THE PILOT REPORTED A VISIBLE PUFF OF SMOKE FROM THE OVERHEAD PANEL AREA TO MAINTENANCE. MAINTENANCE FOUND TWO WHEAT BULBS BROKEN AND POSSIBLY SHORTED TOGETHER IN DE-ICE PANEL BACK LIGHTING. NO OTHER ELECTRICAL ISSUES WERE FOUND. ALL SYSTEMS WERE FUNCTIONAL. THE BULBS WERE REPLACED AND OPERATED NORMALLY.
AFTER LANDING, THE PILOT REPORTED A VISIBLE PUFF OF SMOKE FROM THE OVERHEAD PANEL AREA TO MAINTENANCE. MAINTENANCE FOUND TWO WHEAT BULBS BROKEN AND POSSIBLY SHORTED TOGETHER IN THE DE-ICE PANEL BACK LIGHTING. NO OTHER ISSUES WERE FOUND. ALL SYSTEMS WERE FUNCTIONAL. THE BULBS WERE REPLACED AND OPERATED NORMALLY.
EVENING FLIGHT WIG 1361, DURING CLIMB THE GEAR IN TRANSIT LIGHT WOULD NOT EXTINGUISH. ACFT RETURNED SAFELY. INITIAL INVESTIGATION SHOWS WEAK SPRING IN DOOR ROD END.
THE PILOT REPORTED THAT IN CRUISE, APPROX 20 MINUTES AFTER TAKEOFF, THE RT ENG SURGED MORE THAN ONCE & HE ELECTED TO SHUT IT DOWN. HE THEN DECLARED AN EMER & DIVERTED, MAKING A SUCCESSFUL LANDING. A VISUAL INSPECTION WITH BORESCOPE BY MX PERSONNEL REVEALED MOST, IF NOT ALL, BLADES DISINTEGRATED ON BOTH THE COMPRESSOR & POWER TURBINES. SOME DAMAGE IN THE FORM OF NICKS & CURLED TOES IS VISIBLE ON THE SECOND STAGE COMPRESSOR BLADES ALSO. TURBINE BLADES DO NOT APPEAR TO HAVE SEPARATED AT THE FIR TREE ROOTS. THERE IS SOME RESIDUAL OIL ON THE INSIDE BOTTOM OF THE POWER SECTION. THIS WAS THE FIRST FLIGHT AFTER A RT HOT SECTION INSPECTION. THE SIGNIFICANCE OF THIS IS NOT YET KNOWN.
DURING ROUTINE MAINTENANCE, PERSONNEL NOTICED EMERGENCY EXIT LIGHTING INOPERATIVE ON RIGHT ''B'' ZONE. MAINTENANCE PERSONNEL REPAIRED SEGMENT OF EMERGENCY LIGHTING STRIP AND CLEANED CONNECTORS. AIRCRAFT RETURNED TO SERVICE. (M)
DURING INITIAL CLIMB OUT, 800 FT AGL, NR 2 ENGINE BEGAN SURGING. FLT CREW RETARDED THROTTLE FROM 95 PERCENT N1 TO FLIGHT IDLE. SURGING DISCONTINUED. UPON REACHING ASSIGNED ALTITUDE, FLIGHT CREW ADVANCED THROTTLE TO 75 PERCENT N1, AND SURGING RETURNED. THROTTLE REDUCED TO FLT IDLE. FLIGHT CREW ELECTED TO RETURN TO DEPARTURE POINT FOR MX. NR 2 ENG WAS LEFT AT FLT IDLE, AND AIRCRAFT RETURNED TO DEPARTURE POINT, LANDING WITHOUT FURTHER INCIDENT. MX PERSONNEL INSPECTED ENG EXTERIOR WITH NO EVIDENCE OF DAMAGE. PERFORMED BORESCOPE WITH NO EVIDENCE OF DAMAGE. ENGINE TREND MONITORING INDICATED THAT N2 WAS BELOW TARGET. RERIGGED THE VARIABLE STATOR VANE SYSTEM AND PERFORMED GROUND RUNS.
