NORMAL.
1.AIRCRAFT LOW SPEED T/O ABORTED (ABOUT 95 KTS) DUE TO "ENG FAIL" LT ON WITH "BAN" SOUD HEARD. AFTER ABORT ACTION COMPLETED. "ENG FAIL" RED LT WENT OFF. 2.GND VISUAL INSPETED NO. ENG EXHAUST SLEEVE, FOUND METAL CHIP AROUND INNER SURFACE, AND METAL CHIPS FOUND ON THE OIL MMCD. 3. PFM NO.1 ENG, FND HPC-1 AND THE FOLLOWING STAGES ALL WERE DAMAGED AND OVERLIMIT PER MM 72-00-07. 4.PFM NO.1 ENG CHANGED PER MM 71-00-00 AND PFM "PRE-TESTED TEST", ALL COND NML AND NO LEAK FND.
PILOT REPORTED NR 2 ENGINE, LOW RPM WARNING APPEARED, LEVEL 2 WITH N 1 25 PER CENT, EGT, 100 THEN WENT TO 0. NR 2, 40 PER CENT, OIL QTY 20, RECOVERED AFTER IGN OVRD SELECTED DURING STARTED DESCENT. MAINTENANCE RAN NR 2 ENGINE WITH BOTH A AND B IGNITION SYSTEM. NO FAULT NOTED, NO FAULT LOGED IN CFDS. AIRCRAFT RELEASED FOR SERVICE.
PILOT REPORTED ELECTRICAL BURNING ODOR DURING CLIMB, A IL 300, SMOKE APPEARED IN THE COCKPIT CAUSED FUEL DUMPED AND AIR RETURNED TO LAX. MAINTENANCE AND INSPECTION COCKPIT INDICATORS ALL C/B ARE CLOSED OPEN OVHD PANEL, MAINT PANEL AND AVIONICS PANEL, EXAMINED ALL COMPARTMENT, IND ALL CONDITIONS ARE NORMAL, NO BURN SIGN WAS FOUND. REVIEW LAST FLIGHT FAULT, REPLACED SYSTEM CONTROL PANEL FOR SAFETY AND MAINTENANCE PREVENTION.
ALERT MESSAGE 'NR 2 AIR MANIFOLD FAIL' DURING IN-FLIGHT AND DIVERTED TO CTS. PER MEL 32-23-1 RELEASE A/C. REPLACED MANIFOLD FAULT DETENTION CONTROLLER (NFDC) AND GROUND CHECK NORMAL.
PILOT REPORTED AT LIFT-OFF SMOKE WAS NOTICED FROM THE GALLEY AREA, CAUSED AIR-TURNBACK. MAINTENANCE RAN ALL THREE ENGINES AT HIGH POWER WITH GALLEY POWER OFF, THEN ON BLEED CHECK. NO INDICATION OF SMOKE, FOG. CHECKED GALLEY AREA, NO SMOKE FOUND. AIRCRAFT RETURNED TO SERVICE.
MALFUNCTION OF THIS DIMMER ASSY OF COCKPIT OVERHEAD BACKGROUND LIGHT CAUSED SMOKE IN THE COCKPIT AND AIR TURN BACK DURING FLIGHT ROUTE OF TPE TO ANC ON 7-13-98. EVA REPAIR STATION (FAA CIRS NR QOCY349J) INSPECTED AND FOUND NO HIDDEN DAMAGE, EXCEPT THE OVERHEAD LIGHT DIMMER ASSY INOPERATIVE. REPLACED A NEW DIMMER ASSY IAW MM 33-11-03. AIRCRAFT RETURNED TO SERVICE. (X)
INSPECTION FOUND NR 2 ENGINE T/R COWL RT DAMAGED. ALSO FOUND TRANSLATING COWL RT LOWER SIDE DAMAGE ABOUT 10 CM BY 8 CM WHEN TRANSIT IN ANC. REPLACED RT HALF OF BOTH FAN COWL AND TRANSLATING COWL.
