FOUND THE AIR START VALVE ACCESS PANEL AND THE OIL SERVICE PANEL WERE LEFT OPEN IN FLIGHT. NO DAMAGE WAS OBSERVED ON THE AIR START VALVE ACCESS PANEL AND THE PANEL WAS SECURED CLOSED. THE OIL SERVICE DOOR HINGE WAS FOUND BENT AND DISTORTED. FOUND 3 AREAS OF SCRATCHES/CRACKS ON THE LOWER COWL CAUSED BY THE OIL SERVICE DOOR. EACH SECTION OF DAMAGE ON THE LOWER COWL WAS NO MORE THAN 3 INCHES IN LENGTH AND FURTHER THAN 6 INCHES FROM EACH OTHER. DURING THE ATTEMPT TO RESECURE THE OIL SERVICE DOOR, THE RIVETS WERE REMOVED FROM THE OIL SERVICE DOOR HINGE. REMOVED AND REPLACED E1 LOWER COWLING.

MAINTENANCE INSPECTED THE AIRCRAFT FOUND NO EVIDENCE OF SMOKE OR FIRE, AND REMOVED AND REPLACED BOTH SMOKE DETECTORS, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH FLT 2568 THE CREW REPORTED ON CLIMB THRU FL340 TO FL350 THE NR 1 ENGINE ITT SPIKED INTO THE AMBER RANGE REACHING 917. PULLED BACK AND THE ITT REDUCED AND THEN THE POWER WAS ADVANCED AND ALL INDICATIONS WERE NORMAL. EICAS DISPLAYED WAS E1 EXCEEDANCE. MAINTENANCE INSPECTED AND REPLACED THE NR 1 ENGINE COMPRESSOR BLEED VALVE. OPERATIONAL CHECKS WERE GOOD.
TYS - A/C EN ROUTE FROM MSY TO EWR WHEN CREW STARTED HAVING ISSUES WITH PRESSURIZATION. DIVERTED INTO TYS WHERE MAINTENANCE FOUND THE AC CONNECTION BLOWN OFF. MAINTENANCE RE-SECURED THE DUCT, OPS CHECK GOOD. A/C CONTINUED ON TO EWR AT 13:16.
IAH - FLIGHT 2617, DURING CLIMBOUT OF IAH THE CREW REPORTED A BLEED 1 LEAK INDICATION. THE AIRCRAFT RETURNED TO IAH WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE O-RINGS ON THE NUMBER ONE COUPLING. LEAK CHECKED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
TYS - A/C WAS IN FOR A-CHECK WHEN THE EXIT SIGN AT ROLL 12A WAS FOUND TO BE INOP. MAINTENANCE REMOVED AND REPLACED THE EXIT SIGN IAW EMB 145XR AMM 33-50-10. OPS CHECK GOOD AND WAS RELEASED.
DURING ROUTINE MAINTENANCE THE EMERGENCY EXIT SIGN IN FRONT OF SEAT 3B WAS FOUND INOPERATIVE. MAINTENANCE INSPECTED AND REPLACED THE NR 3 EMERGENCY LIGHT INVERTER. OPERATIONAL CHECKS WERE GOOD.
TUS - FLIGHT 2630, DURING PREFLIGHT INSPECTION THE CREW REPORTED AN EMERGENCY EXIT LOCATOR AT THE EXIT ROW TO BE INOPERATIVE. CONTRACT MAINTENANCE RE-SECURED THE WIRING TO THE EXIT IDENTIFIER, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH - FLIGHT 2010, DURING PRE-FLIGHT INSPECTION THE CREW REPORTED AN EXIT ROW CEILING LIGHT TO BE INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE AFFECTED CEILING LIGHT, OPERATIONALLY TESTED WITH NO DEFECTS. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
MCI - FLIGHT 2669, DURING POST-FLIGHT INSPECTION THE CREW REPORTED THAT THE AFT EMERGENCY EXIT SIGN WAS FOUND TO BE INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE AFFECTED SIGN, OPERATIONALLY TESTED WITH NO DEFECTS NOTED, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
CLE FLT 2101 THE CREW REPORTED THE OVERHEAD EMERGENCY EXIT SIGN WAS INOPERATIVE. MAINTENANCE INSPECTED AND PERFORMED OPERATIONAL CHECKS AND NO DEFECTS WERE FOUND.
RIC MX - DURING ROUTINE MAINTENANCE, MX FOUND ROW 12 OVERHEAD FWD FACING EXIT SIGN IS INOPERATIVE. MX REMOVED AND REPLACED INVERTER AND PERFORMED OPERATIONAL CHECK SUCCESSFULLY. ACFT WAS APPROVED TO RETURN TO SERVICE.
ALB FLT 2983 - THE CREW REPORTED SMOKE HAZE IN THE COCKPIT SHORTLY AFTER TAKEOFF THEN DISSIPATED. ALSO REPORTED A LAV SMOKE WARNING MSG ON THE EICAS FOR ABOUT A MINUTE. MAINTENANCE INSPECTED AND REPLACED THE NR 1 ENGINE. THE TEAR DOWN REPORT FROM ROLLS ROYCE INDICATED THE PRIMARY CAUSE OF SMOKE/HAZE IN THE COCKPIT WAS THE NR 4 CARBON SEAL. THE ENGINE WAS REPAIRED TO SERVICEABLE CONDITION.
IAH - FLIGHT 2528, DURING PREFLIGHT THE CREW REPORTED THE SERVICE DOOR EXTERNAL EMERGENCY LIGHT TO BE INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE LIGHT BULB, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
FLIGHT 2245 - EWR - FLIGHT CREW REPORTED RIGHT WING LEAKING FUEL. CREW PERFORMED AIR RETURN AND LANDED ACFT UNEVENTFULLY. EWR MX REMOVED AND REPLACED FLAPPER VALVE AND PERFORMED OPERATIONAL CHECK SUCCESSFULLY. ACFT WAS APPROVED TO RETURN TO SERVICE.
FLT 2925 - EWR - FLIGHT CREW REPORTED A LOUD POP FOLLOWED BY A VIBRATION FROM THE NOSE OF THE AIRCRAFT DURING CLIMBING. CREW PERFORMED AIR RETURN AND LANDED AIRCRAFT UNEVENTFULLY. EWR MX FOUND GPU DOOR OPEN AND LATCH BROKEN. MX REMOVED AND REPLACED GPU DOOR COVER ASSEMBLY AND PERFORMED FUNCTIONAL CHECK SUCCESSFULLY. AIRCRAFT WAS APPROVED TO RETURN TO SERVICE.
IAH - FLIGHT 3020, THE CREW REPORTED A BURNING SMELL AFTER THE APU START. THE APU THEN FAILED AND WOULD NOT RESTART. THE AIRCRAFT RETURNED TO THE GATE WHERE MAINTENANCE PLACED THE APU ON MEL 49-00-00 CAT - C CTL NUMBER 157685, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE. THE AIRCRAFT RETURNED TO IAH ON 11/04/08 WHERE MAINTENANCE REMOVED AND REPLACED THE APU STARTER GENERATOR, OPERATIONALLY CHECKED WITH NO DEFECTS, REMOVED THE MEL, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.

