ON JUNE 2, 2026 OUR FLIGHT VTE 3393 (N11526, PGA - PHX, PGA) RETURNED TO PGA AFTER WIND SHIELD OUTER PANE CRACKED WHILE CLIMBING THRU 9000'. CREW COMPLETED QRH AND ELECTED TO RETURN TO PGA. THE AIRCRAFT DID NOT DEPRESSURIZE. THE AIRCRAFT LANDED WITHOUT INCIDENT AND MOC WAS NOTIFIED. THIS WINDSHIELD WAS PREVIOUSLY REPLACED DUE TO CRACKING JUST 31 FLIGHT HOURS PREVIOUS. BOTH WERE SIERRACIN WINDOWS. THE COMPANY IS PERFORMING AN INTERNAL INVESTIGATION AS TO THE CAUSE.
AFTER TAKE OFF SELECTED GEAR UP EVERYTHING CAME UP AND ABOUT 500 FOOT LATER HEARD WHAT SOUNDED LIKE NOSE DOORS OPEN, FLY OVER TOWER THEY CONFIRMED NOSE GEAR DOORS WAS OPEN, LANDED IN TUP, STEERING WAS EXTREMELY TIGHT ON RUDDERS WHILE TAXING.
DURING 2A INSPECTION THE UPPER FORWARD CEILING EMERGENCY LIGHT WAS FOUND TO BE INOPERATIVE. MAINTENANCE RESECURED THE AFFECTED EMERGENCY LIGHT CONNECTOR, OPERATIONALLY TESTED WITH NO DEFECTS NOTED, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING 2A INSPECTION THE EMERGENCY EXIT SIGN AT 18D WAS FOUND TO BE INOPERATIVE. MAINTENANCE R & R THE SIGN, OPERATIONALLY TESTED WITH NO DEFECTS NOTED, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK THE CARGO DOOR CUTOUT LOWER BEAM WAS FOUND TO BE CORRODED. MAINTENANCE R & R THE BEAM IAW SRM 51-40-02. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
THE CREW REPORTED THAT THE LT COFFEEMAKER SMELLS LIKE ELECTRICAL WIRES BURNING WHILE IN OPERATION, AND THAT IT COULD BE ASSOCIATED WITH ALL THE WATER LEAKING OUT OF THE RT COFFEE POT. THE CREW PLACED THE COFFEE POTS ON NEF-GL-01, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
THE CREW REPORTED AN ELECTRICAL BURNING ODOR IN THE CABIN. THE ACFT LANDED WITHOUT INCIDENT WHERE MAINTENANCE R & R THE LIGHT BALLAST ON RO 19 AND 21, OPERATIONALLY TESTED WITH NO DEFECTS AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING POST FLIGHT INSPECTION, THE CREW REPORTED THAT THE EXIT ROW EMERGENCY LIGHTING SYSTEM WERE INOPERATIVE. MAINTENANCE R & R THE NR 3 EMERGENCY LIGHT BATTERY PACK, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
CREW REPORTED A STRONG FUEL ODOR ON ROTATION. THE ACFT CONTINUED, WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED THE ACFT AND SECURED FUEL SOLENOID VALVE TO FLOW DIVIDER TUBE ASSY, OPS AND LEAK CHECKED WITH NO DEFECTS, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING A-CHECK, THE RT VERTICAL STABILIZER TO FUSELAGE DRAG ANGLE WAS FOUND TO BE CRACKED AT FRAME 76, STRINGER 1R. MX R & R THE ANGLE IAW SRM 51-40-02. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING INSPECTION THE RIGHT AFT EXTERNAL EMERGENCY LIGHT WAS FOUND TO BE INTERMITTENT. MAINTENANCE REPLACED THE RIGHT EXTERNAL EMERGENCY LIGHT ASSEMBLY WITH NO FURTHER DEFECTS NOTED.
DURING FLIGHT, THE CREW REPORTED THAT THE THRUST RATING BUTTON WAS STUCK AND WERE UNABLE TO CHANGE IT FROM CLIMB MODE. THE CREW DIVERTED TO MKE AND LANDED WITHOUT INCIDENT. MAINTENANCE FOUND THE THRUST RATING CONTROL BUTTON ON THE THRUST PANEL MODULE ASSEMBLY TO BE DIRTY. THE BUTTON WAS CLEANED AND OPERATIONAL CHECKS OF THE THRUST RATING BUTTON WERE GOOD WITH NO FURTHER DEFECTS NOTED.
DURING FLIGHT, THE CREW REPORTED THAT WHEN SELECTING CLIMB THRUST, THE THRUST RATING MODULE ASSEMBLY WENT TO TAKEOFF RESERVE MODE. THE CREW WAS UNABLE TO CHANGE THRUST RATING MODE AND RETURNED TO MKE, LANDING WITHOUT INCIDENT. MAINTENANCE FOUND THE THRUST RATING MODULE ASSEMBLY TO BE FAULTY. MAINTENANCE REPLACED THE THRUST PANEL MODULE ASSEMBLY. OPERATIONAL CHECKS OF THE THRUST RATING MODULE ASSEMBLY WERE GOOD WITH NO FURTHER DEFECTS NOTED.
