SDF - FLIGHT 4448 THE REW REPORTED THAT WHEN APU STARTED WITH THE ENGINES RUNNING HEARD A LOUD CRACKLING AND SMELL OF BURNT ELECTRICAL, ALSO HAPPENED DURING ENGINE SHUT DOWN. THE AIRCRAFT LANDED AT SDF WITHOUT INCIDENT WHERE MAINTENANCE INSPECTED THE AIRCRAFT AND FOUND AN INTERCONNECTION BAR LOOSE IN THE RIGHT HAND POWER DISTRIBUTION BOX. THE BAR WAS TIGHTENED, OPERATIONALLY TESTED WITH NO DEFECTS AND TEH AIRCRAFT WAS APPROVED FOR RWETURN TO SERVICE.
CREW REPORTED"WHEN APU IS STARTED WHILE THE ENGINES WAS RUNNING LOUD CRACKLING AND SMELL OF SMOKE AND BURNT AND ELECTRICAL ALSO HAPPENED DURING ENGINE SHUT DOWN". CONTRACT MX INSPECTED COCKPIT AREA FOR LOOSE OR DAMAGED ELECTRICAL ITEMS AND FOUND ELECTRICAL INNER CONNECTION BAR UNDER COCKPIT FLOORING TO HAVE A LOOSE TERMINAL. TIGHTENED IAW EMB145 AMM 24-65-01. OPS CHECKED GOOD. A/C WAS APPROVED FOR SERVICE.
IAH FLIGHT 4385, THE CREW RPORTED THAT THE FLOOR EMERGENCY LIGHTS BETWEEN SEATS 8 AND 18 WERE INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE NUMBER THREE EMERGENCY LIGHT BATTERY PACK, OPERATIONALLY TESTED WITH NO DEFECTS NOTED AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
CREW REPORTED SMOKE IN THE COCKPIT, VISIBLE SMOKE FROM THE LOWER LEFT CORNER OF FIRST OFFICERS WINDSHIELD. ACFT DIVERTED FROM DESTINATION AND LANDED SAFELY. MX REMOVED AND REPLACED THE NR 2 WINDSHIELD TEMP CONTROLLER. OPS CHECKED GOOD. REMOVED AND REPLACED THE FIRST OFFICER'S WINDOW DUE TO A BURNED TERMINAL. ACFT WAS APPROVED FOR RETURN TO SERVICE.

LEFT OTBD SEAT TRACK NR 5 BETWEEN FRAMES 47 - 52 CORRODED. REPLACED THE SEAT TRACK IAW SRM 53-01-06.
LEFT OUTBOARD SEAT TRACK NR 4 BETWEEN FRAMES 36 - 47 CORRODED. THE SEAT TRACK WAS REPLACED IAW SRM 53-01-06.

