FOUND A CREASED AREA AFT OF A RECENT REPAIR BETWEEN FRAMES 19 - 20 AND STRINGERS 11L AND 12L. MAINTENANCE REMOVED THE OLD DOUBLER AND CUT OUT THE DAMAGE AREA AND INSTALL A FILLER AND A NEW DOUBLER ENCOMPASSING ALL OF THE DAMAGE IAW ETD2018-145-019052.
DURING C-CHECK, INSPECTION FOUND THE LT AILERON PCA BRACKET INBD HAT BUSHING FOR INBD ATTACH POINT HAS NO INTERFERENCE FIT. MAINTENANCE R & R THE LT AILERON PCA BRACKET IAW AMM 27-10-00. ALL WORK ACCOMPLISHED AND WILL BE RELEASED AT THE END OF THE CHECK.
THE CREW REPORTED AT FL350 WITH THRUST SET FOR CRUISE FLIGHT, THEY EXPERIENCED TWO AUTO RELIGHT SEQUENCES WITH NO ASSOCIATED CAUTION AND WARNING MESSAGES BUT DID FEEL THE EFFECT OF THE ASSOCIATED POWER LOSS AND SUCCESSFUL RELIGHT. MAINTENANCE INSPECTED AND REPLACED THE NR 2 ENGINE PMA IAW MM 74-11-10. PERFORMED OPS GROUND RUNS WITH NO DEFECT NOTED. MAINTENANCE ALSO REPLACED THE NR 2 ENGINE FPMU AND THE NR 2B FADEC IAW POWERPLANT ENGINEERING.
CREW REPORTED A ODOR OF SMOKE IMMEDIATELY AFTER THE ACFT WAS POSSIBLY STRUCK BY LIGHTNING. MX INSPECTED ACFT, RAN AIR CONDITIONING PACKS, TAXIED, AND HAD ELECTRICAL POWER ON ACFT FOR HOURS WITH NO ODOR OF SMOKE OR LIGHTNING STRIKE DAMAGE NOTED. ACFT WAS APPROVED FOR RETURN TO SERVICE.
CREW REPORTED A STRONG ELECTRICAL ODOR IN COCKPIT AND CABIN AT FL220. MX PERFORMED FAULT ISOLATION TASK 21-20-00-810-801A AND 21-20-00-810-803A WITH NO DEFECTS NOTED, NO FAILED ELECTRICAL COMPONENTS OR FLUID LEAKS NOTED. RAN ENGINES, APU, AND AIR CONDITIONING PACKS WITH NO SMOKE NOTED. FOUND APU OIL LEVEL OVERSERVICED, SERVICE APU OIL LEVEL ACCORDING. ACFT WAS APPROVED FOR RETURN TO SERVICE.
MAIN DOOR EMERGENCY LIGHT AND MAIN ENTRANCE EMERGENCY LIGHT INOPERATIVE. REPAIRED SHORTED WIRE AT OVERHEAD LIGHT. OPS CHECK GOOD.
THE CREW REPORTED SMOKE IN THE COCKPIT & CABIN AFTER TAKE-OFF, ALONG WITH LAVATORY SMOKE MESSAGE WARNING. AIRCRAFT RETURNED TO DEPARTURE AND LANDED WITHOUT FURTHER INCIDENT. MX INSPECTED AND REPLACED THE NR 1 ENGINE. OPS CHECKED GOOD.
BOTTOM CENTER OF THE VENTRAL TANK WAS FOUND GOUGED. MX REPAIRED VENTRAL TANK BY INSTALLING A DOUBLER IAW EC 2810-01053. ALL WORK ACCOMPLISHED AND WILL BE RELEASED AT THE END OF THE CHECK
DURING ROUTINE MX, EMERGENCY LIGHT INOPERATIVE AT EXIT ROW 12 A, B, C. REPLACED NR 4 EMERGENCY LIGHT BATTERY. OPS CHECKED GOOD.
AFTER SHUTTING DOWN THE APU ON ARRIVAL CREW REPORTED AN ELECTRICAL BURNING ODOR. MX INSPECTED THE ACFT WITH NO DEFECTS NOTED, OPS TESTED APU WITH NO DEFECTS NOTED, THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
ACFT HAD GROUND DAMAGE AND WAS FERRIED FOR THE REPAIRS. THE FUSELAGE BETWEEN FR 65 TO FR 66 AND BETWEEN STR 17L AND 18L WERE DAMAGED. THE WORK WAS ACCOMPLISHED IAW EC5300-01025. ALSO THE TEMPORARY REPAIRS WERE COMPLIED WITH IAW THE REPAIR ETD2012-145-001697C. UPON COMPLETION OF ALL WORK THE ACFT WILL BE RELEASED TO SERVICE.
THE CREW REPORTED A PITCH TRIM BACK-UP FAIL MESSAGE IN FLIGHT. THE AIRCRAFT DIVERTED INTO CLE AND LANDED WITHOUT INCIDENT. MAINTENANCE REPLACED THE BACK-UP PITCH TRIM SWITCH.OPERATIONAL CHECKS WERE GOOD.