UPON LANDING IN MKE, THE TOWER NOTIFIED THE CREW OF SMOKE COMING FROM THE LEFT MAIN LANDING GEAR. THE AIRCRAFT WAS STOPPED AND PASSENGERS WERE EVACUATED. BRAKE TEMPERATURES WERE ALL IN THE NORMAL RANGE. UPON INSPECTION, THE LEFT INBOARD BRAKE HAD AREAS OF YELLOW FLUID ON IT. NO SIGNS OF HYDRAULIC LEAKAGE WERE FOUND ON THE BRAKE. THE LEFT INBOARD BRAKE WAS REPLACED WITH NO FURTHER DEFECTS NOTED DURING TAXI.
DURING DESCENT INTO BNA THE CREW REPORTED NAV 1 AND 2 BOTH FAILED AND THE RMU NAV 1 AND 2 BOXES INDICATED DASHES. THE AIRCRAFT DIVERTED TO SDF WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE REPLACED BOTH NAV 1 AND 2 UNITS WITH NO FURTHER DEFECTS NOTED. (NAV COMPUTER NR 2 P/N: 7510100-631; S/N: 03087150; TSN: 12755; CSN: 9357).
IAH - FLIGHT 2346 DURING PREFLIGHT INSPECTION THE NR 1 ENG WAS FOUND TO BE MISSING PART OF THE EXHAUST DIFFUSER, WITH DAMAGE TO THE NR 1 THRUST REVERSER, EXHAUST CONE, AND CONE SUPPORT. MX REMOVED AND REPLACED ENG 1 EXHAUST CONE, FORCED MIXER CONE FLOWPATH SUPPORT AND FORCED CORE BYPASS MIXER. REF: ROLLS-ROYCE EMM 78-11-10, 78-11-12, AND 78-11-15, PLACED THE THRUST REVERSER ON DMI, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE. ON 07/14/08, THE ACFT LANDED IN SHV WHERE MX REMOVED AND REPLACED THE NR 1 THRUST REVERSER, OPERATIONALLY CHECKED WITH NO DEFECTS, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE. (S)
HSV - FLIGHT 2990 THE CREW REPORTED SMOKE IN THE COCKPIT WITH THE SOURCE IN THE VICINITY OF THE STEER DISCONNECT TRIGGER. THE ACFT RETURNED TO HSV WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE PLACED THE CAPTAIN`S CHART HOLDER ON MEL, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE. THE ACFT LANDED IN IAH WHERE MAINTENANCE REPLACED THE CAPTAIN`S CHART HOLDER. OPERATIONALLY CHECKED WITH NO DEFECTS NOTED, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE. (S)
IAH - FLIGHT 2008, THE CREW REPORTED AN ELECTRICAL BURNING ODOR IN THE COCKPIT ON DESCENT, AN EMEREGENCY WAS DECLARED, AND THE AIRCRAFT LANDED AT IAH WITHOUT INCIDENT. MAINTENANCE INSPECTED THE AIRCRAFT AND REMOVED AND REPLACED THE SIDEWALL LIGHT AT SEAT 7A. OPERATIONAL CHECKS WERE PERFORMED WITH NO DEFECTS NOTED, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
MAIN CABIN DOOR WILL NOT OPEN FROM INSIDE CABIN. MAINTENANCE INSTALLED MAIN CABIN DOOR INTERNAL HANDLE IAW EMB 135 AMM 52-18-05, OPS CHECK GOOD. AIRCRAFT WAS RETURNED FOR SERVICE.
A/C 526 WAS IN FOR A MOD, FOUND BOTH FWD EMERGENCY FLOOR PROX LIGHT STRIPS TO BE INOP. MAINTENANCE REPLACED BOTH LIGHT STRIPS IAW EMB 135 AMM 33-50-08 AND OPS CHECK WAS GOOD. AIRCRAFT WILL BE RELEASED UPON COMPLETION OF SCHEDULED MODIFICATIONS.
CREW REPORTED TWO OVERHEAD EMERGENCY LIGHTS ARE INOPERATIVE. MAINTENANCE RELAMPED BOTH EMERGENCY LIGHTS. OPS CHECKED BOTH GOOD IAW EMB135 MM 33-00-00. AIRCRAFT RETURNED TO SERVICE.
THE CREW NOTED THAT THE EMERGENCY LIGHT WAS INOPERATIVE IN THE CABIN DURING PREFLIGHT. MAINTENANCE REMOVED AND REPLACED THE LIGHT BULB, PERFORMED A OPERATIONAL CHECK, NO DEFECTS NOTED, AND, THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
DAY FLIGHT 3316 CREW REPORTED THAT AFTER TAKEOFF, GOT THE LANDING GEAR LEVER DISAGREE WARNING, AND THE LANDING GEAR WOULD NOT RETRACT. RETURNED TO DAY AND LANDED WITHOUT INCIDENT. AIRCRAFT WAS FERRIED TO TYS WHERE MAINTENANCE PERFORMED A LANDING GEAR RETRACTION CHECK, AND AN AIR GROUND OPERATIONAL CHECK. ALL OPERATIONAL CHECKS WERE GOOD. NO DEFECTS WERE FOUND. AIRCRAFT WAS RETURNED TO SERVICE.
CLE-MEM FLIGHT#3887 THE CREW REPORTED A BAGGAGE SMOKE WARNING IN FLIGHT.THE AIRCRAFT DIVERTED TO SDF AND LANDED WITHOUT INCIDENT.MAINTENANCE INSPECTED AND FOUND NO EVIDENCE OF SMOKE OR FIRE.THEY REPLACED THE BAGGAGE SMOKE DETECTOR AND THE BAGGAGE FIRE BOTTLES THAT THE CREW DISCHARGED. OPERATIONAL CHECKS WERE GOOD.THE AIRCRAFT WAS RETURNED TO SERVICE.