ON 10/28/08 THE AIRCRAFT RETURNED TO IAH WHERE MAINTENANCE REMOVED AND REPLACED ELECTRIC FUEL PUMP 2B, OPERATIONALLY CHECKED WITH NO DEFECTS, CLEARED THE MEL, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.

MAINTENANCE OPERATIONALLY CHECKED THE EMERGENCY LIGHTING WITH NO DEFECTS NOTED AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE. DURING POST FLIGHT INSPECTION ON FLIGHT 7672 IN MCI, THE DISCREPANCY REPEATED. MAINTENANCE REMOVED AND REPLACED THE NR 6 EMERGENCY LIGHT INVERTER, OPERATIONALLY CHECKED WITH NO DEFECTS, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE. (S)

OMA FLIGHT 2078, CREW REPORTED ON T/O ROLL EMERGENCY EXIT OPEN LIGHT INDICATION CAME ON. THE INDICATION WAS THE LEFT WING EMERGENCY EXIT. LIGHT WENT OUT IN FLIGHT. THE EXIT DOOR WAS VERIFIED CLOSED WITH NO PRESSURIZATION LOSS. MAINTENANCE INSPECTED AND RELEASED A/C IAW 52-70-00/3. THE LEFT EMERGENCY EXIT DOOR MICROSWITCH WAS ADJUSTED AND OPERATIONAL CHECKS WERE GOOD. THE AIRCRAFT WAS RELEASED FOR SERVICE.
