Part: CARBURETOR — DAMAGED (ENGINE) · airframe 10,465 hrs
PILOT WAS MAKING 3RD TOUCH AND GO, ON CLIMBOUT ENGINE STARTED VIBRATING EXCESSIVELY, POWER WAS REDUCED AT PATTERN ALTITUDE, MIXTURE ADJUSTED, SWITCHED MAGNETO'S FROM R TO L WITH NO CHANGE IN THE VIBRATION. AIRCRAFT LANDED AND SUBSEQUENT OPERATION WHILE TAXIING TO PARKING NO VIBRATION WAS ENCOUNTERED. A RUN UP WAS ACCOMPLISHED AND AT ENGINE OPERATION ABOVE 1800 RPM THE VIBRATION REOCCURRED. MAINTENANCE DISCOVERED THE PROBLEM TO BE THE CARBURETOR ACCELERATION DISCHARGE TUBE WAS MISSING AND HAD BEEN INGESTED BY THE ENGINE. THE ENGINE AND CARBURETOR HAD 246.9 HOURS SINCE OVERHAUL BY TEXTRON LYCOMING.

BENDIX MAGNETO OVERHAULED BY ELECTRO SYSTEMS, REPAIR STATION UT2R22GL. MAGNETO WAS FURNISHED WITH LYCOMING FACTORY OVERHAULED ENGINE IN 1995. MAGNETO WAS REMOVED FOR 500-HOUR INSPECTION AND C/W AD 96-12-07 AT 489.3 SMOH FOR THE FIRST TIME. FOUND ONE EACH COIL P/N ES 10-382790-1 WITH MFG DATE 2/01/95 CRACKED IN 10 PLACES WITH CRACKS RADIATING OUTWARD FROM CENTER SUPPORTS AND BRUSH CONTACT TANG.
Part: MAGNETO — TEETH MISSING (DISTRIBUTOR GEAR)
BENDIX/TCM DUAL MAGNETO REMOVEDF FROM LYCOMING. FACTORY OVERHAULED ENGINE AT SMOH 489.3 FOR 500-HOUR INSPECTION AND C/W AD 96-12-07. FOUND MAGENTO DISTRIBUTOR GEARS WITH THREE TEETH MISSING FROM ONE GEAR AND ONE-HALF TOOTH MISSING FROM OTHER GEAR. CONTINUED OPERATION WOULD HAVE CAUSED CATASTROPHIC FAILURE. THIS MAGNETO WAS OVERHAULED BY ELECTRO SYSTEMS IN 1995 AND HAD AN ELECTRO SYSTEMS DATA PLATE AFFIXED TO MAGNETO. THIS MAGNETO WAS INSTALLED AT LYCOMING OVERHAUL AND NEVER REMOVED UNTIL SMOH 489.3. RECOMMEND ALL MAGNETOS OVERHAULED BY ELECTRO SYSTEMS BE REMOVED AND INSPECTED IAW BENDIX OVERHAUL MANUAL.
