THIS PUMP WAS INSTALLED NEW WITH ZERO TIME ON IT. IT HAS A 2000 TBO. IT LASTED 92.9 HRS. THE PUMP FAILED INTERNALLY. THE DRIVE GEAR WHEN TURNED BY HAND LOCKS UP INSIDE THE PUMP. (K)

THIS CYLINDER WAS IN THE NR2 POSITION AND IS A TYPE (A) HEAD. THIS CRACKED CYLINDER HEAD WAS FOUND AT THE 100 HOUR INSPECTION. WE ARE FINDING SIGNS OF AIR LEAKAGE ON THE EXHAUST SIDE OF THE CYLINDER HEAD WHEN DOING A COMPRESSION CHECK. WE ARE SPRAYING SOAPY WATER ON THE CYLINDER HEAD.

UPON LANDING, THE NOSE LANDING GEAR COLLAPSED. INVESTIGATION DETERMINED THE ACTUATOR ROD BOLT WAS MISSING. THE CAUSE COULD BE EITHER A FAILED BOLT OR FAILED SAFETY. SUBMITTER STATED CLOSER INSPECTION OF THIS AREA COULD PREVENT THIS OCCURRENCE IN FUTURE CASES.
CYLINDER HEAD CAME OFF IN-FLIGHT 2-28-97. NO DAMAGE OR OIL LEAK OCCURRED. REPLACED CYLINDER.
NOSE LANDING GEAR TIRE BLOWN ON LANDING. NO OTHER TROUBLE.
