THE LEFT BLEED AIR FAILURE WARNING LIGHT CAME ON IN CRUISE. THE SMELL OF SMOKE, POSSIBLY FROM ELECTRICAL INSULATION, WAS ALSO PRESENT IN THE COCKPIT. WE MOVED THE LEFT BLEED AIR SWITCH TO INSTRUMENT AND ENVIRO OFF. AFTER CONSULTING 21-1-3 IN THE MEL WE DETERMINED THAT WE WOULD NOT BE ABLE TO DEFER THE BLEED AIR WARNING LIGHT IF WE CONTINUED TO KRWL AND LANDED THERE BECAUSE WE COULD NOT CONFIRM ALL GYRO INSTRUMENTS EXCEPT FOR THE COPILOTâS RATE OF TURN INDICATOR WEâRE ELECTRICALLY DRIVEN AND ALSO BECAUSE WE WERE FLYING THROUGH ICING CONDITIONS WHEN THE ANOMALY WITH THE LIGHT OCCURRED AND EXPECTED TO FLY BACK THROUGH THOSE SAME ICING CONDITIONS ON THE WAY BACK TO KDEN. KRWL HAS NO MAINTENANCE FACILITIES, NOR DOES IT HAVE AN A&P AT THE AIRPORT, . WE DIVERTED BACK TO KDEN. AFTER WE LANDED AT KDEN, THE MECHANIC TOOK A LOOK UNDER THE FLOORBOARDS AND TOLD US THAT ONE OF THE CONNECTIONS IN THE BLEED AIR LINES FROM THE LEFT ENGINE APPEARED TO HAVE PARTIALLY COME APART.
ON TAKE OFF ROLL, L ENG FIRE WARNING ANNUNCIATOR ILLUMINATED. FOUND INTERNAL RESISTANCE OUT OF SPEC.
NR 1 WHEEL SEPARATED FROM AIRCRAFT AFTER TAKEOFF. THE INVESTIGATION FOUND WHEEL BEARING FAILURE AS THE CAUSE FOR SEPARATION.
DURING A MAINTENANCE INSPECTION OF THE FUSELAGE EXTENSIVE CORROSION WAS DISCOVERED IN THE LOWER FUSELAGE SKIN ASSEMBLY. THE CONDITION WAS CORRECTED BY THE INSTALLATION OF NEW FUSELAGE SKIN PANELS.
DURING CLIMB OUT, PILOT DETECTED FUMES COMING FROM THE CARGO AREA. THERE WAS NO VISIBLE SMOKE, ONLY A BURNING ODOR. THE FUMES CONTINUED UNTIL CABIN WAS DEPRESSURIZED. WHEN THE CABIN WAS RE-PRESSURIZED THE FUMES DID NOT RETURN. AIRCRAFT LANDED UNEVENTFULLY. CARGO WAS INSPECTED DURING OFF LOAD FOR LEAKS, DAMAGE AND UNUSUAL ODORS. NONE FOUND. SUBSEQUENT MAINTENANCE INSPECTION WAS UNABLE TO LOCATE OR DETERMINE THE SOURCE OR CAUSE OF THE FUMES.
RT ENGINE SLOWLY LOST TORQUE DURING CRUISE AT FL 200. ENGINE TORQUE EVENTUALLY FELL TO 0 AND ENGINE WAS SHUTDOWN. ACFT DIVERTED AND LANDED UNEVENTFULLY. PROBLEM WAS DETERMINED TO BE A FAULTY ENGINE FUEL CONTROL UNIT.
AIRCRAFT WAS ON JACKS. WHILE CYCLING GEAR A LOUD POP WAS HEARD FROM THE NOSE LANDING GEAR ACTUATOR. EXAMINATION SHOWED A CRACK IN THE ACTUATOR END CAP AND HYDRAULIC FLUID LEAKAGE.
MAINTENANCE FOUND CORROSION ON FORWARD SPAR LOWER CAP FORWARD FLANGE, IN THE CENTER FUSELAGE BAY, UNDER PREVIOUSLY REPLACED SEALANT. REPAIR REQUIRED REMOVAL OF CORROSION AND STRAP DOUBLER PER FR-CO FACTORY REPAIR DESIGN.
