IAH - FLIGHT 3985, DURING MAINTENANCE INSPECTION THE AISLE PATH LIGHT STRIP AT ROW 8-9 WAS FOUND TO BE INOPERATIVE. MAINTENANCE PLACED THE LIGHT STRIP ON MEL AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE. ON 04/15/20 IAH MAINTENANCE REMOVED AND REPLACED THE AFFECTED LIGHT STRIP, OPERATIONALLY TESTED WITH NO DEFECTS, REMOVED THE MEL, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.

MAINTENANCE REMOVED THE DAMAGED SECTION OF SKIN AND REPAIRED THE AFFECTED ARE IN ACCORDANCE WITH SERVICE BULLETIN SB145-53-0079, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
DURING "A-CHECK" INSPECTION THE COCKPIT LATERAL UPPER MOUNTING BRACKET WAS FOUND TO BE CRACKED. MAINTENANCE PERFORMED A DOUBLER REPAIR IN ACCORDANCE WITH TECHNICAL DISPOSITION ETD2017-145-011028, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
PILOT REPORTED ODOR OF COMBUSTION ON TAKEOFF FROM PACK/OIL BURNING SMELL. ITEM INITIALLY PLACED MEL 21-51-00-1. ON 11/25/16 MX RAN PACK NR 1 MULTIPLE TIMES AND COULD NOT DUPLICATE MEL CLEARED. A/C WAS APPROVED FOR RETURN TO SERVICE.

THE CREW REPORTED ACFT WAS NOT GETTING FULL BRAKING WHEN FULL BRAKING WAS APPLIED. OTBD BRAKES WERE COLD AND INBD BRAKES HOT AFTER LANDING. MAINTENANCE INSPECTED AND REPLACED THE BRAKE CONTROL UNIT. OPS CHECKS WERE GOOD.

CREW REPORTED F/O MFD WENT BLANK FOLLOWED BY ELECTRICAL BURNING ODOR. MAINTENANCE INSPECTED COCKPIT ELECTRICAL WIRING AND NO DEFECTS WERE FOUND. OPEN
DURING SCHEDULED MX INSPECTION EMERGENCY LIGHTS ABOVE THE FWD F/A SEAT, COCKPIT STEP, AND ABOVE THE GALLEY WERE FOUND INOPERATIVE. MX CLEANED AND RESECURED THE CONNECTOR ASSOCIATED WITH OPS CHECK SUCCESSFUL. ACFT WAS APPROVED FOR RETURN TO SERVICE.
CREW REPORTED A STRONG ELECTRICAL ODOR COMING FROM PACK NR 2 FOLLOWED BY APU GEN FAILURE. FOUND THE APU STARTER GEN SHOWING SIGNS OF BEING BURNED. MX R & R THE APU STARTER GENERATOR WITH OPS CHECKS SUCCESSFUL.
DURING CRUISE THE CREW RPORTED AN ELECTRIC BURNING ODOR IN THE CABIN. THE ACFT LANDED WITHOUT INCIDENT WHERE MX INSPECTED THE ACFT AND R & R THE LIGHT ASSY ABOVE ROW 15-16. OPS TESTED WITH NO DEFECTS, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.

TYS - A/C WAS EN ROUTE FROM BHM TO ORD WHEN CREW GOT A AIR GROUND FAIL MESSAGE ON THE ICIAS AND GEAR WOULDN'T RETRACT. CREW RETURNED TO BHM WITHOUT INCIDENT. A/C WAS FERRIED TO TYS WHERE MAINTENANCE COMPLIED WITH LANDING GEAR PROX SWITCHES ADJUST/TEST PROCEDURES IAW EMB 145 AMM 32-63-05. NO FAULT FOUND, OPS CHECK GOOD. REMOVED AND REPLACED THE LGEU IAW EMB 145AMM 32-32-01, OPS CHECK GOOD AND WAS RELEASED BACK TO SERVICE.
FLIGHT 6139 - EWR - FLIGHT CREW REPORTED LAV SMOKE DETECTOR TEST INOPERATIVE. EWR MX REMOVED AND REPLACED LAV SMOKE FIRE DETECTOR PANEL AND PERFORMED OPERATIONAL CHECK SUCCESSFULLY. ACFT WAS APPROVED TO RETURN TO SERVICE.

