ON GROUND ROLL ONTO RAMP, CREW REPORTED DETECTING AN ELECTRICAL SMOKE AND SAW SMOKE EMITTING FROM BEHIND LT SIDE OF INSTRUMENT PANEL. CREW TURNED OFF BATTERY POWER AND PARKED ON RAMP. NO FURTHER ACTIONS REQUIRED. MX FOUND OVERHEAT EVIDENCE ON THE ANNUNCIATOR DIMMER SWITCH/POTENTIOMETER DIMMER WIRES (TOP OF SWITCH HAD EVIDENCE OF BUILD UP OF DUST AND FUZZ THAT HAD SMOLDERED) AND THE POTENTIOMETER FROZEN. SWITCH ASSY WAS REPLACED AND SYS DEFERRED PENDING ADDITIONAL TROUBLE SHOOTING AND SYS INSP.
PILOT REPORTED THAT THE COPILOT WINDSHIELD CRACKED ON DEPARTURE. ACFT RETURNED AND MADE AN UNEVENTFUL LANDING AT THE AIRPORT OF DEPARTURE. MX INSPECTED CRACKED OUTER PANE OF COPILOTS WINDSHIELD AND ELECTED TO REPLACE WINDSHIELD (OPS COULD HAVE BEEN CONTINUED WITH RESTRICTIONS IAW POH).
PILOT COULD NOT OPEN THE FORWARD PASSENGER/CREW DOOR FROM THE INSIDE, HANDLE WAS BROKEN (DOOR SEEMED FROZEN).
PILOT REPORTED IT TOOK 3 ATTEMPTS TO GET THE LANDING GEAR MOTOR TO OPERATE. THEN IT WORKED FINE. MX VERIFIED THAT FREEZING THE HYDRAULIC POWER PACK RELAY WOULD CAUSE THIS PROBLEM. REPLACED THE HYDRAULIC POWER PACK RELAY.
DURING A PILOT TRAINING FLT MLG WAS EXTENDED USING ALTERNATE EXTENSION PUMP IAW QRH AND POH. AFTER EXERCISE, PILOT RESET MLG CONTROL CIRCUIT BREAKER & SELECTED GEAR UP. PILOT REPORTED THAT ALTHOUGH THEY COULD HEAR MLG MOTOR/PUMP RUNNING, MLG DID NOT RETRACT, ALL DOWN & LOCKED GREEN LIGHTS REMAINED ON, & CONTROL CIRCUIT BREAKER TRIPPED AFTER 16 SECONDS (MOTOR CONTROL CIRCUIT PROTECTION TIME DELAY). ACFT LANDED UNEVENTFULLY. MX COULD NOT DUPLICATE FAULT EVEN AFTER ATTEMPTING TO RECREATE EXACT ORIGINAL SCENARIO - UNLESS HAND PUMP HANDLE WAS NOT STOWED. ACFT RELEASED FOR SERVICE. WHILE ACFT WAS BEING LOADED FLT CREW NOTICED THAT MLG SELECTOR HANDLE SAFETY LATCH WAS DISENGAGING AS SOON AS SHIP POWER TURNED ON. MAX DETERMINED THERE WAS AN INTERMITTENT INTERNAL FAILURE OF RT MLG SAFETY (SQUAT) SWITCH & REPLACED SAME. (S)
NOSE TIRE LOOSING AIR PRIOR TO DEPARTURE. REPLACED NOSE TIRE ASSY (FOUND INNER TUBE SPLIT ON CENTER SEAM .1250 INCH). THIS IS AN ADDITIONAL FAILURE OF A MFG INNER TUBE. THIS WAS A CANADIAN MANUFACTURED TUBE AND THE DEFECT WAS AT THE CENTER SEAM.
FUEL DRIPPING FROM NR 1 ENGINE, GANG DRAIN. REPLACED NR 1 LOW PSI FUEL PUMP. DISASSEMBLY OF PUMP REVEALED A RUPTURED DIAPHRAGM, PN RA5326 WITH 1165 TSI.
MANUAL GEAR EXTENSION DUE NO GREEN LIGHTS AFTER HANDLE DOWN. CONFIRMED HYDRAULIC MOTOR RELAY LOGGED WITH WATER AND DUPLICATED FAILURE BY FREEZING RELAY, REPLACED RELAY.
RT FUEL PRESSURE ILLUMINATED WHEN RT STANDBY PUMP NOT ON, REPLACED FAILED LOW PSI PUMP DUE FUEL LEAKING FROM DIAPHRAGM.
PILOT REPORTED FUEL LEAKING FROM NR 1 ENGINE DRAIN LINE. REPLACED LOW PSI PUMP DUE DIAPHRAGM RUPTURE (SUSPECTED CAUSE).
