FLIGHT 4425, DURING TAXI THE CREW REPORTED A PITCH TRIM RUNAWAY FOLLOWED BY PITCH TRIM F/O SWITCH FAIL MESSAGE. THE AIRCRAFT RETURNED TO THE GATE WHERE MAINTENANCE REMOVED AND REPLACED THE F/O'S PITCH TRIM SWITCH, OPERATIONALLY TESTED AITH NO DEFECTS AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
THE CREW REPORTED NOSE GEAR INDICATION SHOWING RED WITH MAIN GEAR SHOWING GREEN. ABNORMAL GEAR EXTENSION PERFORMED PER QRH. AIRCRAFT DIVERTED INTO CLE AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED AND REPLACED THE LANDING GEAR SOLENOID VALE IAW 32-33-04. OPERATIONAL CHECKS WERE GOOD.
ACFT WAS FOUND SMOKING ON SHUTDOWN, ON FURTHER INSPECTION WITH A BOROSCOPE, FOUND ENGINE TO BE POOLING OIL AND THE DETERMINATION TO REMOVE ENGINE WAS MADE. THE #2 ENGINE WAS REMOVED AND REPLACED IAW WITH TASK CARD 6271001 AND ENGINE CHANGE NOTIFICATION GR 212167022518. LEAKS AND OPS CHECKS WERE GOOD AND A/C WAS RELEASED BACK TO SERVICE. .
PILOT REPORTED "STRONG OIL ODOR DURING CLIMB AND DESCENT WHILE PACK 1 WAS BEING POWERED BY BLEED 1. ODOR WAS MOST NOTICABLE IN THE COCKPIT". MX PERFORMED STEPS PER FIM 21-20-00 WITH NO OIL, SMOKE SMELL NOTICED. NO EXCESSIVE OIL CONSUMPTION NOTED. A/C WAS APPROVED FOR RETURN TO SERVICE.
FA REPORTED STRONG ELECTRICAL ODOR MID CABIN. NO SMOKE PRESENT. DIVERTED. MX PERFORMED FIM TASK 21-20-810-801A AND COULD NOT DUPLICATE ODOR. RAN BOTH ENGINES AT HIGH POWER WITH PACKS ON NO DEFECTS NOTED. ACFT WAS APPROVED FOR RETURN TO SERVICE.
BURNING ODOR REPORTED IN CABIN. R & R CABIN LIGHTING CONTROL MODULE WITH OPS CHECK GOOD. ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING ROUTINE MAINTENANCE THE NR 1 EMERGENCY LIGHT BATTERY WAS FOUND INOPERATIVE. MAINTENANCE REPLACED THE NR 1 EMERGENCY LIGHT BATTERY. OPS CHECKS WERE GOOD.
DURING PREFLIGHT INSPECTION, THE CREW REPORTED THAT THE EMERGENCY LIGHT SIGN, CAPT SIDE, NEAR WING EXIT, LOWER WAS INOPERATIVE. MAINTENANCE RESET THE EMERGENCY LIGHT SYS, OPS TESTED WITH NO DEFECTS NOTED, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING CREW INSPECTION THE FWD EXIT SIGN WAS FOUND TO BE INOPERATIVE. MAINTENANCE R & R THE NR 6 INVERTER, OPERATIONALLY TESTED WITH NO DEFECTS NOTED, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING ROUTINE MAINTENANCE, THE LT AFT OVERWING EMERGENCY LIGHT WAS FOUND INOPERATIVE. MAINTENANCE INSPECTED AND REPLACED THE LAMP. OPS CHECKED GOOD.
THE CREW REPORTED TAHT THE CAPTAIN`S MFD WENT BLANK FOLLOWED BY AN ELECTRICAL SMOKE ODOR. THE ACFT LANDED WITHOUT INCIDENT AND MAINTENANCE INSPECTED THE ACFT, FOUND THE CAPTAIN`S MFD CONNECTOR TO BE LOOSE, SECURED THE MFD, OPS TESTED WITH NO DEFECTS, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK, THE CARGO DOOR FORWARD FRAME WAS FOUND TO BE CORRODED. MAINTENANCE REMOVED THE CORROSION AND PERFORMED DOUBLER REPAIRS IAW EC5230-01342. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK THE SERVICE DOOR CUTOUT AFT SILL WAS FOUND TO BE CORRODED. MAINTENANCE R & R THE SILL IAW SRM 51-40-02. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK, THE RT ELEVATOR OTBD HINGE FITTING WAS FOUND TO BE WORN AND WOULD NOT RETAIN THE HAT BUSHINGS. MAINTENANCE R & R THE FITTING IAW SRM 51-40-02. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK THE RT OMEGA BEAM WAS FOUND TO BE CORRODED AT FRAMES 18 AND 23. MAINTENANCE REMOVED THE CORROSION AND PERFORMED DOUBLER REPAIRS IAW SRM 53-21-10 AND EC5320-01965. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
CREW REPORTED DURING FLIGHT, THE BAGGAGE FIRE EXTINGUISHER LIGHT ON THE OVERHEAD PANEL CAME ON. NO SMOKE IN BAGGAGE COMPARTMENT. MAINTENANCE PEFORMED AN OPS CHECK OF THE FIRE DETECTION TEST ON THE OVERHEAD PANEL. NO DEFECTS NOTED. OPS CHECKED GOOD. ACFT WAS APPROVED FOR RETURN TO SERVICE.

