WHILE PERFORMING AD 2009-23-01 (E)(3) NDT INSPECTION, INDICATION OF CRACKING WAS IDENTIFIED IN THE END CAP â SEE ATTACHED NDT REPORT. THIS INSPECTION WAS PERFORMED ON UC-93 AND THE ACTUATOR HAD 1015 CYCLES SINCE LAST INSPECTION. NO CRACKS WERE VISUALLY VISIBLE AND NO LEAKS WERE DETECTED.
STRONG SMELL OF SOMETHING BURNING SUB PANEL LIGHTS FLICKERING THEN COMPLETELY SHUT OFF. FOUND EEL PANEL A321 114-384007-15 SHORTING INTERNALLY.
CAPT AND FO LEFT BRAKE PEDAL UNRESPONSIVE.
PILOT REPORTED THAT THE RIGHT HAND (CO-PILOT'S) AIRSPEED INDICATOR WAS INTERMITTENT. AIRCRAFT RETURNED BACK TO ANCHORAGE FOR AN UNSCHEDULED LANDING, NO ISSUES WITH LANDING. MAINTENANCE PERSONAL REMOVED AND REPLACED THE RIGHT HAND (CO-PILOT'S) AIRSPEED INDICATOR, SYSTEM OPERATIONAL CHECK SUCCESSFUL, AIRCRAFT RETURNED TO SERVICE.
PILOT REPORTED WHILE TAXIING THE RIGHT HAND ENGINE HAD A HUNG START WHILE USING BATTERY POWER TO START THE ENGINE AND THAT THE ENGINE FAILED DURING THE TAXI, PILOT RETURNED TO HOME STATION. MAINTENANCE IS CURRENTLY TROUBLESHOOTING THE ISSUE BUT HAS FOUND THAT THE FUEL CONTROL UNIT IS THE REASON AND IS AWAITING POST ENGINE RUNS FOR OPERATIONAL CHECKS. REMOVED FUEL CONTROL TEST/TEARDOWN DISCLOSED CDP BELLOWS DISTORTED (STRETCHED) CAUSING LOW FUEL SCHEDULING.
ON ENGINE SHUTDOWN, THE PILOT NOTICED AN ELECTRICAL ODOR, ACCOMPANIED WITH SMOKE COMING FROM THE CENTER CONSOLE AUTO-PILOT CONTROL HEAD. MAINTENANCE CONFIRMED THE SMOKE WAS FROM AUTO-PILOT CONTROL HEAD BOX, AND REMOVED THE COMPONENT.
AIR STAIRS DOOR FRAME CRACKED ALONG BOTTOM AFT SECTION.
NOSE GEAR DOWNLOCK SWITCH FAILED FOR WATER INTRUSION.
RED HANDLE LIGHT ON, MAINS DOWN AND LOCKED NO NOSE GEAR GREEN LIGHT. GEAR WAS MANUALLY PUMPED DOWN TO GET 3 GREEN DOWN AND LOCKED LIGHTS. NOSE GEAR DOWN LOCK SWITCH RERIGGED.
AFTER TAKEOFF, CREW REPORTED SEEING & SMELLING ELEC SMOKE FROM BEHIND THE FUEL QTY PANEL. THE CREW DEPLOYED OXYGEN MASKS & MADE AN UNEVENTFUL EMER LANDING AT THE AIRPORT OF ORIGIN. THE CREW REPORTED THE SMOKE & ODOR HAD DISSIPATED UPON LANDING. NO SYS FAULTS OR FAILURES WERE OBSERVED. IDENTIFIED & CORRECTED CONTACT BETWEEN A POWER CABLE TERMINALS & AIRFRAME STRUCTURE BEHIND THE HINGED FUEL QTY PANEL & RETURNED THE ACFT TO SERVICE.
