Maintenance disclosures
THE FLIGHT CREW REPORTED THAT DURING AIRCRAFT POWERUP THE STANDBY ATTITUDE INDICATOR FAILED TO INITIATE. THE MAINTENANCE TEAM REMOVED THE FAILED SECONDARY FLIGHT DISPLAY AND INSTALLED A REPAIRED UNIT. OPS CHECKS WERE SATISFACTORY AND THE AIRCRAFT WAS RETURNED TO SERVICE.
FLIGHT CREW REPORTS THEY ARE SUSPECT OF AN OXYGEN LEAK AFTER O2 QUANTITY DROPPED TO 76% AFTER BEING SERVICED. THE MAINTENANCE TEAM TROUBLESHOT AND DETERMINED THAT THE PILOTS SIDE AND 3RD CREW MEMBER EMERGENCY OXYGEN MASKS WERE LEAKING. THE PILOTS SIDE MASK P/N MC10-12-151 S/N 5232 WAS OVERHAULED DEC2019 AND THE 3RD CREW MEMBER MASK P/N MC10-12-151 S/N 87101 WAS OVERHAULED FEB2019. BOTH MASKS HAVE A 6 YEAR OVERHAUL PERIOD. BOTH MASKS WERE REPLACED WITH OVERHAULED UNITS AND THE AIRCRAFT WAS RETURN TO SERVICE.
IN CRUISE, MASTER CAUTION, (LOW PRESS R) ILLUMINATED, OIL PRESSURE INDICATOR READ 20 PSI. CHECKLIST INITIATED, ENG WAS SHUTDOWN. UNEVENTFUL LANDING WAS MADE. INVESTIGATION REVEALED THAT A WASHER FOREIGN TO ENG HAD BROKEN THROUGH A SCREEN, ENTERED OIL PUMP, DAMAGEING PUMP. IT IS UNKNOWN HOW WASHER GOT INTO OIL TANK (TANK HAS A SCREEN AT FILLER OPENING). WASHER MIGRATED FROM TANK THROUGH THE OIL LINES TO A SCREEN WHICH IS PART OF THE PUMP (AND CONSEQUENTLY NOT INSPECTED UNTIL PUMP REMOVAL). THE SCREEN FAILED ALLOWING THE WASHER AND PART OF THE FAILED SCREEN TO ENTER THE PUMP AND DAMAGE THE PUMP ELEMENTS. IT IS SUSPECTED THAT THE WASHER MAY HAVE BEEN IN OIL SYS FOR SOMETIME, PERHAPS EVEN SINCE MFG. (K)
