UPON INSPECTION OF ALL PARTS NR 2 WHEEL HAD ONE BLADE MISSING.
DURING PRESTART CHECK. NR 1 FIRE WARNING SYSTEM WOULD NOT TEST. AFTER ENGINES WERE RUN AND ENGINE COMPARTMENTS WERE DRIED OUT, SYSTEM TESTED NORMALLY.
UPON DISASSEMBLY OF TURBINE FOR EVALUATION DUE TO ENGINE QUITTING DURING OPERATION. UPON INSPECTION OF ALL PARTS NR 2 WHEEL HAD ONE BLADE MISSING. PERFORM INSPECTION OF WHEELS UPON ASSEMBLY AT MANUFACTURER FOR POSSIBLE IMPERFECTIONS.
IN-FLIGHT, HYDRAULIC BLOCK ILLUMINATED; SYSTEM SWITCHED TO NR 2 HYDRAULIC SYSTEM. RETURNED TO BASE WITHOUT INSTALLED NEW HARNESS.
IN CRUISE FLIGHT, THE NR 2 ENGINE FIRE LIGHT FLASHED ON AND OFF CONTINUOUSLY. DESCENDED TO 300 FEET, CHECKED SHADOW FOR SMOKE, NONE SEEN, RETURNED TO BASE. ALL ENGINE INSTRUMENTS NORMAL. REMOVED NR 2 ENGINE COWL, INSPECTED LOWER DETECTOR, INSPECTED UPPER DETECTOR, FOUND LEAD LOOSE ON UPPER DETECTOR. INSTALLED NEW LOCK WASHER AND NUT. OPS CHECK OK.
DISCOVERED DEFECT DURING TOLERANCE INSPECTION. THE TIE BOLT SHOULDER UNDER SIZE BY .002 INCH, .440 INCH, BPS .4420 INCH TO .4425 INCH.
DURING TEAR DOWN INVESTIGATION OF UNIT, FOUND 2523240 BEARINGS FROZEN CAUSING FLYWEIGHTS TO STICK.
DAILY INSPECTION FOUND BLADE SEPARATED 9 INCHES FROM TIP AND 2 INCHES FROM LEADING EDGE ON CAMBER SIDE OF BLADE. SEPARATED AT FACE PLATE TRANSITION AREA. SEPARATION BEYOND LIMITS.
DURING PMI INSPECTION (50-HOUR INSPECTION) FOUND T/R PC LINK CORRODED. LINK SCRAPPED DUE TO CORROSION AT BEARING HOLE.
