IAH - FLIGHT 4328, DURING MX INSPECTION THE LEFT HAND EXTERIOR FWD EMERGENCY LIGHT WAS FOUYND TO BE INOPERATIVE. MAINTENANCE REATTACHED CANNON PLUG P8561, OPERATIONALLY TESTED WITH NO DEFECTS AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
THE CREW REPORTED BAD VIBRATION IN THE YOKE AT FL360 AND .70 MACH. SLOWED AIRCRAFT TO .65 MACH VIBRATION DID NOT GO AWAY. THE AIRCRAFT DIVERTED AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED AND REPLACED ELEVATOR AUTOPILOT SERVO.OPERATIONAL CHECKS WERE GOOD
FLIGHT 4324, THE CREW REPORTED A BURNT WIRE SMELL FROM THE CD AREA IN THE GALLEY. THE AIRCRAFT LANDED IN MFE WITHOUT INCIDENT WHERE MAINTENANCE DEACTIVATED THE CD PLAYER. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE. MAINTENANCE REMOVED AND REPLACED THE CD PLAYER, OPERATIONALLY TESTED WITH NO DEFECTS OR SMELLS NOTED. THE MEL WAS REMOVED AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH - DURING MAINTENANCE INSPECTION THE BELLY SKIN WAS FOUND TO BE CORRODED UNDER THE AFT RADIO ALTIMETER BASE. MAINTENANCE REMOVED THE CORROSION, CUT OUT THE DAMAGED SKIN AND PERFORMED A FILLER / DOUBLER REPAIR IN ACCORDANCE WITH TECHNICAL DISPOSITION ETD2019-145-062532, PRESSURIZATION AND OPS CHECKS WERE SATISFACTORY AND THE AIRCRAFT WAS APPROVED OFR RETURN TO SERVICE.
THE CREW REPORTED A LAVATORY SMOKE WARNING, DURING APPROACH AT 3000FEET. THE AIRCRAFT LANDED WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE LAVATORY SMOKE DETECTOR, OPS TESTED WITH NO DEFECTS NOTED, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
PILOT REPORTED THE MAIN CABIN DOOR HARD TO OPEN. MX ADJUSTED THE DOOR TO FUSELAGE GAP AND OPS CHECKED GOOD. ACFT APPROVED FOR RETURN TO SERVICE.

IAH - FLIGHT 2020, DURING CLIMB THE CREW REPORTED A PACK 1 VALVE CLOSED MESSAGE ON EICAS. THE AIRCRAFT RETUIRNED TO IAH WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED SLEEVE ON THE PACK ONE DUAL TEMP CONTROL VALVE, OPERATIONALLY AND LEAK CHECKED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
FLIGHT 3006, DURING PRE-FLIGHT INSPECTION THE BAGGAGE SMOKE DETECTOR WOULD NOT RESET AFTER TESTING. MAINTENANCE REMOVED AND REPLACED THE FORWARD BAGGAGE COMPARTMENT SMOKE DETECTOR, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH - FLIGHT 2147, THE CREW REPORTED A BAGGAGE SMOKE INDICATION ON EICAS AND OVERHEAD LIGHT WHILE ON THE GROUND. THE CREW VERIFIED NO SMOKE PRESENT, RESET THE SMOKE DETECTION SYSTEM, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED OFR RETURN TO SERVICE.
TYS - A/C WAS DEPARTING CLT WHEN THE BAGGAGE COMPARTMENT LIGHT CAME ON. THE CREW RESET THE SYSTEM AND NO FURTHER ACTION WAS REQUIRED.
CLE FLT 2678 THE CREW REPORTED PACK NR 1 QUIT ON ITS OWN FOLLOWED BY A ELECTRICAL SMELL. MAINTENANCE INSPECTED AND PERFORMED OPERATIONAL CHECKS OF PACK 1. NO DEFECTS WERE FOUND. THE AIRCRAFT WAS RELEASED FOR SERVICE.
IAH - FLIGHT 2758 THE CREW REPORTED THAT THE NUMBER TWO ENGINE FIRE DETECTOR FAIL MESSAGE CAME ON INTERMITTENTLY DURING FLIGHT. THE AIRCRAFT LANDED AT IAH WITHOUT INCIDENT WHERE MAINTENANCE CLEANED AND RE-SECURED THE CONNECTOR ON THE ENGINE ONE FIRE LOOP, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.

