FLIGHT CREW REPORTED SMOKE IN THE CABIN. MAINTENANCE INSPECTED THE AIRCRAFT AND FOUND THE COPILOT AVIONICS COOLING FAN HAVE FAILED. THE FAN WAS REPLACED WITH NO FURTHER DEFECTS NOTED.
FLIGHT CREW REPORTED SMOKE IN THE CABIN. MAINTENANCE INSPECTED THE AIRCRAFT AND FOUND THE COPILOT AVIONICS COOLING FAN HAVE FAILED. THE FAN WAS REPLACED WITH NO FURTHER DEFECTS NOTED.
PILOT REPORTED STRONG BURNING ODOR AND SMOKE COMING FROM THE COPILOT'S SIDE DURING CLIMB. MAINTENANCE INSPECTED AND FOUND THE ELECTRIC HEATER FAN IN THE NOSE WERE MAKING A GRINDING NOISE WHEN TURNED ON AND OFF. THE FAN WAS REPLACED WITH NO FURTHER DEFECTS NOTED.
DURING INVESTIGATION OF PILOT REPORT FOR SMOKE IN THE COCKPIT IT WAS DISCOVERED THE LEFT ENGINE RIGHT EXHAUST PIPE HAD BROKEN, WHICH CAUSED DAMAGE TO A TKS LINE AND AN ALTERNATE AIR CONTROL CABLE IN THE VICINITY. THE EXHAUST PIPE WAS REPLACED WITH NO FURTHER DEFECTS NOTED.
PILOT REPORTED "PT2 HEATER OFF CAS MESSAGE" AFTER TAKE-OFF. MAINTENANCE INSPECTED AND FOUND LEFT AND RIGHT PITOT HEAT SWITCHES AND CONNECTORS MELTED. BOTH THE SWITCH AND THE SWITCH CONNECTOR WERE REPLACED WITH NO FURTHER DEFECTS. PART NUMBERS REPLACED WERE 30-16-0687001 AND 16-705-103-13.
PILOT REPORTED âCABIN HEATER WARNING LIGHT ILLUMINATED, AND HEAT WAS VERY COLD WITH NO AIR FLOW. MAINTENANCE INSPECTED AND FOUND THAT THE HEATER CONTROL SWITCH (OEM P/N: 212-24-00-0028-000) WAS MELTED, ALONG WITH BOTH ENDS OF THE WIRING AT CONNECTOR âVVâ LOCATED BEHIND THE RH INSTRUMENT PANEL. THIS WIRING WILL BE REPAIRED WITH NEW TERMINAL ENDS AT BOTH SIDES OF CONNECTOR âVVâ AND A NEW SWITCH WILL BE INSTALLED. THE SYSTEM ALSO INCORPORATES A 25A CIRCUIT BREAKER (OEM P/N: 2TC2-25). THIS BREAKER WILL BE REPLACED OUT OF AN ABUNDANCE OF CAUTION.
THE NLG ATTACH STRUCTURE WAS FOUND TO BE DAMAGED DURING AN INSPECTION INITIATED DUE TO SIMILAR FINDINGS ON OTHER COMPANY AIRCRAFT. THE DAMAGE IS LOCATED AT FWD MOST FUSELAGE BULKHEAD, LWR CENTER SECTION, IN THE AREA CORRESPONDING TO THE NLG MOUNTS. THE LT AND RT VERTICAL STIFFENERS (PART NUMBER 212-53-11-0402-1L AND -1R) LWR 4 RIVETS ARE SHEARED AND PULLING AWAY FROM BULKHEAD. THE BULKHEAD IS CRACKED IN THESE AREAS APPROXIMATELY 1.25" EACH SIDE. THE STRUCTURE AFT OF NLG BULKHEAD WAS INSPECTED WITH THE FOLLOWING DEFECTS NOTED: 1) RIVET ROW AT FWD EDGE OF BAGGAGE COMPARTMENT FLOOR (RUNNING LT TO RT) SHEARED FASTENERS AND DISTORTION. THE FWD SECTIONS OF FLOORBOARD SUPPORTS AS THEY ARE RIVETED TO FORWARD-MOST FUSELAGE BULKHEAD SHEARED FASTENERS, DAMAGED, CRACKED, AND DISTORTED. THE OEM HAS BEEN NOTIFIED AND A FLEETWIDE INSPECTION HAS BEEN ISSUED FOR ALL COMPANY AIRCRAFT. THE ROOT CAUSE OF THIS ISSUE IS UNKNOWN AT THIS TIME. AN ATTACHMENT ILLUSTRATING THIS ISSUE WILL BE SUBMITTED AS A SUPPLEMENT TO THIS SDR.
DURING A TRAINING FLIGHT WHILE PRACTICING STALL RECOVERY, THE ELEVATOR TRIM POSITION INDICATOR MOVED 'UNCOMMANDED' TO THE FULL NOSE DOWN POSITION AND THE WEIGHT OF CONTROLS CONFIRMED THE CORRECT POSITION OF TRIM INDICATOR. THE AIRCRAFT WAS CONTROLLABLE AND LANDED SAFELY. THE AIRFRAME OEM INSTRUCTED THE REPLACEMENT OF ELEVATOR TRIM ACTUATOR WITH AND IMPROVED TRIM ACTUATOR RECPORTED IN SB-372-CS DATED 03/10/2020.
DURING CRUISE FLIGHT PFD 1 WAS INDICATION 8 KNOTS FASTER THAN PFD 2 AND PFD 2 WAS INDICATING 100 FEET HIGHER THAN PFD 1. BOTH PITOT TUBES WERE FOUND TO BE LEAKING AT THE PITOT TUBE SHELL. THE PITOT TUBES WERE REPLACED WITH NEW ASSEMBLIES AND THE AIRCRAFT WAS RETURNED TO SERVICE. THE PITOT TUBE PART NUMBERS ARE: 212-34-10-0015-001 (LEFT) AND 212-34-10-0015-002 (RIGHT).