DURING TAXI FOR TAKEOFF, FLIGHT CREW WAS UNABLE TO START NR 1 ENGINE ON B-IGNITION. FLIGHT CREW ELECTED TO RETURN TO GATE FOR MAINTENANCE. AIRCRAFT RETURNED TO GATE WITHOUT FURTHER INCIDENT. MAINTENANCE PERSONNEL REPLACED THE NR 1 ENGINE B-IGNITION EXCITER BOX. THIS DID NOT CORRECT MALFUNCTION. DEFERRED MALFUNCTION PER MEL 74-1. AIRCRAFT RETURNED TO SERIVCE. WILL PROVIDE FOLLOW-UP REPORT WHEN CORRECTIVE ACTION IS ACCOMPLISHED. (X)
DURING AIR TURN BACK FOR PASSENGER MEDICAL EMERGENCY, FLIGHT CREW ACTIVATED FUEL DUMP PROCEDURES TO REDUCE A/C WEIGHT TO BELOW MAXIMUM LANDING WEIGHT. DURING FUEL DUMP PROCEDURES, 'DUMP VALVE OPEN' LIGHT FAILED TO GO OFF WHEN DUMP SWITCH WAS POSITIONED TO CLOSE. FLIGHT CREW RESET C/B, BUT SAME INDICATION WAS NOTED. ADDITIONALLY FLIGHT CREW WAS UNABLE TO TRANSFER FUEL, CONFIRMING THE LEFT DUMP VALVE WAS IN THE OPEN POSITION. AIRCRAFT CONTINUED TO DEPARTURE AIRPORT AND MADE AN UNEVENTFUL LANDING. GROUND MX FOUND LEFT FUEL DUMP SYSTEM SHUT OFF VALVE WOULD NOT CLOSE ELECTRICALLY. PART WAS NOT AVAILABLE ON SITE. VALVE WAS MANUALLY CLOSED PER MM PROCEDURES AND DEFERRED PER MEL PROCEDURES. A/C RETURNED TO SERV
DURIING TAXI-OUT, FLIGHT ATTENDANT SMELLED FUMES (UN-IDENTIFIED) AND NOTIFIED FLIGHT CREW. FLIGHT CREW ELECTED TO RETURN TO GATE FOR INVESTIGATION. AIRCRAFT RETURNED TO GATE WITHOUT INCIDENT. MAINTENANCE PERSONNEL INSPECTED AIRCRAFT OF EVIDENCE OF ELECTRICAL FIRE AND FLUID CONTAMINATION IN CABIN. NO EVIDENCE OF EITHER WAS NOTED. NO EVIDENCE OF FUMES WAS NOTED BY MAINTENANCE PERSONNEL. AIRCRAFT RETURNED TO SERVICE. (X)

DURING CLIMB THRU 30,000 FT, FLT CREW RECEIVED NR 2 ENG FIRE WARN LIGHT. ALL OTHER IND NORM AT THIS TIME. FLT CREW ELECTED TO SHUTDOWN ENG AND OBSERVE. UPON ENG SHUTDOWN, FIRE IND WENT OUT. FLT CREW ELECTED TO DIVERT A/C, LANDED W/O FURTHER INCIDENT. MX PERSONNEL INSPECTED ENG AND NACELLE, NO IND OF FIRE WAS NOTED. GROUND RUN ENG FOR APPROX 20 MIN AT VARIOUS PWR SETTINGS, WITH NO FAULT IND. PERFORM 'BITE' TEST OF FIRE DETECT CONTROL UNIT. UNIT PASSED 1ST AND 2ND TEST, BUT FAILED 3RD TEST. SUSPECT INTERMITTENT FAULT IN FIRE DETECT CONTROL UNIT. MX PERSONNEL REPLACED FIRE DETECT CONTROL UNIT AND PERFORMED BITE TEST WITH NO FAULT. A/C RETURNED TO SERV. (M)
DURING TAKEOFF AT ROTATION, FLIGHT CREW EXPERIENCED COMPRESSOR STALL ON NR 2 ENGINE. FLIGHT ENGINEER NOTED A 6 PERCENT DROP IN N-1 SPEED. DURING INITIAL CLIMB-OUT, CREW NOTED SECOND COMPRESSOR STALL. AIRCRAFT RETURNED TO POINT OF DEPARTURE, LANDING WITHOUT FURTHER INCIDENT. MAINTENANCE PERFORMED BORESCOPE WITH NO FAULTS FOUND. AS A PRECAUTION, THE MAIN ENGINE CONTROL AND COMBUSTION INLET TEMPERATURE SENSOR WERE REPLACED. GROUND RUN PERFORMED AND ALL PARAMETERS NORMAL. ACFT RETURNED TO SERVICE. (X)
