DURING 100 HOUR INSPECTION FOUND CRACK IN WEB OF AFT MLG TRUNNION ASSY AIRCRAFT. (K)
FOUND CRACKS EMINATING FROM LAST RIVET ON OUTBOARD UPPER ENGINE SUPPORT BRACKET. IT APPEARS THAT THIS RIVET WAS SMASHED HARDER THAN NECESSARY DURING MANUFACTURE.
RIGHT ENGINE SHUT DOWN IN FLIGHT DUE TO WARNING OF LOW OIL PRESSURE. FOUND OIL COOLER INLET NOSE FAILED AT BEND RADIUS. SUSPECT HOSE WAS ROUTED INCORRECTLY DURING A RECENT ENGINE INSTALLATION.
FLAP TRANSMISSION INSTALLED ON PIPER PA31T. EXCESSIVE JACK SCREW WEAR EXCEEDED LIMITS IN 60-DAY PERIOD. PART WAS ORIGINALLY REBUILT BY MANUFACTURER. PART RETURNED TO MANUFACTURER FOR EVALUATION. TOTAL TIME ON PART, 99.6 HOURS, APPROXIMATELY 70 CYCLES.
HEATER VENTING FUEL OVERBOARD FROM COMBUSTION CHAMBER DRAIN CAUSING FLAME STREAK THE LENGTH OF THE LOWER FUSELAGE. FUEL DRAIN IS LOCATED IN LINE AND 3 INCHES AHEAD OF HEATER EXHAUST. RECOMMEND AIR CREWS LOOK FOR THIS CONDITION DURING PRE-FLIGHT.
LEFT ENGINE QUIT ON TAKEOFF. ENGINE SUBJECT TO EVALUATION/CAUSE OF FAILURE BY MANUFACTURER.
