CRACK DISCOVERED ON THE RH INTERIOR FUSELAGE AT FS 196. CRACK ON RH TIE CROSS PN: 50-420013-442 AND BOTTOM DOUBLER PN: 50-420013-279 DURING A PHASE 3 INSPECTION.
WENT TO PUT THE GEAR DOWN, RIGHT MAIN GEAR LIGHT DIDN'T ILLUMINATE. WENT AROUND, RAN THE QRH, CYCLED THE GEAR AND TROUBLE SHOT. TOWER SAID OUR GEAR APPEARED TO BE DOWN. WHILE TROUBLE SHOOTING, WITH GEAR DOWN AND FULL FLAPS, WE DIDN'T GET AND GEAR AURAL WARNING SOUNDS OR LIGHTS. INFORMED THE CREW AND PASSENGERS OF THE SITUATION, RETURNED TO HONOLULU AND LANDED SAFELY. CORRECTIVE ACTION: REPLACED RH MLG DOWNLOCK SWITCH ASSEMBLY. GROUND OPERATIONAL CHECK SATISFACTORY
DURING CLIMBOUT, FIVE MINUTES INTO FLIGHT, THE PILOT OBSERVED THE RT ATTITUDE INDICATOR TUMBLED, MADE 180 DEGREE TURN BACK TO MAINTENANCE BASE. ATTITUDE INDICATOR STOPPED ITS TUMBLE AND RIGHTED ITSELF. MX PERFORMED OPS CHECK, FOUND NO DEFECTS NOTED AND COULD NOT DUPLICATE ATTITUDE INDICATOR TUMBLE. REPLACED ATTITUDE INDICATOR AND PERFORMS OPS/LEAK CHECK NO DEFECTS NOTED.
AIRCRAFT LEVEL AT 15,000FT, ATC SHOWED ALTITTUDE READOUT OF 22,700FT. AFTER DESCENT FROM 15,000FT ATC BEGAN TO SEE NORMAL ALTITTUDE READOUT. AIRCRAFT RETURNED TO MX BASE WITHOUT INCIDENT FOR REPAIRS. NO EMERGENCY ACTION WAS REQUIRED. AFFECTED SYSTEM BEING TROUBLESOT AT THIS TIME.
DURING FLIGHT, PILOT OBSERVED THAT THE NR 1 TRANSPONDER WAS INTERMITTENT, PILOT TURNED AROUND.
PILOT WAS CLIMBING OUT FROM AIRPORT AT 9,000 MSL IN LEVEL FLIGHT. PILOT SIDE VSI WAS INDICATING 400' CLIMB AT ALTITUDES 9,000', 11,000' & 13,000' MSL WITH AIRCRAFT IN LEVEL FLIGHT. RETURNED BACK TO AIRPORT FOR MX TO REPAIR. MX FOUND FAULTY VSI COMPONENT.
ON 5/6/2010, THE LANDING GEAR DOWN CYCLE WAS INTERRUPTED BY A SURGE AND THE PILOT DISCONTINUED THE APPROACH TO LAND AAT THE AIRPORT AND RECYCLED THE LANDING GEAR AND FOUND NORMAL OPERATION AND INDICATION. AS A PRECAUTION, THE FLEW TO THE AIRPORT WHERE MX IS LOCATED INSTEAD OF LANDING AT HIS FIRST INTENDED DESTINATION. A MECHANIC OPERATED THE LANDING GEAR ON JACKS AND FOUND A SLIGHT INTERRUPTION DURING THE GEAR DOWN EXTENSION CYCLE. THE LANDING GEAR HYDRAULIC POWERPACK WAS REPLACED WITH AN OVERHAULED UNIT AND THE SYSTEM WAS OPERATIONALLY CHECKED FOR NORMAL AND CORRECT OPERATION.
ON DECEMBER 27,2009 THE LANDING GEAR FAILED TO EXTEND UNDER NORMAL ELECTRICAL POWER DUE TO A NUISANCE OPENING IN THE 60 AMP UNDERFLOOR CIRCUIT BREAKER. THE PILOT TOOK EMERGENCY ACTION WITH THE CONTROL TOWER TO VISUALLY CHECK THE LANDING GEAR FOR THE DOWN POSITION BEFORE LANDING. AFTER THOUROUGH MX CHECKS AND INSP, THE LANDING GEAR SYS INCLUDING THE POWERPACK WERE FOUND TO BE OPERATING NORMALLY AFTER RESETTING THE 60 AMP BREAKER.
PILOT NOTICED A BURNING SMELL AND SUBSEQUENT SMOKE ACCUMULATION IN THE COCKPIT. AFTER BEING UNABLE TO BE CERTAIN OF THE CAUSE, HE TURNED OFF ALL ELECTRICAL POWER AND LANDED AT OUR MAINTENANCE BASE. THE CABIN ENVIRONMENTAL VENTALATION BLOWER MOTOR WAS FOUND SEIZED AND VERY HOT. THE CIRCUIT IS PROTECTED BY A 30 AMP LIMITER IN THE LT DC POWER PANEL. FURTHER INVESTIGATION IS BEING CONDUCTED TO DETERMINE IF 30 AMPS MAY OR MAY HAVE BEEN EXCEEDED. THE VENT BLOWER MOTOR WAS REPLACED WITH A SERVICEABLE PART AND THE SYSTEM TESTED TO ASSURE NORMAL OPERATIONS. THE AC WAS TEST FLOWN AND RETURNED TO SERVICE WITH NO FURTHER PROBLEMS. (K)
