NR 1 ENGINE FAILED TO MAKE T/O POWER. FOUND BROKEN PS4 LINE AT NR 1 ENGINE FUEL CONTROL ASSY. REPLACED HARD LINE WITH FLEX HOSE ASSY IAW 20-10-12. PERFORMED GROUND MAINTENANCE RUN AT ALL POWER SETTINGS, CHECKED OUT IAW BMM CHAPTER 72-00, NO DEFECTS NOTED.
Part: DUCT — CRACKED (NR 2 ENG)
DURING CLIMB-OUT FROM TOLEDO, NR 2 ENGINE FIRE WARNING SOUNDED. CREW FIRED ONE EXTINGUISHER AND SHUT DOWN THE ENGINE. AIRCRAFT LANDED UNEVENTFULLY AT TOL. MAINTENANCE DISCOVERED 13TH STAGE BLEED SADDLE DUCT CAP (RT) FAILED AT WELD ALLOWING HOT BLEED AIR INTO THE COWLING NACELLE. REPLACED CAP AND FIRE BOTTLE, LEAK CHECKED DUCT, AND RETURNED TO SERVICE.