DURING CRUISE, SMOKE APPEARED IN THE CABIN AND ODOR WAS ALSO OBSERVED. ACFT LANDED OK AND THEN WAS FERRIED, MAINTENANCE R & R THE NR 2 ACM IAW AMM 21-51-03, ALSO R & R THE NR 2 CONDENSER MIXER IAW AMM 21-51-04. LEAK AND OPS CHECK GOOD AND WAS RELEASED BACK TO SERVICE. SN OF MIXER WAS 200108018.
AFTER TAKEOFF, AT 4000FT F/A CALLED AND STATED SMOKE IN THE CABIN. PILOT AND F/O COULD SEE SMOKE AND DECLARED EMERGENCY, RETURNED TO AIRPORT. MAINTENANCE INSPECTED ACFT AND FOUND NR 2 ACM SEIZED. PLACED ON MEL. ON 8/4/15 MAINTENANCE R & R NR 2 ACM WITH OPS CHECK GOOD. ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING MAINTENANCE INSPECTION, THE EMERGENCY LIGHT FLOOR STRIP NR 3 WAS DISCOVERED INOPERATIVE. MAINTENANCE R & R NR 3 FLOOR LIGHT STRIP WITH OPS CHECK GOOD. AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK CARGO DOOR CUTOUT LOWER BEAM 65-68 IS CORRODED (LOCAL LEVEL 2). MX PERFORMED REPAIR ON CARGO DOOR CUTOUT LOWER BEAM AT FRAMES 65-68 IAW SRM 51-40-02. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK LT UPPER SEAT TRACK SUPPORT CORRODED. (LOCAL LEVEL 2) AT FRAME 50. MX PERFORMED REPAIR TO LT UPPER SEAT TRACK SUPPORT FRAME 50 IAW EC5320-01522, REV B. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
A SMALL PUNCTURE IN THE SKIN AT FRAME 10 NEAR STRINGER 14 NEXT TO THE TAT PROBE. MX PERFORMED A DOUBLER REPAIR ON THE AFFECTED SKIN, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
CLE FLT 3095 THE CREW REPORTED ON DEPARTURE FROM CLE THE TOWER INFORMED THEM OF A FLUID LOSS COMING FROM THE RIGHT WING. THE AIRCRAFT RETUNED TO CLE AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED AND FOUND THE RIGHT WING OUTBOARD FLAPPER VALVE SEAL TORN. THE VALVE WAS REPLACED AND LEAK CHECKS WERE GOOD.
CLE FLT 2510 THE CREW REPORTED A ENGINE NR 2 BLEED LEAK MESSAGE ON CLIMB. THE AIRCRAFT RETURNED TO CLE AND LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE INSPECTED AND REPLACED THE AIR START CHECK VALVE AND RE-SECURED A LOOSE DUCT ON NR 2 PACK AT THE DUAL TEMP CONTROL VALVE. OPERATIONAL CHECKS WERE GOOD.
TYS - A/C CURRENTLY IN C-CHECK WHEN INSPECTION FOUND THE RT PCA BRACKET WORN OUT OF LIMITS. MAINTENACE R/R THE BRACKET IAE EC5750-01177. A/C WILL BE RELEASED AT THE END OF CHECK.
TYS - A/C CURRENTLY IN C-CHECK WHERE INSPECTION FOUND THE LT AILERON BUSHIONS MISSING WHERE THE O/B PCA ROD ATTACHES. MAINTENANCE R/R THE PCA BRACKET IAW EC5750-01177. A/C WILL BE RELEASED AT THE END OF THE CHECK.
ORD FLT 6144 THE CREW REPORTED A BURNING SMELL IN THE COCKPIT AND FORWARD CABIN ON TAXI IN. MAINTENANCE INSPECTED AND COULD NOT DUPLICATE ANY BURNING SMELL. OPERATIONAL CHECKS WERE GOOD IAW 21-20-00-810-801-1. THE SMELL WAS ALSO NOTED ON FLIGHT 5910. MAINTENANCE COULD NOT DUPLICATE ANY BURNING SMELL.
FLIGHT 2643, THE CREW REPORTED THAT NR 1 A/C PACK WAS BLOWING HOT AIR. THE AIRCRAFT DIVERTED TO GPT WHERE IT LANDED WITHOUT INCIDENT. THE COOLING SYSTEM WAS RESET, OPERATIONALLY TESTED GOOD, AND THE AIRCRAFT CONTINUED TO IAH. IN IAH, THE DISCREPANCY REPEATED AND MAINTENANCE PLACED THE NR 1 PACK ON MEL, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE. ON 07/09/10, MAINTENANCE REMOVED AND REPLACED RELAYS K0187 AND K0335, OPERATIONALLY TESTED WITH NO DEFECTS, REMOVED THE MEL, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.

DURING COMPLIANCE OF SB AE3007A-72-368 ON THE NR 2 ENGINE A CRACK INDICATION OF THE 2ND STAGE HIGH PRESSURE TURBINE WHEEL WAS FOUND WHILE PERFORMING THE ULTRASOUND INSPECTION. THE NR 2 ENGINE WAS REPLACED.
NR 4 EMERGENCY BATTERY INOP. REMOVED AND REPLACED BATTERY IAW EMB 145 MM 33-50-04, OPS CHECK GOOD. AIRCRAFT RELEASED AND RETURNED TO SERVICE.
FLIGHT 2197 - EWR - FLIGHT CREW REPORTED FLOOR EMERGENCY LIGHTS SEAT ROW 17A INOPERATIVE. EWR MX REMOVED AND REPLACED NR 6 INVERTER AND PERFORMED OPERATIONAL CHECK SUCCESSFULLY. AIRCRAFT WAS APPROVED TO RETURN TO SERVICE.
FLIGHT 2988 - EWR - FLIGHT CREW REPORTED STRONG ELECTRICAL BURN SMELL WITH WINDSHIELD WIPER ACTIVATED. EWR MX REMOVED AND REPLACED LT WINDSHIELD WIPER SWITCH AND PERFORMED OPERATIONAL CHECK SUCCESSFULLY. AIRCRAFT WAS APPROVED TO RETURN TO SERVICE.
CLE FLIGHT 2592 - THE CREW REPORTED A UNUSUAL LOAD SOUND FROM UNDER THE COCKPIT FLOOR ONLY IN CLIMB AND CRUISE. ALSO REPORTED A SLIGHT VIBRATION OF YOLK AND RUDDER PEDALS. MAINTENANCE INSPECTED AND REPLACED THE NOSE LANDING GEAR UPLOCK BOX "B"PROXIMITY SWITCH. OPERATIONAL CHECKS WERE GOOD.
CLE FLIGHT 2409 THE CREW REPORTED A AIR/GROUND FAIL MESSAGE IN FLIGHT. THE LANDING GEAR WOULD NOT RETRACT, ALSO RECEIVED A ENG REV 1&2 DISAGREE MESSAGE ON THE ECIAS. MAINTENANCE INSPECTED AND REPLACED THE NOSE LANDING GEAR AIR/GROUND PROXIMITY SWITCH AND HARNESS. OPERATIONAL CHECKS WERE GOOD. HARNESS P/N 145-21495-405.
MKE FLIGHT 2560 - THE CREW REPORTED A AIR/GROUND FAIL MESSAGE ON TAKEOFF. THE AIRCRAFT RETURNED TO MKE AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED AND CLEANED DEBRIS FROM RIGHT MAIN LANDING GEAR PROXIMITY SWITCH. OPERATIONAL CHECKS WERE GOOD. THE AIRCRAFT WAS RELEASED FOR SERVICE.