FLIGHT 3124 - EWR - FLIGHT CREW REPORTED L/G AIR GROUND FAIL INDICATION ON EICAS. EWR MX PERFORMED AIR GROUND SYSTEM RESET AND PERFORMED OPERATIONAL CHECK SUCCESSFULLY. ACFT WAS APPROVED TO RETURN TO SERVICE.
FLIGHT 4940 - EWR - FLIGHT CREW REPORTED FO AUDIO CONTROL PANEL GAVE OUT ELECTRICAL SMELL. EWR MX REMOVED AND REPLACED FO AUDIO CONTROL PANEL AND PERFORMED OPERATIONAL CHECK SUCCESSFULLY. ACFT WAS APPROVED TO RETURN TO SERVICE.

RIC - DURING ROUTINE MAINTENANCE, RIC MX FOUND BOTH AFT EXTERIOR EMERGENCY LIGHTS INOPERATIVE. MX REMOVED AND REPLACED BATTERY CHARGER NR 4 AND PERFORMED OPERATIONAL CHECK SUCCESSFULLY. AIRCRAFT WAS APPROVED TO RETURN TO SERVICE.
LT AND RT EMERGENCY AFT LIGHT INOP. R&R FUSE ON NR 4 EMERGENCY LIGHT BATTERY. OPS CHECK GOOD IAW EMB 145 VCMM 33-21-70. ACFT RELEASED AND RETURNED TO SERVICE.
DURING ROUTINE MX, RIC MX FOUND LT AND RT OVERWING EMERGENCY LIGHTS INOPERATIVE. MX R&R NR 4 BATTERY POWER SUPPLY AND PERFORMED OPS CHECK SUCCESSFULLY. ACFT WAS APPROVED TO RETURN TO SERVICE.
THE CREW REPORTED T/O YAW DAMPER ENGAGE RUDDER RT DEFLECTION. WHEN TRIM KNOB WAS ACTIVATED, EXTREME LT WAS OPERATED. THE ACFT RETURNED TO IAH WHERE IT LANDED WITHOUT INCIDENT. MAINT REMOVED AND REPLACED THE TRIM CONTROL MODULE, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
IAH - FLIGHT 2836, THE CREW REPORTED AN AIR GROUND FAIL MESSAGE ON EICAS AFTER T/O THAT REMAINED ON DURING THE ENTIRE FLIGHT. THE AIRCRAFT LANDED AT IAH WITHOUT INCIDENT WHERE MAINTENANCE INSPECTED THE AIRCRAFT, OPERATIONALLY TESTED THE AIR GROUND SYSTEM WITH NO DEFECTS NOTED, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE. MAINTENANCE REMOVED AND REPLACED THE NUMBER FOUR EMERGENCY LIGHT BATTERY PACK POWER SUPPLY, OPERATIONALLY CHECKED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH FLIGHT 2140 - THE CREW REPORTED A ELECTRICAL EMERGENCY ABNORMAL MESSAGE ON THE EICAS. THE CREW DIVERTED INTO CLE AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED AND AFTER RESET COULD NOT DUPLICATE. THE NR 1 AND NR 2 BUSS TIE CONTACTORS AND ESSENTIAL TIME RELAY AND POWER SWITCH WERE REPLACED FOR PRECAUTIONARY REASONS. OPERATIONAL CHECKS OF THE DC DISTRIBUTION SYSTEM WERE GOOD.
OVERWING EMERGENCY LIGHTS INOP. REMOVED AND REPLACED EMERGENCY LIGHT NR 4 BATTERY POWER SUPPLY IAW EMB 145XR AMM 33-50-04. OPS CHECK GOOD. AIRCRAFT RELEASED AND RETURNED TO SERVICE.
IAH - FLIGHT 2556 THE CREW REPORTED THAT THE OVERWING EMERGENCY EXIT LIGHTS ILLUMINATED IN FLIGHT WITH ALL OTHERS OFF, AND FINALLY EXTINGUISHED AFTER APPROXIMATELKY 20 MINUTES. THE AIRCRAFT LANDED AT IAH WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE AFFECTED EMERGENCY LIGHT BATTERY PACK, OPERATIONALLY CHECKED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.

EWR LOG BOOK SQUAWK MASSIVE FUEL LEAK. LOCATED LEAK ON APU FILTER. RED BYPASS BUTTON WAS LEAKING WHEN PUMP PRESSURE APPLIED. REMOVED AND REPLACED APU FILTER HOUSING. OPS AND LEAK CHECK GOOD IAW EMB145 MM 49-31-07.
FLIGHT 3499 FROM RDU TO IAH, CREW REPORTED THAT THERE WAS A BURNING SMELL IN THE COCKPIT. THE CREW ELECTED TO DIVERT AND LANDED AT HSV WITHOUT FURTHER INCIDENT. THE FAN WAS DEFFERED UNDER MEL 21-23-03. THE MEL WAS CLEARED ON 8/8/2003 IN EWR WHERE MAINTENANCE REMOVED AND REPLACED THE AFFECTED FAN. THE SYSTEM OPERATIONALLY CHECKED GOOD AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
