INSTRUMENT PANEL STIFFENER CRACKED
FOUND OIL LEAKING AROUND VERTICAL FIN AREA. INSPECTED AREA AND FOUND TAIL ROTOR GEARBOX LEAKING AND NOTICED THAT TAIL ROTOR CASTING WAS ALSO CRACKED. UPON DISASSEMBLY FOUND THAT TAIL ROTOR HEAD BOLT HAS BEEN GALLED AND TEFLON FROM TORSION TENSION STRAP HAS BEEN FORCED OUT AND DAMAGED T/R HEAD AND BOLT. UPON REMOVAL OF T/R GEARBOX FOUND NONE OF THE ATTACHMENT BOLTS WERE TORQUED AND CAUSED MOUNTING STUDS IN GEARBOX TO FAIL AND CRACKED GEARBOX ATTACHMENT CASTING. CASTING HAS BEEN CHANGED BEFORE DUE TO HIGH-LOC INSTALLED.
IN CRUISE FLIGHT, TO PICK UP A PATIENT AT 2500MSL, ACFT EXPERIENCED A T/R CHIP LIGHT. THE ACFT COMPLETED THE EMERGENCY PROCEDURE FOR SAME, DIVERTED TO AN AIRPORT ABOUT 3 MILES AWAY, CALLING THE COMM CENTER AND ADVISED OF THE SITUATION. NORMAL SHUTDOWN COMPLETED AND MECHANIC NOTIFIED. AMC PILOT CHIP PLUG MX ACTION COMPLETED AND TO INCLUDE LOG BOOK ENTRIES. PILOT DID A 10 MIN PENALTY RUN, SHUTDOWN AND CHECKED BOTH T/R AND IGB PLUGS ALL WAS NORMAL. HE BRIEFED MED CREW AND FLEW BACK TO UNC WITH OUT FURTHER INCIDENT.
WHILE PERFORMING A PREFLIGHT A CRACK WAS FOUND IN THE TAIL ROTOR BLADE NEAR THE DATA TAG.
