COMPANY DISPATCH RECALLED THE AIRCRAFT WHEN IT WAS SUSPECTED THAT A BUSHING MIGHT HAVE BEEN LEFT OUT OF THE UPPER CLEVIS MOUNT OF THE RIGHT ENGINE. THE RIGHT ENGINE HAD BEEN INSTALLED ON 10/24/2010. THE FLIGHT CREW ADVISED MIAMI ATC AND ASKED TO RETURN TO KATL. ATC VECTORED THE AIRCRAFT ON A HEADING TO KATL BUT BECAUSE OF WEATHER IN THE ATL AREA, THE FLIGHT CREW ELECTED TO DIVERT. THE DIVERSION WAS APPROVED AND AN OTHERWISE NORMAL APPROACH AND LANDING WAS MADE AT KTYS. MAINTENANCE TOOK THE AIRCRAFT TO A HANGAR AND INSPECTED THE RIGHT ENGINE. THE BUSHING WAS CONFIRMED AS MISSING. MAINTENANCE REPLACED THE BUSHING AND RAN THE ENGINE. NO ABNORMAL VIBRATIONS WERE NOTED AND THE AIRCRAFT WAS RELEASED FOR SERVICE. THE FOLLOWING DAY, MAINTENANCE SUPERVISOR PERSONNEL DECIDED TO HAVE THE AIRCRAFT RETURNED TO THE HANGAR TO REPLACE THE CLEVIS AS A PRECAUTION. ON 10/28/10 THE AIRCRAFT ARRIVED AT KTYS HANGAR AND THE ENGINE WAS LOWER, THE CLEVIS REPLACED AND THE ENGINE REINSTALLED. POST MAINTENANCE ENGINE RUNS WERE ONCE AGAIN NORMAL FOR VIBRATION. ACCORDING TO GE REMOTE DIAGNOSTICS, AT NO TIME DID THE VIBRATION LEVEL OF THIS ENGINE EXCEED .4 MILS.
ON TAKEOFF ROLL FROM KATL THE AIRCRAFT DEVELOPED A SEVERE NOSE WHEEL SHIMMY. THE FLIGHT CREW EXECUTED A REJECTED TAKEOFF AT APPROXIMATELY 85 KNOTS. DURING THE SLOWING OF THE AIRCRAFT WITH BRAKES AND THRUST REVERSERS, TOWER REPORTED SEEING SMOKE FROM THE RT ENGINE. CFR INSPECTED THE AIRCRAFT AND THERE WAS NO FIRE. THE AIRCRAFT WAS RELEASED TO THE GATE FOR PASSENGER DEPLANING. MAINTENANCE FOUND FOD DAMAGE TO THE FAN CASE OF THE RT ENGINE. FURTHER TROUBLESHOOTING BY BORESCOPE REVEALED DAMAGE TO THE FIRST AND SECOND STAGE COMPRESSOR BLISKS. THE AIRCRAFT WAS REMOVED FROM SERVICE AND THE ENGINE WILL BE REPLACED.

DURING SINGLE PACK OPERATION AT FL250 BETWEEN ATL-PWM THE ISOL VALVE BEGAN OSCILLATING OPEN AND CLOSED. AS THIS WAS HAPPENING THE RT PACK BEGAN TO OPEN AND CLOSE. THE PACK WOULD THEN FREEZE. THE CABIN WAS BEGINNING TO CLIMB AND DESCEND AS WELL. NOTIFIED MOC AND DISPATCH. RETURNED TO ATL. THE DECISION WAS ALSO MADE BY FLIGHT CREW TO DESCEND TO 10000FT. CREW DIDNâT BELIEVE THE CABIN WAS GOING TO HOLD ANY LONGER. THE SITUATION WAS NOT IMPROVING. NOTIFIED DISPATCH, AND DISPATCH DECIDED ALONG WITH MOC FOR ACFT TO LAND TYS. LANDED UNEVENTFULLY, MAINT IN TYS WORKED ON ACFT, DEPARTED ON TO PWM. WHILE IN TYS THE ISO VALVE WAS OPERATIONALLY CHECKED IAW 36-13-15 AND DEEMED AIRWORTHY. AS OF 04/12/10 THERE HAVE BEEN NO REPEATS.
LT ENGINE BLEED VALVE OPENING AND CLOSING CRUISING LT PACK TO SHUT OFF. REMOVED & REPLACED ACSC1 PER CRJ900 AMM 21-61-04, OPS CHECKS GOOD. OK FOR SERVICE.
