BHM-FLIGHT 4369, THE CREW REPORTED SMELL OF SMOKE AND VISIBLE SMOKE ON SIDEWALL LIGHT PANEL 20A WITH A VISIBLE BURN MARK. THE AIRCRAFT LANDED IN BHM WITHOUT INCIDENT WHERE MAINTENANCE INSPECTED THE AIRCRAFT. DEACTIVATED LIGHT ASSEMBLY AT SEAT 20A AND PLACE IN MEL 33-20-00-2 AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE. THIS SDR FILED OPEN PENDING FINAL CORRECTIVE ACTION.

THE LT FWD EMERGENCY LIGHT ON PANEL 191 WAS FOUND TO BE INOPERATIVE. MAINTENANCE R & R THE NR 1 EMERGENCY LIGHT POWER SUPPLY, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
ACFT IS IN FOR A C-CHECK INSPECTION. INSPECTION FOUND THE VENTRAL TANK CHAFE AT A PREVIOUS LOWER SKIN PATCH BETWEEN TANK DRAINS. THE OLD DOUBLER HAS BEEN REMOVED AND BLENDED SMOOTH, FABRICATED A NEW DOUBLER FOLLOWING THE INSTRUCTIONS OUTLINED ON EC 2015-145-120319-A, ALSO, INITIATED A DETAILED INSPECTION EVERY 30,000 CYCLES. OPS AND LEAK CHECKS WERE GOOD AND WILL BE RELEASED AT THE END OF THE CHECK. ALL WORK WAS ACCOMPLISHED IAW WITH EC 2810-01082.

THE CREW REPORTED SMOKE IN THE COCKPIT AND CABIN IN FLIGHT. THE ACFT RETURNED TO DEPARTURE AND LANDED WITHOUT FURTHER INCIDENT. MX INSPECTED AND REPLACED THE NR 1 ENGINE. OPS CHECKED GOOD.
DURING MX INSPECTION, THE UPPER ATTACH AXLE FOR THE LT MLG SHOCK ABSORBER WAS FOUND TO BE CRACKED. MX R & R THE AFFECTED AXEL, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING SCHEDULED OVERNIGHT MX THE GALLEY DOOR LOWER EMERGENCY EXIT SIGN WAS FOUND TO BE INOPERATIVE. MX RESET AND PERFORMED AN OPS CHECK OF THE GALLEY DOOR LOWER EXIT SIGN IAW AMM 33-50-09. ACFT WAS APPROVED FOR RETURN TO SERVICE.
ORD FLT 5959 THE CREW REPORTED A BLEED 1/2 LOW TEMPERATURE MESSAGE ON THE EICAS DURING CLIMBOUT. THE AIRCRAFT RETURNED TO ORD AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED AND RE-SECURED THE NR 1 FAN AIR VALVE CLAMP. OPERATIONAL CHECKS WERE GOOD.
FLT 6091, CREW REPORTED A BLEED TWO OVERTEMP INDICATION IN ICING CONDITIONS. THE ACFT RETURNED TO WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE NR 2 HIGH TEMP THERMOSTAT, OPERATIONALLY TESTED WITH NO DEFECTS, AND ACFT APPROVED FOR RETURN TO SERVICE.
TYS - A/C WAS EN ROUTE TO ORD FROM TYS WHEN CREW GOT A MESSAGE "BLEED 2 OVERTEMP" DURING CLIMBOUT. A/C RETURNED TO TYS AND EXCHANGED PLANE WITH A/C124. MAINTENACE REMOVED AND REPLACED THE NR 2 PRECOOLER IAW EMB 145XR AMM 36-11-04. OPS CHECK AND LEAK CHECK GOOD AND WAS RELEASED. ON LOGPAGE 2756199.
LNK FLT 6177 - THE CREW REPORTED A LANDING GEAR AIR/GROUND FAIL MESSAGE AFTER TAKEOFF. THE AIRCRAFT RETURNED TO LNK AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED AND REPLACED THE LANDING GEAR ELECTRONIC UNIT. OPERATIONAL CHECKS WERE GOOD.
CREW REPORTED AN AIR GROUND FAIL MESSAGE ON EICAS WHEN LOWERING THE LANDING GEAR. THE AIRCRAFT LANDED AT IAH WITHOUT INCIDENT WHERE MAINTENANCE REMOVED AND REPLACED THE SEAL IN THE NOSE LANDING GEAR SHOCK STRUT, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
FLIGHT 2511 - EWR - FLIGHT CREW REPORTED LANDING GEAR AIR GROUND FAIL MESSAGE ON EICAS. EWR MX APPLIED BREAK OUT BOX AND PERFOREMD OPERATIONAL CHECK BUT COULD NOT DUPLICATE THE MESSAGE. A CFT WAS APPROVED TO RETURN TO SERVICE.
FLIGHT 3145, THE CREW REPORTED THE ENGINE ONE HP VIBRATION IN AMBER AT THRUST SET DURING CLIMB. THE AIRCRAFT RETURNED TO IAH WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE NUMBER ONE ENGINE, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE. FINAL DETERMINATION OF A SPECIFIC DEFECTIVE PART COULD NOT BE MADE DURING ENGINE MAINTENANCE VISIT EVEN THOUGH HIGH VIBRATION WAS CONFIRMED AT THE SHOP.
EWR - CREW REPORTED SMOKE IN CABIN AND COCKPIT AFTER T/O AND RETURNED TO GATE. THE A/C WAS MX FERRIED TO TYS FOR REPAIR. TYS REMOVED AND REPLACED THE NR 1 ENGINE AND THE LT PACK CONDENSER/MIXER. ALL OPS AND LEAK CHECKS GOOD AND PROBLEM WAS RESOLVED.

