PILOT REPORTED DURING A MX CHECK FLIGHT THAT THE GEAR REMAINED DOWN WITH ALL THREE GREEN LIGHTS ILLUMINATED WHEN THE GEAR HANDLE WAS MOVED TO THE UP POSITION. ACFT RETURNED TO DEPARTURE UNEVENTFULLY. MX TROUBLESHOT THE GEAR SYS AND THE FOLLOWING MX WAS PERFORMED. TROUBLESHOOT AND FOUND BROKEN WIRE ON LANDING GEAR SWITCH (RT SIDE). R & R RT SIDE LANDING GEAR SWITCH IAW 32-30-00 GEAR PROBLEM STILL NOT CORRECTED. TROUBLESHOOT PRINTED CIRCUIT BOARD (TIME DELAY). R & R PCB PROBLEM STILL WAS NOT CORRECTED. TROUBLESHOOT ALL WIRING IN LANDING GEAR SYS AND FOUND BAD GROUND TO HYD POWER PACK ASSEMBLY -GROUND CABLE. CLEANED AND TIGHTENED POWER PACK GROUND CABLE AND PERFORMED NUMEROUS OPERATIONAL AND FUNCTIONAL CHECKS TO INCLUDE EMERGENCY EXTENSION/ MANUAL SYS CHECKS. ALL OPS AND FUNCTIONAL CHECKS WERE GOOD. OPS CHECK FLIGHT COMPLETED AND NO FURTHER DEFECTS FOUND OR NOTED. ACFT RETURNED TO SERVICE.
AFTER TAKEOFF, THE LANDING GEAR WOULD NOT RETRACT -COME UP-AFTER CYCLING THE GEAR STILL WOULD NOT COME UP AND WAS STILL INDICATING 3 GREEN IN THE DOWN POSITION. CREW ADVISED THE TOWER AND DECLARED AN EMERGENCY TO LAND. ACFT LANDED WITH NO FURTHER INCIDENT. MX TROUBLESHOT LANDING GEAR POWER AND FOUND CIRCUIT BREAKER POPPED. REST CIRCUIT BREAKER AND PERFORMED LANDING GEAR SWING MULTIPLE TIMES. ALSO, PERFORMED MANUAL AND EMERGENCY EXTENSION OPS CHECKS- GOOD. WORK DONE IAW MM 32-30-00 TROUBLESHOOTING CHART. ALSO INSPECTED AND CHECKED WIRING FROM LANDING GEAR CIRCUIT BREAKER TO PUMP MOTOR. INSPECTED LT NACELLE AFT ELECTRICAL BAY AND FOUND TO BE SERVICEABLE IAW MM 24-30-00. PERFORMED VISUAL INSP OF POWER WIRE FROM PUMP TO CIRCUIT BREAKER AND FOUND POWER WERE GOOD IAW MM 32-31-01. PERFORMED INSP OF A316 PCB BOARD AND FOUND NO EVIDENCE OF BURNING OR ARCING IAW MM 24-30-00. PERFORMED INSP OF TRIPLE FED BUSS DIODE SYS AND OPS CHECK IAW MM 24-50-00 - ALL CHECKS WERE GOOD. PERFORMED ENGINE RUN ON ACFT WITH FULL ELECTRICAL LOAD IAW MM 24-50-00. ACFT OPS CHECK FLIGHT PERFORMED. OPS CHECK OF GEAR PERFORMED AND CYCLED MULTIPLE TIMES ALL OPS CHECKS WERE SATISFACTORY.
AFTER ROTATION, CAPTAIN REPORTED AN UNCOMMANDED RT ENGINE AUTO FEATHER. THE ACFT RETURNED TO THE AIRPORT WITHOUT FURTHER INCIDENT ARRIVING BACK IN THE BLOCKS FOR MX TROUBLESHOOTING AND EVALUATION AT 1355Z. MX TROUBLESHOT ACFT AND FOUND BETA VALVE OUT OF ADJUSTMENT AND BETA CABLE LOOSE. MX RE-ADJUSTED BETA VALVE AND RE-RIGGED BETA CABLE. PERFORMED GROUND PERFORMANCE PROCEDURE FROM CHART 2 TORQUE GRAPH N /TEST SWITCH IN LOW PITCH AND CHART 3 TORQUE GRAPH N/TEST SWITCH OFF. ALL PARAMETERS ARE WITHIN LIMITS. WORK COMPLETED IAW MM 79-00-00. OPS CHECK FLIGHT PERFORMED TO VERIFY PROPER OPERATION OF NR 2 PROPELLER AND ENGINE IN ALL PHASES OF FLIGHT-SPECIFICALLY TAKEOFF ROLL, ROTATION AND CLIMBOUT. OPS CHECK FLIGHT COMPLETED, PERFORMED TOTAL OF 4 TAKEOFFS EACH WITHOUT ANY DISCREPANCIES, OPS CHECK FLIGHT SATISFACTORY.
PILOT REPORTED TO MX AFTER LANDING, THAT A BURNING PLASTIC/RUBBER SMELL IN CABIN WITH BLEEDS ON, CABIN TEMP SET TO COLD. MX TROUBLESHOT ACFT AND FOUND RUBBISH UNDER PILOT SEAT AIR VENT. REMOVED RUBBISH AND CLEANED AIR VENT IAW MM 12-20-00. MX OPS CHECKS PERFORMED WITH BLEED SYSTEM ON, NO SMELL OR ANY ODORS NOTED.
