WHILE PERFORMING A COMPANY FLEET CAMPAIGN DISCOVERED FORWARD ELEVATOR BELLCRANK CRACKED WEB.
PILOT REPORTED "OIL LEAKING FROM RIGHT ENGINE" - MAINTENANCE FOUND OIL LEAK TO BE COMING FROM THE ENGINE OIL SCAVENGE PUMP HOUSING. REPLACED HOUSING. FOUND CAVITATION EROSION THROUGH CASTING.
PILOT REPORTED: ON APPROACH PUT LANDING GEAR DOWN. LEFT MAIN AND NOSE GEAR CAME DOWN. PUMP STOPPED WORKING HAD AN INTRANSIT LIGHT AND RIGHT MAIN NEVER CAME DOWN. 20 SECONDS LATER PUMP STARTED WORKING INTRANSIT LIGHT WENT OUT AND RIGHT MAIN CAME DOWN. MAINTENANCE REPLACED R/H MAIN LANDING GEAR ACTUATOR. SEE NEXT SDR YADA20190429002
MAINTENANCE REPORTED: WHILE HAND PUMP CYCLING LANDING GEAR AFTER R/H MAIN LANDING GEAR ACTUATOR REPLACEMENT, R/H MAIN GEAR WILL NOT COME DOWN.
MAINTENANCE DISCOVERED THAT THE NR 1 ENGINE HAD AN OIL LEAK AT THE SCAVENGE PUMP HOUSINGâ THE PUMP WAS LEAKING THROUGH THE CASTING. THIS IS THE THIRD SUCH FAILURE OF THIS PART IN ONE YEAR FOR US.
PILOTS REPORTED A STRONG SMELL OF ELECTRICAL FUMES IN THE COCKPIT AND THAT O2 MASKS WERE USED. AIRCRAFT LANDED AS SCHEDULED IN DILLINGHAM. MAINTENANCE WAS DISPATCHED TO (DLG) AND FOUND THAT THE AFT VENT BLOWER BEARING SEIZED. THE VENT BLOWER WAS REMOVED AND REPLACED, OPERATIONAL CHECK GOOD
COMPONENT TEAR DOWN FINDINGS: BLOWER MOTOR END CAP BEARING P/N:M4000AC-3, FAILED INTERNALLY, CAUSING HIGH MOTOR DRAG AND HEAT BUILDUP. HEAT SINK RESISITORS, WIRE TERMINALS AND NOISE SUPPRESSORS SHOWED HIGH HEAT AREAS (MOST LIKELY DUE TO HIGH CURRENT DRAW FROM MOTOR). NO OTHER ASSEMBLY / PART FAILURES NOTED AT DISASSEMBLY OF COMPONENT. UNIT TO BE OVERHAULED AND RETURNED TO SERVICE.
PILOT REPORTED WITH ENG BLEEDS OFF AND VENT ON HIGH, THE ODOR OF BURNING OIL/FUMES FILLED THE COCKPIT. PILOT HAD PREVIOUSLY REPORTED THAT THE AIRCRAFT WOULD NOT PRESSURIZE WITH BLEEDS ON UNLESS TEMPERATURE SWITCH WAS HELD IN INCREASE & COCKPIT WAS EXTREMELY WARM. AFTER FURTHER EVALUATION MAINTENANCE FOUND THE RT AFT EMERGENCY EXIT DOOR SEAL WAS ROLLED AND DEBONDED FROM DOOR SURFACE, REPLACED DOOR SEAL AND ALL OPS/PRESSURE CHECKS WERE GOOD.
PILOT REPORT RUDDER PEDALS FROZEN AT ALTITUDE WITH YAW DAMP OFF. MAINTENANCE DISCOVERED AND REMOVED ICE AND WATER UNDER FLOOR BOARDS, CLEANED AND INSPECTED, OPS CHECKED GOOD.