TYS - DURING A CURRENT 3C INSPECTION THE BONDIND STRIP ON PANEL 197CL WAS FOUND CORRODED. MAINTENANCE REPAIRED THE PANEL IAW EMB 135/145 EC 5750-01315 AND PAINTED IAW EMB 145 XR AMM 2 51-21-02. ALL WORK WAS ACCOMPLISHED AND AIRCRAFT WILL BE RELEASED AT THE END OF THE CHECK.
TYS - DURING A CURRENT 3C INSPECTION PANEL 197 CL WAS FOUND CORRODED AT THE BONDING STRAP. MAINTENANCE REPAIRED THE PANEL IAW EMB 135/145 EC 5750-01315 AND PAINTED IAW EMB 145XR AMM 51-21-02. ALL WORK WAS ACCOMPLISHED AND AIRCRAFT WILL BE RELEASED AT THE END OF THE CHECK.
TYS - DURING A CURRENT 2C INSPECTION THE VENTRAL TANK FWD CENTER BRACKET WAS FOUND CORRODED. MAINTENANCE CUT OUT THE DAMAGE AREA, FAB A FILLER, SHIM AND EXTERNAL DOUBLER. APPLIED SEALANT AND ANTI-BACTERIAL COMPOUND PLUS A FUEL BARRIER SEALANT. LEAK CHECK GOOD. WORK WAS ACCOMPLISHED IAW EC 2810-01032. AIRCRAFT WILL BE RELEASED AT THE END OF THE CHECK.
AUS - DURING SERVICE CHECK THE UPPER EMERGENCY EXIT LIGHT ON THE RIGHT HAND SIDE WAS FOUND TO BE INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE AFFECTED SIGN, OPERATIONALLY CHECKED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
FLT 2496 - EWR - CREW REPORTED ENGINE NR 1 SMOKE COMING OUT FROM THE EXHAUST AND CONTINUING FOR 15 MINUTES AFTER SHUT DOWN ON TAXI IN. EWR MX PERFORMED OPERATIONAL CHECK OF NR 1 ENGINE (HIGH POWER RUN) AND COULD NOT DUPLICATE THE PROBLEM. ACFT WAS APPROVED TO RETURN TO SERVICE.
FLT 2287, CREW REPORTED COCKPIT SMELLS OF SMOKE WHEN PACK NR 2 IS SELECTED. MX INSPECTED APU AND FOUND SIGNS OF EXCESSIVE OIL SERVICE. MX CLEANED APU AREA AND PERFORMED OPS CHECK OF PACK NR 2. OPS CHECK GOOD, NO SIGNS OF SMOKE FOUND.
SHV - DURING C-CHECK THE SHEAR CLIP ON FRAME 68 BETWEEN STRINGERS 15 AND 16L WAS FOUND TO BE CORRODED. MAINTENANCE REMOVED THE CORROSION AND INSTALLED A DOUBLER REPAIR IAW EC5330-01458. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
SHV - DURING C-CHECK THE SHEAR CLIP ON FRAME 65 BETWEEN STRINGERS 15 AND 16L WAS FOUND TO BE CORRODED. MAINTENANCE REMOVED THE CORROSION AND INSTALLED A DOUBLER REAPAIR IAW EC5330-01458. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
CLE - THE CREW REPORTED THE AURAL WARNING SOUNDED CONTINUOUSLY IN FLIGHT AT ALL ALTITUDES UNTIL THE LANDING GEAR WAS LOWERED. MAINTENANCE FOUND AND REPAIRED CHAFED WIRES IN THE CENTER PEDESTAL JUST AFT OF THE FLAP SELECTOR HANDLE. OPERATIONAL TESTS OF THE AURAL WARNING SYSTEM AND THE LANDING GEAR SYSTEM WERE GOOD.
CREW REPORTED, FLIGHT ATTENDANT SMELLED ELECTRICAL SMOKE FROM GALLEY LIGHT CIRCUIT BREAKER. MAINTENANCE INSPECTED COFFEE POT(S) SOCKET AND GALLEY LIGHT ASSEMBLY, NO FAULT FOUND. OPS CHECKED GOOD. AIRCRAFT RELEASED FOR SERVICE.