ON ROTATION RT ENG TORQUE WENT TO 4000 LBS. FOR APPROXIMATELY 2 SECONDS. ENG STARTED TO AUTOFEATHER & PILOT PULLED THROTTLE BACK & SHUTDOWN ENG. PERFORM A BORESCOPE INSPECTION OF ENGINE, NO DEFECTS NOTED. PERFORMED REMOVAL & INSPECTION OF OIL FILTER & HOUSING ASSY. NO DEFECTS NOTED. PERFORMED COMPRESSOR TURBINE WASH. PERFORMANCE & GROUND CHKS PERFORMED IAW MM, OPS CHK GOOD, NO LEAKS NOTED. REMOVED & INSPECTED RGB SCREEN FOR CONDITION & BLOCKAGE, NO DEFECTS NOTED. REMOVED & INSPECTED OIL FILTER & FILTER BOWL FOR EVIDENCE OF COKE/SLUDGE PER MM, NO DEFECTS NOTED. REMOVED & INSPECTED RGB CHIP DETECTOR IAW MANUAL, NO DEFECTS NOTED. INSPECTED FRICTION LOCK ON ENG & PROP CONTROLS, NO DEFECTS NOTED. VERIFIED MAX NP WITHIN LIMITS, NO DEFECTS NOTED. PERFORMED OVERSPEED GOVENOR OPS CHK, NO DEFECTS NOTED. INSPECTED FEATHER ACTUATION POINT ON PROP LEVER-OPS CHECK GOOD.
DURING PREFLIGHT INSPECTION, CREW REPORTED A CRACK ON THE AIRSTAIR DOOR NEAR THE HINGE. MX FOUND A CRACK ON THE INNER DOOR SKIN NEAR THE AFT END DOOR HINGE. UPON INITIATION OF A REPAIR TO AFFECTED AREA, MX FOUND CORROSION ON THE INNER DOOR SKIN, CLOSURE AND METAL HONEY COMB CORE COMPONENTS THAT EXCEEDED LIMITS. BASED ON INFORMATION PROVIDED TO THE AIRCRAFT MFR, MX REPAIRED THE DAMAGED AREA OF THE DOOR IAW FIELD REPAIR FR-CO-01572, REV. 2. ISSUED 8/6/07. 8/16/07 UPON INSTALLATION OF THE REPAIRED DOOR, IT WAS FOUND THAT THE REPAIR DEFORMED THE CONTOUR OF THE DOOR PREVENTING PROPER INSTALLATION. THE REPAIRED DOOR WAS DETERMINED TO BE BEYOND ECONOMICAL REPAIR. 8/28/07 A NEW AIRSTAIR DOOR ASSEMBLY WAS INSTALLED AFTE
(MRR NR 02-012405) LOSS OF HYDRAULIC PRESSURE AND QUANTITY LED TO THE INABILITY TO OPERATE THE LANDING GEAR WITH NORMAL PROCEDURES. LANDING GEAR HAD TO BE EXTENDED MANUALLY WITH THE HAND PUMP. NOTE: THIS WAS A PILOT TRAINING FLIGHT ONLY. FAILED LANDING GEAR HYDRAULIC SYSTEM HOSE. REMOVED AND REPLACED HYDRAULIC HOSE. (NM05200502790) (K)

MBL - AT 10,000 FEET DURING MODERATE RAIN, SMOKE IN COCKPIT WITH WINDSHIELD FUEL VENTS, ALT STATIC, PITOT AND STALL HEAT ON AND THE ENGINE ANTI-ICE ON. MAINTENANCE DEFERRED WINDSHIELD HEAT IAW MEL 30-7. FOUND RIGHT WINDSHIELD GROUND BLOCK PULLED AWAY FROM WINDSHIELD SURFACE AND ARCING WHEN POWER APPLIED. (M)

UC-122 HAD FLAPS IN TAKEOFF POSITION, GEAR DOWN AND LOCKED WITH 3 GREEN INDICATED. THE RED LIGHTS ILLUMINATED IN THE GEAR HANDLE AND AURAL WARNING SOUNDED. THE AIRCRAFT WAS FERRIED TO SPI FROM ORD FOR REPAIRS. THE GEAR SWING COMPLETED IN SPI REVEALED NO DUPLICATE PROBLEMS. (X)
OSH - CREW REPORTED THAT THE GEAR INTRANSIT LIGHT AND HORN WOULD NOT EXTINGUISH WHEN GEAR SELECTED UP. CREW ELECTED TO DO AN AIR TURN BACK AND MADE AN UNEVENTFUL LANDING. MAINTENANCE JACKED AIRCRAFT AND PEFORMED FOUR COMPLETE GEAR SWING CYCLES. SYSTEM OPS CHECKED GOOD. AIRCRAFT RELEASED FOR SERVICE. (M)
DEN - AIRCRAFT NR 159GL AND N122GL, CONTACTED ELEVATORS ON THE GROUND. MAINTENANCE REPLACED THE RIGHT ELEVATOR DUE TO TIP DAMAGE. (M)
ORD-BRL - FLT NR 5927 - THE LEFT ENGINE FAILED AND WAS SECURED. THE CREW DIVERTED TO GBC ON SINGLE ENGINE AND COMPLETED AND UNEVENTFUL LANDING. MAINTENANCE CHANGED ENGINES. SUSPECT BEARING FAILURE. (M)
SPW - DURING ROUTINE INSPECTION, MAINTENANCE FOUND DORSAL FIN CRACKED ON LT SIDE AT REAR PRESSURE BULKHEAD ATTACH POINT. THE CRACK WAS BETWEEN TWO RIVET HOLES. INSTALLED REPAIR IAW BEECH 1900 SRM 51-70-00, PAGE 17.
SPW - DURING SCHEDULED VISUAL INSPECTION REQUIRED BY AD 95-02-18 IAW BEECH SB 2255R6, AREA OF THE RT ENGINE TRUSS WAS FOUND CRACKED. REPLACED TRUSS WITH SERVICEABLE.
