DURING THE ACCOMPLISHMENT OF AN TECHNICAL BULLETIN BT109EP-128 ON A MAIN GEARBOX; A CROWN GEAR, WAS FOUND TO HAVE A CRACK. THE CROWN GEAR WILL BE REPLACED BY A NEW SUPPORT ASSY.
A CRACK WAS FOUND IN THE INTERIOR OF THE LEFT SIDE OF THE VERTICAL FIN. THE CRACKS WERE LOCATED ON HORIZONTAL RIBS AT WL 235.2 AT STATION 9725.0 MM. LOWER CRACK IS APPROXIMATELY 4 INCHES LONG. SECOND CRACK IS APPROX. 1.25 INCH LONG. MFG WAS CONTACTED FOR SERVICEABILITY INSTRUCTIONS. A LETTER WAS INSSUED PERMITTING OPERATION FOR 100 HOURS PROVIDED DAILY AND 25 HOUR INSPECTIONS WERE COMPLIED WITH INSTRUCTIONS.
THIS PY AIR LINE CRACKED AND CAUSED SUDDEN DECELERATION. PILOT MADE NORMAL SINGLE ENGINE LANDING WITHIN PUBLISHED OEI LIMITATIONS. VIBRATIONS WERE RECORDED ON MAIN DRIVESHAFT. THE ENGINE ASSY AND MAIN DRIVE SHAFT WERE REPLACED.
TUBE OR WELD CRACKED WHERE TUBE ENTERS FUEL NOZZLE FITTING. THIS IS THE SIXTH OCCURRENCE SINCE MAY, 1997. THREE WERE P/N 4-401-376-01 MANIFOLDS AND 3 WERE P/N 4-301-236-04 MANIFOLDS. THIS IS BEING ADDRESSED BY ALLIED SIGNAL.
FOUR MINUTES AFTER TAKEOFF, NR 2 ENGINE WENT TO FLT IDLE. A SINGLE ENGINE LANDING WAS INITIATED. MAINTENANCE PERSONNEL FOUND A T-FITTING ON THE PC FILTER HOUSING BROKEN OFF. THE FILTER ASSY WAS REPLACED, THE ENGINE WAS OPERATIONAL CHECKED AND RETURNED TO SERVICE. (M)
DURING FLIGHT, THE PILOT NOTED THE NR 2 ENGINE OIL PRESSURE DROPPING AND THE OIL TEMPRATURE RISING. HE SHUT DOWN THE ENGINE AS A RESULT, AND PERFORMED A SINGLE ENGINE LANDING. WHILE INVESTIGATING THE CAUSE, THE MECHANIC DISCOVERED THE SHAFT OF THE OIL COOLER BLOWER TO HAVE SHEARED NEXT TO THE SPINE ADAPTER. THE IMPELLERS APPEAR TO HAVE SEIZED INSIDE THE HOUSING.