PILOT REPORTED NR 2 ENGINE LOW FUEL PRESSURE WARNING CAME ON. WARNING EXTINGUISHED AFTER SELECTING ELECTRIC STANDBY PUMP ON. MAINTENANCE REPLACED LOW PSI FUEL PUMP DUE INTERNALLY DISCONNECTED DRIVE SHAFT (SECOND OCCURRENCE OF THIS TYPE FAILURE ON THIS ENGINE WITHIN 30 DAYS). VERIFIED PUMP DRIVE SHAFT WOULD EASILY DE-COUPLE FROM ROTOR WHEN TURNING SHAFT BY HAND.
PILOT REPORTED FLAPS WOULD NOT RETRACT AFTER LANDING, VERIFIED FAILED LT IB FLAP ACTUATOR, 1116 CSOH.
PILOT REPORTED FUEL LEAK IN RT ENGINE. MAINTENANCE REPLACED LOW PSI FUEL PUMP DUE FUEL LEAKING FROM RELIEF VALVE AREA, DIAPHRAGM FAILURE VERIFIED (3Q05 DATED DIAPHRAGM). THIS WAS A NEW (NEW STYLE) PUMP.
PILOT REPORTED NR 1 ENGINE WOULD NOT ACCELERATE ABOVE 35% ON START - REPLACED DEFECTIVE FUEL CONTROL UNIT.
PIREP - CREW SMELLED/SAW SMOKE IN COCKPIT, NR 1 ENGINE BEGAN TO SURGE, NR 1 ENGINE OIL PRESSURE WARNING LIGHT CAME ON. CREW SHUT DOWN NR 1 ENGINE. MAINTENANCE VERIFIED NR 1 ENGINE GAS GENERATOR INTERNAL FAILURE.
PIREP - NR 1 ENGINE LEAKING FUEL AT DRAIN LINE. REPLACED FAILED (DIAPHRAGM RUPTURED) LOW PSI BOOST PUMP. THIS IS A NEW SYTLE PUMP.
PILOT REPORTED LANDING GEAR CONTROL CIRCUIT BREAKER TRIPPED TWICE, GEAR WAS MANUALLY EXTENDED AT DESTINATION, UNEVENTFUL LANDING MADE. MAINTENANCE C/N/D ON GROUND, NO FAULTS DETECTED.
TAIL FLOOD WHEN TURNED ON, CAN SMELL ELECTICAL SMOKE. THEN SWITCH (FLOOD LIGHTS) FLIPS OFF. INSPECTED WIRING AT FLOOD LIGHT SWITCH AND FOUND OVERHEATED WIRE AT LOOSE TERMINAL ON CIRCUIT BREAKER SWITCH.
PIREP - HAD TO EXTEND LANDING GEAR MANUALLY. MAINTENANCE REPLACED "WATER LOGGED" HYDRAULIC POWER PACK RELAY.
PIREP - LANDING GEAR FAILED TO EXTEND USING NORMAL SYSTEM. MANUAL EXTENSION PERFORMED. MAINTENANCE REPLACED WATER LOGGED HYDRAULIC POWER PACK RELAY (SUSPECT THAT RELAY WAS FROZEN AFTER FLIGHT AT ALTITUDE). SEALED WING LEADING EDGE GAP TO HELP REDUCE FREE WATER / MOISTURE IN ELECTRICAL BAY.
PIREP - LANDING GEAR HANDLE HAD TO BE CYCLED SEVERAL TIMES BEFORE GEAR WOULD EXTEND. MAINTENANCE COULD NOT DUPLICATE. REPLACED HYDRAULIC PUMP MOTOR RELAY FOR T/S (COULD NOT VERIFY RELAY FAILURE).
PIREP - RIGHT T-HANDLE LIGHT COMES ON INTERMITTENTLY. MAINTENANCE VERIFIED "OPEN" RIGHT ENGINE FORWARD FIRE LOOP.
PIREP - LEFT LANDING LIGHT CIRCUIT BREAKER POPS OFF APPROXIMATELY 5 MINUTES AFTER LIGHT TURNED ON, ELECTRICAL BURNING SMELL NOTED IN FLIGHT DECK. MAINTENANCE DEFERRED THE DEACTIVATED CIRCUIT. MAINTENANCE WAS UNABLE TO DUPLICATE FAILURE, FOUND EVIDENCE OF HEATED WIRES AT BUSS SIDE OF CIRCUIT BREAKER SWITCH AND SWITCH EXHIBITED EVIDENCE OF HEATING (SUSPECT HIGH RESISTANCE FROM LOOSE TERMINAL). REPLACED CIRCUIT BREAKER SWITCH AND RE-TERMINATED AFFECTED WIRES, CLEARED DEFERRAL.
