CREW SAID THERE WAS A SLIGHT FUEL SMELL. AIRCRAFT INSPECTED AND NO FUEL LEAK DETECTED. AIRCRAFT RETURNED TO SERVICE.
ELECTRICAL SMELL AND VISIBLE SMOKE IN CABIN AFTER AIR CONDITIONER WAS TURNED ON, WENT AWAY WHEN AIR CONDITIONER WAS TURNED OFF. REMOVED TAIL BOOM ACCESS AND BAGGAGE COMPARTMENT ACCESS PANELS, INSPECTED AIR CONDITIONER AFT BLOWER ASSEMBLY AND FOUND WIRING TO THE BLOWER FAN IN THE 2 PIN CONNECTOR OVERHEATED. REINSTALLED TAIL BOOM AND BAGGAGE COMPARTMENT ACCESS PANEL. PERFORMED GROUND RUN WITH NO DEFECTS NOTED. WORK PERFORMED IAW BHT206L1-MM CHAPTER 53 AND CABIN AIR CONDITIONING SYSTEM MAINTENANCE INSTRUCTIONS â DOC. NO. ICA-206EC270M-1.
PILOT REPORTED SEARCH LIGHT BECAME INOPERATIVE AND CIRCUIT BREAKER POPPED WHILE IN FLIGHT. CREW REPORTED FAINT ELECTRICAL BURNING ODOR AND REPORTED CABIN LIGHTS FLICKERING. UPON MAINTENANCE INSPECTION FOUND THE SEARCH LIGHT DIODE AND JUMPER TO HAVE INDICATIONS OF BURNING AND FAILURE.
DURING PREFLIGHT INSPECTION PILOT FOUND ONE OF THE MAIN ROTOR HUB PILLOW BLOCK EARS TO BE CRACKED.
ARTIFICIAL HORIZON HAS FOGGED GLASS.
BENCH CHECK REQUIRED. FOUND SELECTOR KNOBS INTERMITTENT WHEN TURNED.
TRANSCEIVER HAD INTERMITTENT OPERATION. PERFORMED PRELIMINARY INSPECTION AND FOUND UNIT HAS NO TONE DEVIATION AND NO CTCSS TONES.
DIGITS OF LED DISPLAY CHANGE WITH VIBRATION IF FACE PLATE IS PRESSED. PERFORMED PRELIMINARY INSPECTION AND FOUND BROKEN WIRE, REPAIRED WIRE, CLEANED THUMBWHEEL NO HELP. UNIT AUDIO CUTTING OUT ON ALL CHANNELS.
IN CRUISE FLIGHT, PILOT NOTICED A CHANGE IN SOUND. CONTINUED FLIGHT (7 MILES AWAY) TO DESTINATION. POST-FLIGHT INSPECTION REVEALED RT ENGINE COWL SIDE DOOR MISSING. COWLING HAD MADE MINOR CONTACT WITH ONE M/R BLADE. REF: BK-117 IPC, FIGURE 221, ITEM 10.
ROTOR STAR HAS WEAR IN ZONES 3-4. WEAR IN ZONE 2, DUE TO BEARING P/N F32827 AND BEARING P/N F32828 SPINNING IN BORE.

AFTER LEAK CHECK PER SB LT101-73-10-0124, PART II A IT WAS DISCOVERED THAT THE FUEL MANIFOLD WAS LEAKING AT ONE OF THE FUEL NOZZLES WHERE THE RIDGED TUBE IS BRAZED TO THE NOZZLE.
YAW SYSTEM SENSITIVE TO INPUT GYRO IS DEFECTIVE
DIR GYRO SLOW TO ENGAGE AND INTERMITTENTLY INOPERATIVE.
CONTROL UNIT DROPPED OFF LINE SEVERAL TIMES, WOULD NOT COME ON ONE TIME.
STATIC INVERTER BROKEN SCREWS ON MOUNT.
UNIT ROLLED TO THE INVERTED POSITION AND STAYED THERE FOR GROUND RUN AND POWER CHECK. AIRCRAFT WAS HOVERED AND TEST FLOWN FOR FURTHER OBSERVATION. GYRO ROLLED UPRIGHT BUT DID NOT REFLECT PROPER ALTITUDE.
T/R BUSHING RING STAKING CAME LOOSE. REPLACED WITH SERVICEABLE PART.
HORIZON GYRO FRONT GLASS FOGGED, SLOW OPERATION.
ARTIFICAL GYRO +5 TO 10 DEGREE CONSTANT LEFT BANK.
NO. 1 ENGINE HAD HIGH TOT. BLEED AIR VALVE FAILED.
AIRSPEED INDICATOR FAILED TO "0".

M/R BLADE HAS HOLE IN THE LEADING EDGE ABRASION STRIP NEAR THE TIP.
ACTUATOR RATCHETY AND SLOW. WOULD SOMETIMES STICK TO ONE SIDE.
AIRCRAFT ENGINE LEAK CHECK REVEALED: FUEL LEAKING FROM FUEL MANIFOLD TUBING. INSTALLED A SERVICEABLE MANIFOLD.
FUEL PUMP SEAL LEAKAGE.
WILL NOT TRANSMIT. FOUND NO 29V ON POWER SUPPLY. REPLACED U2 (IC), U1 (IC),VR1 (DIODE) AND REPAIRED POWER SUPPLY. SYNTHESIZER BOARD HAS BAD TUNING VOLTAGE. REPLACED U11 (IC), U12 (IC), U13 (IC), U9 (IC), U10 (IC). REPAIRED. FOUND WEAK RECEIVER SENSE AT 406MHZ.ALIGNED THE RECEIVER AND ADJUSTED SYNTHESIZER PRESELECTOR VOLTAGE TO SPECS, REPAIRED. FOUND TRANSMIT TONE SET A CODE 11 FOR A TONE OF 91.5HZ DRIFTING OFF FREQUENCY. REPLACED U14 (IC) ON AUDIO BOARD. REPAIRED. BENCH CHECK GOOD.
CRACKS WERE FOUND ON THE FRAME WHERE IT RIVETED TO THE TAIL CONE SKIN. CRACKS WERE ALSO FOUND UNDER THE UPPER AFT ECU MOUNT. INSTALLED REPAIR KIT P/N 117-SB20-101-3.1 ACCORDING TO SB BK117-20-101. INSTALLED A REPAIR DOUBLER ON TAIL CONE. SHIMMED ECU TO AVOID ANY PRELOADS ON MOUNTS.
COMPASS FLAG WILL NOT PULL.
UNIT SHORTED. SMOKE CAME FROM UNIT WHEN TURNED ON.
SWITCH WILL NOT CLOSE.
BROKEN EJECTION SPRING OF LATCH REQUIRES TWO HANDS TO OPERATE LATCH ASSY. REPLACED WITH SERVICEABLE LATCH ASSY AND RETURNED TO SERVICE.