CREW REPORTED A BAGGAGE SMOKE EICAS MESSAGE (NO SMOKE NOTICED) WITH ASSOCIATED BAGGAGE EXT INDICATION LIGHT ON OVERHEAD PANEL STAYING ON. DURING INSPECTION OF ACFT THE MX PANEL DOOR WAS FOUND BROKEN AND THE PILOTS PERFORMANCE MANUAL WAS IMPACTING THE SMOKE SYS TEST TOGGLE SWITCH. MX ADJUSTED DOOR AND REMOVED PILOTS MANUAL FROM AREA AND PERFORMED A SUCCESSFUL OPS CHECK OF THE BAGGAGE SMOKE DETECTION SYS. ACFT WAS APPROVED FOR RETURN TO SERVICE.
GOUGE ON FUSELAGE BETWEEN FR 19 AND FR20 AND STRINGER 11L AND 12L. DIMENSION .055 IN DEEP AND .768 IN LENGTH. MX APPLIED EXTERNAL DOUBLER REPAIR TYPE "B" IAW SRM 53-21-01 AND ETD 2012-017648.
STRONG ODOR OF OIL FROM PACKS DURING ENTIRE FLIGHT MX PERFORMED HIGH POWER GROUND ENGINE RUNS WITH CREW WITH NO ABNORMAL OR OILY ODORS NOTED AT THAT TIME. ACFT WAS APPROVED FOR RETURN TO SERVICE.
BOTTOM CENTER OF VENTRAL TANK HAS AREA OF CHAFING. DAMAGED AREA FOUND TO BE WITHIN LIMITS. BLENDED CHAFED AREA, RESTORED TO ORIGINAL FINISH. CLOSED.