PILOT REPORTED RT GEN OFF LINE AND NO ENGINE STARTER AFTER LANDING - REPLACED STARTER/GENERATOR DUE FRONT BEARING FAILURE
NR 2 ENGINE LEAKING FUEL FROM NACELLE. MAINTENANCE REPLACED LOW PSI FUEL PUMP DUE EXTRUDED VALVE BODY GASKET. FUEL WAS NOT DUMPED OVERBOARD AS THE VALVE BODY WAS LEAKING EXTERNALLY.
PIREP - FUEL OBSERVED DRIPPING FROM NR 1 ENGINE DRAIN MAST. VERIFIED LOW PSI FUEL PUMP DIAPHRAGM RUPTURE, THIS UNIT WAS IN COMPLIANCE WITH THE LATEST SB FOR DIAPHRAGM DESIGN.
PIREP - FUEL LEAKING FROM NR 1 ENGINE GANG DRAIN. REPLACED LOW PSI DUEL PUMP DUE RUPTURED DIAPHRAGM.
FOUND CRACKED BELLY SKIN WHILE PERFORMING TURBULENT AIR INSPECTION. CORROSION POSSIBLY ORIGINATING FROM OEM INSTALLATION OF UNPRIMED SHIM STOCK AT STRINGER JOINTS.
GEAR WARNING REMAINED ON AFTER GEAR INDICATED THREE GREEN. PILOT DID A GO-AROUND AND ON NEXT GEAR EXTENSION, ALL INDICATIONS NORMAL. UNEVENTFUL LANDING. MAINTENANCE REPLACED NOSE GEAR UPLOCK SWITCH.
DURING CAMP INSPECTION, MAINTENANCE LOCATED CORROSION ON SEAT TRACK FLOOR PANEL FLANGES. SEAT TRACK AND ADJACENT LOCALIZED CORROSION SUSPECTED TO HAVE BEEN CAUSED BY CARGO SPILLAGE. SEAT TRACK REPLACED, OTHER AREAS REPAIRED.
PILOT REPORTED, AFTER 3 GEAR EXTENSION CYCLES AND THE QRH MANUAL EXTENSION PROCEDURE, NOSE GEAR GREEN LIGHTS WOULD NOT ILLUMINATE. MAINTENANCE COULD NOT DUPLICATE (SUSPECT INTERMITTENT SWITCHES) AND REPLACED NOSE GEAR ACTUATOR.
UNUSUAL ODOR EMANATING FROM AIR DUCTS WHEN BLEED AIR SYSTEM TURNED ON. RETURNED TO AIRPORT FOR INVESTIGATION, MAINTENANCE RECONNECTED MAIN HEATER DUCT IN BELLY (BELLY HAD CARGO SPILL RESIDUE IN AREA).
WHILE REPAIRING A FUEL LEAK IN THE RIGHT WING MAIN FUEL TANK, FOUND CORROSION ON THE MAIN SPAR LOWER CAP FORWARD FLANGE REQUIRING MAJOR REPAIR. THIS AREA WAS IDENTIFIED AS A SUSPECT CORROSION LOCATION IN RAYTHEON COMMUNIQUÃ NR 71.
PIREP - COCKPIT DOOR HANDLE DOES NOT WORK FROM OUTSIDE. REPLACED SHEARED DOOR HANDLE ROLL PIN.
FOUND DIAPHRAGM LEAKING (RUPTURED) - THIS PUMP WAS BEING INSPECTED DUE TO AN ABORTED TAKEOFF AFTER ENGINE FLAME OUT. 1384 HOUR & 6 MONTHS AFTER INSTALL.
NR 2 ENGINE FLAMED OUT ON TAKEOFF. ABORTED TAKEOFF. MAINTENANCE COULD NOT DUPLICATE.
PILOT REPORTED CAPTAIN'S ALTIMETER FAILED (FLAGGED) AT 600 FT AFTER TAKEOFF. RETURNED TO AIRPORT. MAINTENANCE REPLACED THE FAILED ALTIMETER.
MAINTENANCE NOTICED FUEL DRAINING FROM ENGINE GANG DRAIN - REPLACED LOW PSI FUEL PUMP DUE RUPTURED DIAPHRAGM
PILOT NOTICED FUEL LEAKING FROM ENGINE DRAIN ON PRE-FLIGHT. MAINTENANCE REPLACED THE DEFECTIVE PUMP.