POST LANDING INSPECTION REVEALED THAT THE NR 1 ENG HAD A SEPARATION OF PN 114-980077-17 SKIN FROM THE PN 114-980077-5 BOND ASSEMBLY & WAS NO LONGER ATTACHED TO THE ACFT. MX VERIFIED THAT ALL BOND ASSEMBLY FASTENERS WERE IN PLACE & THAT THE FAILURE WAS THE BOND BETWEEN THE SKIN & THE THICKER SUBSTRUCTURE. ACFT WAS CONTACTED CONCERNING THE FAILURE OF THE BOND & PROVIDED AN APPROVED FIELD REPAIR FOR CORRECTING FAILED BONDS ON THIS ASSEMBLY. A VISUAL INSPECTION ON THE FLEET DID NOT DISCLOSE IMMINENT FAILURE OF ADDITIONAL PANELS. WE ARE ISSUING A FLEET CAMPAIGN TO PERFORM DETAILED INSPECTION & REPAIRS AS REQUIRED ON ALL AFFECTED PANELS IN THE FLEET IAW THE FACTORY PROVIDED SERVICE DOCUMENTATION FR-CO-00037.
IN FLIGHT, AFTER LG HAD BEEN RETRACTED & STOWED, THE LG RELAY CB POPPED. THE LG HANDLE WAS SELECTED DOWN & CB PUSHED IN, CB POPPED AGAIN. MANUAL GEAR EXTENSION WAS THEN ACHIEVED. FOUND THE LG PUMP MOTOR BURNED. THE MOTOR WAS REPLACED AS WELL AS THE SUSPECTED PROBLEM SOURCE LG TIME DELAY CIRCUIT BOARD.
HAD TO PUMP GEAR DOWN TO GET THREE GREEN, NOSE GEAR HAD NOT INDICATED GREEN. NO IN-TRANSIT LIGHT. FERRY GEAR DOWN. MX COULD NOT DUPLICATE. GEAR RETRACT & EXTEND, OPERATIONS CHECKED GOOD MULTIPLE TIMES. REPLACED NOSE GEAR DRAG BRACE DOWN LOCK SWITCH FOR COMPANY CONVENIENCE. SOME CORROSION FOUND IN DOWN LOCK SWITCH CONNECTOR PINS.
WHILE INVESTIGATING RISING ENG ITT TREND, FOUND SIGNIFICANT DAMAGE TO COMPRESSOR SHROUD SEGMENTS & CT BLADES. CAUSE OF DAMAGE APPEARS TO BE FROM EXCESSIVE CARBON BUILD UP ON THE COMBUSTION LINER COMING LOOSE & ABRADING SEGMENTS & CT BLADES TO THE POINT THE SEGMENTS LOST PROPER ATTACHMENT TO THE VANE RING. THIS REPORT WILL BE AMENDED AFTER ADDITIONAL REVIEW OF REMOVED COMPONENTS IS COMPLETED.
WHILE TAXIING TO REPOSITION ACFT, FLIGHT CREW REPORTED SMELLING & SEEING SMOKE FROM THE INSTRUMENT PANEL. THE ACFT RETURNED TO THE RAMP WITHOUT INCIDENT. PERFORMED A GND RUN ON THE ACFT & OBSERVED THE COPILOTS ELECTRONIC VSI INDICATOR SCREEN FLICKER & THEN WENT BROWN, THE SMELL OF A HOT ELEC COMPONENTS WAS NOTED. NO WIRING DAMAGE WAS NOTED & THE INTERNALLY FAILED EVSI WAS REPLACED & AN OPERATIONAL CHECK FLIGHT PERFORMED SATISFACTORILY.
PIREP, RED GEAR HANDLE LIGHT ILLUMINATES WITH GEAR DOWN & HANDLE DOWN. REPLACED NOSE GEAR DOWN LOCK SWITCH AFTER UNEVENTFUL LANDING AT ORIGINATING AIRPORT.
ON CLIMB OUT, THE PILOT RETRACTED THE LANDING GEAR BUT THE RED "GEAR INTRANSIT LIGHT" IN THE LANDING GEAR SELECTOR HANDLE NEVER EXTINGUISHED. THE GEAR WAS EXTENDED AND THE ACFT WAS LANDED AND RETURNED TO BASE. MX PERSONNEL FOUND THE A CONNECTOR PLUG AND WIRING ASSOCIATED WITH THE NOSE GEAR UPLOCK SWITCH WAS CORRODED. THE UPLOCK SWITCH, CONNECTOR PLUG AND WIRING WERE REPLACED AND THE SYS OPS CHECKED GOOD.
PILOT REPORTED LEFT BRAKE "FLAT". REPLACED SEEPING FLEXIBLE BRAKE LINE - DATE CODE 2Q06.
PILOT REPORTED LT BRAKE FLAT ON TAXI AND LANDING. MX BLEED BRAKES.
