CRACK IN 145 STATION
CRACK IN R/H ROLLOVER CLOSE OUT PANEL APPROX 1 INCH LONG EXTENDING TO EDGE OF PANEL. P/N 206-031-117-443. THIS PART WILL PROBABLY HAVE TO BE REMOVED TO REPAIR THE FRAME MIGHT CONSIDER REPLACEMENT OF PANEL IF PROCURABLE.
VAN HORN MAIN ROTOR BLADE INBOARD TAB ATTACHMENT TO BLADE SURFACE HAS CRACKS AT BOTH INBOARD AND OUTBOARD ATTACH POINTS ON BOTH UPPER AND LOWER BLADE SURFACES.
DURING 300 HOUR INSPECTION, AN AMT DISCOVERED CLIP P/N 206-033-306-008 CRACKED JUST FWD OF TB ATTACH ACCESS AREA IN THE UPPER R/H QUADRANT ADJACENT TO THE TR AND ELEVATOR BELL CRANK SUPPORT FITTING.
APPROX 3 NM NW OF AIRPORT HEARD A NOISE. A FEW MOMENTS PASSED AND THEN THERE WAS A "POP", A QUICK PRESSURE CHANGE AND AIRFLOW INTO THE COCKPIT. DISCOVERED THE GAPING HOLE WHERE THE CHIN BUBBLE SHOULD BE INSTALLED. IMMEDIATELY TURNED TOWARD THE AIRPORT AND NOTIFIED CENCOM OF A PRECAUTIONARY LANDING. MEANWHILE WE REDUCED SPEED AND BEGAN A SHALLOW APPROACH INTO THE AIRPORT.
DURING A ROUTINE INSP, MECHANIC FOUND CRACK ON BULKHEAD STA NR 142, WL 61, ON THE RT SIDE GENERATING FROM A NUTPLATE SCREW LOCATION.
