FLIGHT 4087, DURING MAINTENANCE INSPECTION THE EMERGENCY LIGHTS AT THE FA SEAT AND EMERGENCY EXIT WERE FOUND TO BE INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE AFFECTED EMERGENCY LIGHT BATTERY PACKS, OPERATIONALLY TESTED WITH NO DEFECTS AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
XNA - FLIGHT 3951 THE CREW REPORTED AN ELECTRICAL SMELL IN THE COCKPIT ONLY WITH PACK ONE (1) ON, AND THAT THE SMELL DISSIPATED WHEN PACK ONE (1 WAS TURNED OFF. THE AIRCRAFT WAS INSPECTED AND PACK ONE (1) WAS PLACED ON MEL 21-51-00-3. ON 04/04/20 THE AIRCRAFT LANDED IN IAH WHERE MAINTENANCE REMOVED AND REPLACED THE #1 ACM, OPERATIONALLY TESTED WITH NO DEFECTS, REMOVED THE MEL, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
PILOT REPORTED "LAV SMOKE" WARNING ON EICAS. MX PERFORMED INSPECTION ON LAV, NO SMOKE SMELL NOTED PLUS PERFORMED TEST ON LAV SMOKE DETECTION SYSTEM WITH OPERATIONAL CHECK GOOD. A/C WAS APPROVED FOR RETURN TO SERVICE.
TYS A/C IS IN C-CHECK WHEN INSPECTION FOUND A DENT IN THE LEFT LEADING EDGE NR 2 LOCATED ON THE NOSE OF THE LEADING EDGE. MAINTENANCE REPAIRED THE LT LEADING EDGE NR 2 IAW SRM 57-40-00. ALL WORK ACCOMPLISHED AND WILL BE RELEASED AT THE END OF THE CHECK.
BURNING ODOR REPORTED IN THE CABIN. MX PERFORMED A CABIN GENERAL VISUAL INSPECTION AND FOUND CABIN CONTROL MODULE AT SEAT 1A, FWD BALLAST OVERHEATING, AND THE SIDEWALL LIGHT BURNED OUT. REMOVED AND REPLACED THE LIGHT MODULE, BALLAST, AND SIDEWALL LIGHT, OPS CHECKED GOOD. ACFT WAS APPROVED FOR RETURN TO SERVICE.
THE CREW REPORTED THAT THE AISLE EMERGENCY LIGHT STRIP, 1ST ON RIGHT, WAS INOPERATIVE. REMOVED AND REPLACED THE EMERGENCY LIGHT STRIP, OPS TESTED WITH NO DEFECTS. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
DURING FLIGHT, THE CREW REPORTED AN ODOR OF SMOKE IN THE COCKPIT WITH THE LOSS OF THE CAPTAINS PFD, MFD, AND EICAS SCREEN. THE AIRCRAFT RETURNED TO DEPARTURE, AND LANDED WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE LEFT IC-600, OPERATIONALLY TESTED WITH NO DEFECTS NOTED, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH - THE CREW REPORTED THAT THE GALLEY EMERGENCY LIGHT ABOVE THE SERVICE DOOR WAS NOT HOLDING A CHARGE. MAINTENANCE RESET THE EMERGENCY LIGHT SYSTEM, OPERATIONALLY TESTED WITH NO DEFECTS NOTED AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
DURING CREW INSPECTION THE CABIN SERVICE DOOR AND FORWARD BULKHEAD, EMERGENCY LIGHTS WERE FOUND TO BE INOPERATIVE. MAINTENANCE RESET THE AFFECTED EMERGENCY LIGHT BALLAST, OPS TESTED THE EMERGENCY LIGHTS WITH NO DEFECT NOTED,AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK, SERVICE DOOR FORWARD SILL HAS CORROSION. R & R NEW SERVICE DOOR FORWARD SILL IAW SRM 51-40-02. ACFT APPROVED FOR RETURN TO SERVICE.
THE CREW REPORTED A W/S 1 HEAT FAIL MESSAGE WITH A STRONG ODOR OF BURNING PLASTIC. THE ACFT LANDED WITHOUT INCIDENT, WHERE MAINTENANCE INSPECTED THE ACFT AND R & R THE NR 1 W/S TEMP CONTROLLER, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE. DURING GROUND OPS FOR FLIGHT 5912, THE CREW AGAIN REPORTED A W/S 1 HEAT FAIL MESSAGE ASSOCIATED WITH A BURNING ODOR. MAINTENANCE INSPECTED THE ACFT AND FOUND CONTACTOR K0001 TO BE NOT DISENGAGING. THE CONTACTOR AND W/S 1 TEMP CONTROLLERS WERE R & R, OPERATIONALLY TESTED WITH NO DEFECTS NOTED, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.

THE CREW WAS NOTIFIED OF FUEL STREAMING FROM THE BOTTOM OTBD RT WING. THE ACFT RETURNED TO DEPARTURE WHERE IT LANDED WITHOUT INCIDENT. MX INSPECTED THE ACFT, AND R & R THE OTBD FLAPPER VALVE, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING MX INSPECTION, THE AFT OVERWING EMERGENCY EXIT SIGN WAS FOUND NOT TO ILLUMINATE. MX R & R THE NR 4 INVERTER WITH OPS CHECKED SUCCESSFULLY. RETURNED TO SERVICE.

IAH - DURING MAINTENANCE INSPECTION, THE RIGHT HAND OVERWING EMERGENCY EXIT LIGHT WAS FOUND TO BE INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE AFFECTED LIGHT BULB, OPERATIONALLY CHECKED WITH NO DEFECTS NOTED, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