RIC MX - DURING ROUTINE MAINTENANCE, RIC MX FOUND LT AND RT OVER THE WING EMERGENCY LIGHTS INOPERATIVE. MX REMOVED AND REPLACED NR 4 EMERGENCY CHARGER ASSY AND PERFORMED OPERATIONAL CHECK SUCCESSFULLY. ACFT WAS APPROVED TO RETURN TO SERVICE.
IAH-FLIGHT 3044 - THE CREW REPORTED THAT THE NOSE GEAR DOORS WOULD NOT CLOSE. THE ACFT LANDED AT IAH WITHOUT INCIDENT WHERE MAINTENANCE REMOVED AND REPLACED THE NOSE LANDING GEAR DOOR SOLENOID VALVE. OPERATIONALLY TESTED WITH NO DEFECTS, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE. (S)

FLIGHT 2023 - AFTER TAKEOFF, THE CREW NOTICED A FUEL SMELL WITH THE APU RUNNING THE AIR CONDITIONING PACKS. MAINTENANCE PERFORMED SYSTEM CHECKS OF THE APU BLEED SYSTEM AND AIR CONDITIONING PACKS. THE DISCREPANCY COULD NOT BE DUPLICATED AND THE DISCREPANCY WAS CLEARED.
AUS - FLIGHT 2081, THE CREW REPORTED AN UNCOMMANDED NR 1 ENGINE SHUTDOWN IN CRUISE. THE AIRCRAFT DIVERTED TO AUS WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE AFFECTED ENGINE, OPERATIONALLY CHECKED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE. *SDR FILED OPEN AWAITING DETERMINATION OF CAUSE*.

DURING SCHEDULED MAINTENANCE THE LEFT EMERGENCY EXIT LOWER EMERGENCY LIGHT WAS FOUND TO BE INOPERATIVE. MAINTENANCE REMOVED AND REPLACED AN EMERGENCY LIGHT POWER INVERTER, OPERATIONALLY TESTED THE LIGHT AND CLEARED THE DISCREPANCY.
IAH - FLIGHT 2239 THE CREW REPORTED A LANDING GEAR AIR/GND FAILURE INDICATION AFTER T/O. THE AIRCRAFT RETURNED TO IAH WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED THE AIRCRAFT AND FOUND A CHAFED WIRE ON THE RT TRAILING ARM WEIGHT IN WHEELS PROX SWITCH. THE WIRE WAS REPAIRED IN ACCORDANCE WITH EMB-145 WIRING MANUAL, OPS CHECKED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
CLE FLIGHT 2169 THE CREW REPORTED THAT THE BACK-UP TRIM WAS INOP IN FLIGHT. RETURNED TO CLE AND LANDED WITHOUT INCIDENT. MAINTENANCE REPLACED THE HORIZONTAL STABILIZER CONTROL UNIT. ALL OPERATIONAL CHECKS WERE GOOD. AIRCRAFT RETURNED TO SERVICE.
3/25/03 EXPRESS JET AIRLINES WAS NOTIFIED BY ROLLS ROYCE THAT AIRCRAFT 11109, NR 2 ENGINE HAD THE WRONG P/N COMBUSTION LINER INSTALLED AT THE FACTORY. THE AIRCRAFT WAS FERRIED TO CLEVELAND FOR A ENGINE CHANGE. MAINTENANCE REPLACED THE NR 2 ENGINE.OPERATIONAL CHECKS WERE GOOD. THE AIRCRAFT WAS RELEASED FOR SERVICE.
THE CREW NOTED THAT THE LANDING GEAR WOULD NOT COME UP. AFTER USING QRH, GEAR CAME UP. MAINTENANCE REMOVED AND REPLACED THE LANDING GEAR CONTROL LEVER. PERFORMED A OPERATIONAL CHECK, NO DEFECTS NOTED. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