SDF - EMERGENCY LIGHT OVER GALLEY DOOR INOP. REMOVED AND REPLACED NR 1 EMERGENCY LIGHT BATTERY AND BATTERY POWER SUPPLY IAW EMB 145 AMM 33-50-04. OPS CHECK GOOD.

AFTER TAKEOFF, LG AIR/GROUND DISAGREE MESSAGE APPEARED ON EICAS. LANDING GEAR WOULD NOT RETRACT NORMALLY UNTIL OVERRIDE, THEN ENG 1-2 T/R DISAGREE MESSAGE APPEARED ON EICAS. CREW RETURNED TO CHS. ACFT WAS FERRY FLIGHT TO RIC. RIC MX PERFORMED PROX SW FUNCTIONAL CHECK AND FOUND 3RD UPLOCK LOGIC CORRUPTED. MX REMOVED AND CLEANED CONNECTORS P0774 AND P0775. MX PERFORMED RESET OF AIR GROUND LOGIC SYSTEM. LANDING GEAR RETRACTION AND EXTENSION FUNCTIONAL TEST WAS SUCCESSFULLY PERFORMED. THRUST REVERSER OPERATIONAL CHECK WAS SUCCESSFULLY PERFORMED AND ACFT WAS APPROVED TO RETURN TO SERVICE.
TYS, CONTINUATION OF CONTROL NR C2XA05TY008, AIR RETURN FOR LANDING GEAR AIR GROUND FAIL ON TAKEOFF. MAINTENANCE FOUND THE LEFT MLG FORWARD AND AFT WOW PROX SWITCHES ONLY 250K RESISTANCE, SHOULD BE 1.4M. MAINTENANCE REMOVED AND REPLACED THE LEFT GEAR CANNON PLUG CONNECTORS AND ALL 4 PROX SWITCHES, (P/N 80-057-01, S/N'S 2898, 2783 AND 2885 TSO 3099.9, CYC 2419 / S/N 3437 TSO 3332, CYC 2617. TEST FLEW SATISFACTORY AND WAS RELEASED TO SERVICE.
TYS - A/C WAS DEPARTING BHM WHEN THE ICAS MESSAGE SHOWED GEAR NOT RETRACTED. A/C RETURNED TO BLOCK AND MAINTENANCE REMOVED AND REPLACED THE LGEU, BUT DID NOT FIX PROBLEM. A/C WAS FERRIED TO TYS AND MAINTENANCE REMOVED AND REPLACED THE NR 1 AND NR 2 PROX SWITCHES. A/C WAS REL FOR A TEST FLIGHT AND TEST FLIGHT WAS SATISFACTORY. A/C WAS THEN REL FOR REVENUE SERVICE.

SHV DURING C CHECK THE RIGHT HAND LOWER COWLING WAS FOUND TO HAVE A GOUGE MARK AT FRAME 8 ON MOST OUTBOARD STRINGER. MAINTENANCE REPAIRED AFFECTED AREA IAW EMB 145 SRM 54-10-01. THE AIRCRAFT WAS APPROVED TO RETURN TO SERVICE.

SHV-DURING C CHECK THE CROSSBEAM AT FRAME 23 WAS FOUND TO BE CORRODED. MAINTENANCE REMOVED AND REPLACED THE AFFECTED CROSSBEAM IAW EMB 145 SRM 51-40-02. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
SHV-DURING C CHECK STRINGER 19 RIGHT BETWEEN FRAMES 52 AND 53 WAS FOUND TO BE GOUGED. MAINTENANCE REMOVED THE DAMAGED AREA AND REPAIRED THE AFFECTED STRINGER IAW EMB 145 SRM 53-24-02. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
SHV-DURING C CHECK THE CARGO DOOR CUTOUT LOWER BEAM BETWEEN FRAMES 65 AND 68 WAS FOUND TO BE CORRODED. MAINTENANCE REMOVED AND REPLACED THE AFFECTED BEAM IAW EMB 145 SRM 51-40-02. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
THE CREW REPORTED A BURNING SMELL COMING FROM THE LEFT HOT JUG IN THE GALLEY. MAINTENANCE INSPECTED AND REPLACED THE LEFT HOT JUG CONNECTOR AND HOT JUG. OPERATIONAL CHECKS WERE GOOD.
CLE FLIGHT#3915 THE CREW REPORTED A LOUD ROARING NOISE FROM AREA BELOW COCKPIT FLOOR IN FLIGHT.MAINTENANCE INSPECTED AND FOUND THE NOSE GEAR DOORS WOULD NOT RETRACT.THE NOSE GEAR DOOR SEQUENCE PROXIMITY SWITCH WAS REPLACED.OPERATIONAL CHECKS WERE GOOD.
