Maintenance disclosures
THE MECHANICS HAD THE POWER ON THE AIRCRAFT AND NOTICED SPARKS IN THE OVERHEAD AREA ABOVE CIRCUIT BREAKER PANEL, WHEN THEY MOVED THE ROTOR BRAKE HANDLE FORWARD. THE AREA WAS INSPECTED AND THEY DISCOVERED THAT SOME WIRES HAD BEEN CONTACTING THE ROTOR BRAKE ROD AND CHAFED THROUGH K16 & K63 WIRE BUNDLE. WIRES WERE REPAIRED AND REROUTED. WE INSPECTED THE REMAINDER OF OUR FLEET AND FOUND NO OTHER AIRCRAFT IN THIS CONDITION.
OIL COOLER HAD EXCESSIVE VIBRATION. BEARING ROUGH.
DURING AN INSPECTION, A CRACK WAS DISCOVERED ON A MAIN ROTOR DROOP STOP.
BEFORE LANDING CHECKLIST, TO LAND AND WHEN THE LANDING GEAR HANDLE WENT DOWN, GOT A NR2 SERVO, NR 2 HYD MIN, AND HYD UTIL PRESS CAUTION LIGHTS. THEN WENT TO THE HYD SYNOPTIC PAGE. SAW THE NR 2 HYD FLUID LEVEL GO THROUGH 22 PERCENT AND THEN IT WENT TO ALL AMBER WITH BLACK LIGHTNING BOLTS SHOWING. THE TRSOV AND UTILITY SOV2 WAS SHOWING CLOSED. THE HYD PRESSURE WAS 208 BAR AND TEMP WAS 55. USED THE CAUTION CHECK LIST. HAD A GREEN LIGHT ON THE NOSE GEAR, BUT HAD AMBER LIGHT ON BOTH MAIN GEAR. FLEW BACK TO MX, SAID THAT THE MAINS WERE NOT ALL THE WAY DOWN. SO WE USED THE EMERGENCY DOWN PUSH BUTTON AND BOTH MAINS CAME DOWN ALL THE WAY, SO HAD 3 GREEN LIGHTS. LANDED, UNLOADED THE PASSENGERS THEN SHUTDOWN ACFT.
NR 2 SERVO CAS ILLUMINATED IN FLIGHT. EMERGENCY CHECK LIST USED. LANDING GEAR WAS LOWERED NORMALLY IAW THE CHECKLIST AND ACFT LANDED UNEVENTFULLY AS SOON AS PRACTICAL. REPLACED NR 2 RT SERVO PRESSURE SWITCH.
NR 2 SERVO CAS ILLUMINATED IN FLIGHT. EMERGENCY CHECK LIST USED. LANDING GEAR WAS LOWERED NORMALLY IAW THE CHECKLIST AND ACFT LANDED UNEVENTFULLY AS SOON AS PRACTICAL. PROBLEM COULD NOT BE DUPLICATED AND ACFT WAS RETURNED TO SERVICE.

FIRESEAL, TORN AND DETERIORATED.
MECHANICAL DAMAGE BEYOND LIMITS.
CRACKS FOUND IN FITTING AT UPPER LT TAILBOOM ATTACHMENT POINT RADIATING FROM LOCK FASTENERS.
DAMAGED TO SCISSORS FROM CONTACT OF LONG COUPLING, REPLACED SCISSORS.
TAIL ROTOR BONDING STRAP BROKE OFF IN FLIGHT, REPLACED STRAP.
TIRE EXCESSIVELY WORN DOWN TO CORDS, REPLACED BOTH MAIN TIRES.
DAMAGED TO SCISSORS FROM CONTACT OF LONG COUPLING, REPLACED SCISSORS.
FOUND FIREWALL CRACKED, REPLACED FIREWALL.
GROOVE WORN IN SHANK, REPLACED COUPLING.
DURING 300 HOUR INSPECTION ROTATING SCISSORS HAD EXCESSIVE PLAY AND WHEN ASSY WAS DISSASSEMBLED THE BUSHINGS WERE GALLED.
DURING 300 HOUR INSPECTION ROTATING SCISSORS HAD EXCESSIVE PLAY AND WHEN ASSY WAS DISASSEMBLED THE BUSHINGS WERE GALLED, REPLACED BUSHINGS.
IN CRUISE, FLIGHT AT 6500 FT, THE NR 2 SERVO CAUTION LIGHT CAME ON. OPENED THE SYNOPTIC PAGE AND FOUND ALL PRESSURE AND TEMPS NORMAL. HOWEVER THE 4 SERVO LIGHTS WERE YELLOW. FOLLOWED THE EMERGENCY PROCEDURES CHECKLIST AND TURNED AROUND. AFTER A FEW MINUTES THE CAUTION LIGHT BEGAN TO GO OFF AND ON AND AFTER A MINUTE OF THAT IT WENT OUT AND STAYED OUT. RETURNED FOR MAINTENANCE TO CHECK THE PROBLEM.
FOUND FIREWALL CRACKED, REPLACED FIREWALL.
EXHAUST NOZZLE HAD 12 INCH CRACK ALONG TACK WELDED SEAM NEAR AFT END OF TAIL PIPE. REPLACED WITH SERVICEABLE EXHAUST NOZZLE.
LINES WERE INSTALLED WRONG, CAUSING THEM TO BE KINKED.