PILOT REPORTED NOSE GEAR POSITION INDICATED UNSAFE WITH GEAR SELECTED DOWN. CYCLED GEAR AND PERFORMED GROUND VISUAL CHECK PRIOR TO UNEVENTFUL LANDING. MAINTENANCE CONFIRMED FAILED NOSE GEAR DOWNLOCK SWITCH AND REPLACED SAME.
PILOT REPORTED RIGHT HAND ENGINE FUEL FEED ANNUNCIATOR ON INTERMITTENTLY AFTER TAKEOFF. RETURNED FOR UNEVENTFUL LANDING. MAINTENANCE COULD NOT DUPLICATE THE PROBLEM. SYSTEM DEFERRED FOR ADDITIONAL REPORTS.
PILOT REPORTED THAT INSIDE DOOR HANDLE ON AIRSTAIR DOOR WAS SHEARED. MAINTENANCE REPLACED HANDLE ASSY.
PILOT REPORTED USING LANDING GEAR EMERGENCY PROCEDURE DUE CONTINUED GEAR UNSAFE WARNING EVEN WITH ALL GREEN DOWN AND LOCKED LIGHTS ON. UNEVENTFUL LANDING COMPLETED. MAINTENANCE LOCATED AND CORRECTED A LOOSE POWER LEVER POSITION SWITCH.
ON OPERATION CHECK FLIGHT AFTER OVERHAULED ENGINE INSTALLATION, PILOT REPORTED THAT THE POWER LEVER GOT HUNG UP ON POWER REDUCTION. AN UNEVENTFUL LANDING WAS COMPLETED AND MAINTENANCE LOCATED AND CORRECTED INCORRECT (FOR THIS MODEL ENGINE) PROPELLER REVERSING HARDWARE INSTALLATION. A BOLT & SPACER (PWC PT6A-65 IPC 76-10-00, ITEMS 261 BOLT & 265 SPACER) WERE INSTALLED DURING OVERHAUL AND MIGHT HAVE CAUSED AN ISSUE IN FLIGHT.
PILOT REPORTED FUEL LEAKING FROM LT ENGINE OB LOWER DRAIN. REPLACED LOW PSI FUEL PUMP DUE FAILED PRESSURE RELIEF DIAPHRAGM.
PILOT REPORTED LANDING GEAR DID NOT EXTEND WHEN HANDLE WAS SELECTED DOWN. ALTERNATE HAND PUMP EXTENSION PERFORMED, FERRY WITH GEAR DOWN FOR REPLACEMENT OF MS24171D1 POWER PACK RELAY (CONTAMINATED WITH WATER).
PILOT REPORTED LANDING GEAR DID NOT EXTEND WHEN HANDLE WAS SELECTED DOWN. ALTERNATE (HAND PUMP) EXTENSION PERFORMED. REPLACED HYDRAULIC MOTOR RELAY (CONTAMINATED WITH WATER).
PILOT REPORTED FUEL LEAKING FROM ENGINE DRAIN. MAINTENANCE REPLACED LOW PSI FUEL PUMP.
PILOT REPORTED NR 2 ENGINE TORQUE STARTED TO SURGE (1500 LBLS) AND WAS SHUT DOWN PER THE QRH. MAINTENANCE REPLACED THE NR 2 ENGINE CONSTANT SPEED GOVERNOR AND OVERSPEED GOVERNOR FOR TROUBLESHOOTING (THE REPORTED PROBLEM COULD NOT BE DUPLICATED ON THE GROUND).
NR 2 ENGINE STARTED TO SURGE AND THEN LOST POWER - PILOTS SECURED THE ENGINE AND MADE AN UNEVENTFUL LANDING AT THE AIRPORT OF ORIGIN. REPLACED FCU.
PILOT REPORTED THAT IT TOOK MULTIPLE ATTEMPTS TO EXTEND THE GEAR WITH THE ELECTRO/HYDRAULIC SYSTEM. GEAR DID EXTEND AND UNEVENTFUL LANDING ACCOMPLISHED IN ANC. MAINTENANCE REPLACED THE HYD. POWER PACK RELAY (HEAVY CONDENSATION).
RIGHT ENGINE FIRE LIGHT CAME ON IN FLIGHT REQUIRING ENGINE SHUTDOWN. PILOT REPORTED THAT WARNING DISCONTINUED 15 MINUETS AFTER ENGINE SHUTDOWN. SYSTEM OPS CHECKED GOOD ON GROUND AFTER LANDING. MAINTENANCE INSPECTED WARNING SYSTEM AND REPLACED FORWARD LOOP DUE TO A TIGHT BEND IN THE LOOP, REPLACED THE AFT LOOP DUE LOW RESISTANCE AND REPAIRED A CHAFE AREA ON THE WIRING HARNESS.
LANDING GEAR FAILED TO EXTEND USING GEAR SELECTOR - HAND PUMPED GEAR DOWN.