ORD FLT 6033 - THE CREW REPORTED WALL PANEL 5A WAS EXTREMELY WARM TO TOUCH, STRONG SMELL OF BURNING PLASTIC. ALL DISSIPATED WHEN PACK 2 BLEED WAS DESELECTED. MAINTENANCE INSPECTED AND RESECURED THE AIR CONDITION DUCT BEHIND SIDEWALL PANEL. OPERATIONAL CHECKS WERE GOOD.
FLT 5917 IAD CREW REPORTED BLEED VALVE NR 2 FAIL ON EICAS DURING TAKEOFF ROLL. IAD MX REMOVED AND REPLACED NR 2 ENGINE BLEED VALVE IAW EMB145 AMM 36-11-05 WITH OPERATIONAL CHECK SUCCESSFUL. ACFT WAS APPROVED FOR RETURN TO SERVICE.
CLE - FLT 2635 THE CREW REPORTED A ELECTRICAL SMELL IN FLIGHT. DESELECTED THE RECIRCULATION FAN AND SMELL WENT AWAY. MAINTENANCE INSPECTED AND PERFORMED OPERATIONAL CHECKS. NO SMELLS OR ODORS WERE DETECTED. THE AIRCRAFT WAS RELEASED FOR SERVICE.
EWR - FLT 2563 THE CREW REPORTED A BLEED 2 OVERTEMP MESSAGE ON THE ECIAS ON CLIMBOUT. THE AIRCRAFT DIVERTED INTO CLE AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED AND REPLACED THE NR 2 HIGH STAGE VALVE PRESSURE SWITCH AND THE NR 2 FAN AIR VALVE. OPERATIONAL CHECKS WERE GOOD.
ORD - FLT 6021 THE CREW REPORTED AN APU FIRE MESSAGE ON THE GROUND AT THE GATE. MAINTENANCE INSPECTED FOR SIGNS OF FIRE. NO FUEL OR OIL LEAKS WERE NOTED AND NO BURNING OR SMOKE SMELLS WERE FOUND. FOUND APU INLET WAS CLOGGED WITH ICE. REMOVED ICE AND PERFORMED OPERATIONAL CHECKS AND NO DEFECTS WERE FOUND.

TYS - NR 1 ENGINE INLET HAS CORROSION INSIDE FASTENER HOLES. REAMED AND INSTALLED BUSHING ON NR 1 ENG INLET FASTENER HOLES TO REMOVE CORROSION.
IAH - FLIGHT 700 - THE CREW REPORTED A STRONG CHEMICAL SMELL NOTED DURING DESCENT THROUGH 20,000 FEET. THE AIRCRAFT LANDED AT IAH WITHOUT INCIDENT WHERE MAINTENANCE INSPECTED THE AIRCRAFT, PERFORMED ENGINE RUNS WITH PACKS ON, ALL WITH NO DEFECTS NOTED, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.

SCHEDULE MX - SVC/RTN - ONT - DURING OVERNIGHT INSPECTION MX PERSONNEL FOUND EXIT LIGHT BY LEFT EXIT WINDOW WON'T LIGHT UP. MX PERSONNEL REMOVED AND REPLACED AFT FLOOR EMERGENCY BATTERY PACK AND PERFORMED OPERATIONAL CHECKS SUCCESSFULLY. THE A/C WAS APPROVED TO RETURN FOR SERVICE.
CLE FLIGHT 3173 - THE CREW REPORTED A LANDING GEAR/ AIR GROUND FAIL MESSAGE WHEN GEAR RETRACTED. THE AIRCRAFT RETURNED TO CLE AND LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE INSPECTED THE NOSE LANDING GEAR AND REPACKED THE NOSE STRUT. OPERATIONAL CHECKS WERE GOOD.

MAINTENANCE REMOVED AND REPLACED SHORTED FIRST OFFICER'S MAP HOLDER IAW EMB145 WIRING MANUAL 33-11-50, OPS CHECKED GOOD. MEL CLEARED AND AIRCRAFT RETURNED TO SERVICE.
IAH - FLIGHT 5884 THE CREW REPORTED AN ELECTRICAL BURNING ODOR IN THE COCKPIT AT FLIGHT LEVEL 340 FOR APPROXIMATELY 1.5 MINUTES WITH NO VISIBLE SIGNS OF SMOKE, AND C/B H11 POPPED. THE AIRCRAFT LANDED AT IAH WITHOUT INCIDENT WHERE MAINTENANCE INSPECTED THE AIRCRAFT AND PLACED THE COCKPIT READING LIGHTS ON MEL 33-10-00 CATR -C- CONTROL 105910 AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