THE CREW REPORTED THE MAIN CABIN DOOR WOULD NOT INDICATE LATCH CLOSED. MAINTENANCE INSPECTED AND REPAIRED WIRE W717-0010-24. MAIN CABIN DOOR WARNING OPERATIONAL CHECKS WERE GOOD.
TYS DURING A CURRENT 2C INSPECTION THE MIDDLE BUSHION ON THE RUDDER 2 WAS FOUND WORN. MAINTENANCE REMOVED AND REPLACED THE BRACKET WITH A NEW BUSHION IAW EMB 135/145 SRM 51-40-02 AND EC 5540-01028. ALL WORK WAS ACCOMPLISHED AND WILL BE RELEASED AT THE END OF THE C-CHECK.
IAH - FLIGHT 2080, THE CREW REPORTED AN ELECTRICAL OR SULFUR SMELL IN THE COCKPIT AND CABIN DURING TAXI AND CLEARED IN 3 TO 5 MINUTES. THE AIRCRAFT RETURNED TO THE GATE WHERE MAINTENANCE INSPECTED THE AIRCRAFT WITH NO DEFECTS NOTED, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK THE APU FIREWALL AT FRAME 81 HAS CHAFE MARKS FROM RIVETS. MAINTENANCE REPAIRED THE APU FIREWALL AT FRAME 81 IAW EC5330-01507. THE AIRCRAFT WAS APPROVED FOR RETURNED TO SERVICE.
CREW REPORTED, VERY STRONG FUEL OR EXHAUST ODOR IN GALLEY AND GALLEY CLOSETS. RAN NR 1 AND NR 2 ENGINES, OPS CHECK OF ENGINE BLEED SYSTEMS CHECKS GOOD, VISUAL INSPECTION OF FORWARD CABIN AREA [GALLEYS, CLOSETS, SPACES] NO DEFECTS, NO ODORS NOTED IAW EMB145 AMM 71-00-01, 36-11-01, AND 05-20-18. AIRCRAFT RETURNED TO SERVICE.
OKC - FLIGHT 2964, DURING PRE-FLIGHT INSPECTION, THE CREW REPORTED THAT THE EMERGENCY LIGHT AT THE FORWARD FLIGHT ATTENDANT SEAT WAS INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE NUMBER TWO EMERGENCY LIGHT INVERTER, OPERATIONALLY CHECKED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE. NOTE: THIS PART IS NOT SERIALIZED, AND NO TIMES ARE AVAILABLE.
DURING PRE-FLIGHT INSPECTION, THE CREW REPORTED THAT THE EMERGENCY EXIT LIGHT AT THE MAIN CABIN DOOR WAS INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE AFFECTED LIGHT BULB, OPERATIONALLY CHECKED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.

SHV- DURING C-CHECK, THE CENTER OMEGA BEAM BUTTSTRAP WAS FOUND TO BE CORRODED. MAINTENANCE REMOVED AND REPLACED THE AFFECTED BEAM IAW EMB 145 SRM 51-40-02. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.

ON TAKEOFF ROLL THE CREW NOTED A STEERING INOP MESSAGE. ABORTED THE TAKEOFF. MAINTENANCE CYCLED THE DISCONNECT SWITCH, PERFORMED AN OPERATIONAL CHECK OF THE NOSE WHEEL STEERING SYSTEM, NO DEFECTS NOTED. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH- DURING FLIGHT 3588 THE CREW NOTED THAT THE NUMBER 2 PACK ON IN FLIGHT PRODUCES BURNING SMELL AND SMOKE IN MAIN CABIN. IAH- MAINTENANCE REMOVED AND REPLACED THE NUMBER 2 AIR CYCLE MACHINE, PERFORMED A OPERATIONAL CHECK OF THE AIRCONDITIONING SYSTEM, NO DEFECTS NOTED, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH- DURING PREFLIGHT THE CREW NOTED THAT THE EXIT SECTION OF THE EMERGENCY LIGHTS IN THE FLOOR INOPEERATIVE. IAH- MAINTENANCE RESECURED THE CONNECTORS ON THE EMERGENCY LIGHT FLOOR STRIP BY SEAT 11 and 12, PERFORMED A OPERATIONAL CHECK OF THE EMERGENCY LIGHTING SYSTEM,NO DEFECTS NOTED, AND, THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
DURING SCHEDULED MAINTENANCE THE LEFT EMERGENCY EXIT SIDEWALL LIGHT WAS FOUND TO BE INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE AFFECTED LIGHT ASSEMBLY AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH- THE CREW NOTED A ELECTRIC SMELL FROM THE GALLEY HOT JUG AREA. MAINTENANCE INSPECTED THE AIRCRAFT, AND, THE AIRCRAFT WAS RELEASED IAW MEL. MAINTENANCE C-CHECK, REMOVED AND REPLACED RECEPTACLE CONNECTORS AT AFT GALLEY AREA, PERFORMED A OPERATIONAL CHECK OF THE HOT JUG SYSTEM, NO DEFECTS NOTED, AND, THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
CLT - FLIGHT 4083, THE CREW REPORTED THAT THE NUMBERONE ENGINE EXPERIENCED AN UNCOMMANDED SHUTDOWN OF FLIGHT. THE CREW TRIED TO RESTART THE ENGINE WITH NO SUCCESS, AND DIVERTED TO CLT WHERE THE AIRCRAFT LANDED WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE NUMBER ONE ENGINE, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.SDR FILED OPEN AWAITING FINAL DETERMINATION OF CAUSE...
CLT FLIGHT 4083. CREW REPORTED THAT THE LEFT HAND ENGINE FAILED IN FLIGHT, ATTEMPTED RESTARTS WERE UNSUCCESSFUL. THE AIRCRAFT RETURNED TO THE FIELD WITHOUT FURTHER INCIDENT. MAINTENANCE INSPECTED THE AIRCRAFT AND FOUND THE ENGINE TO BE SEIZED-UP AND METAL ON THE CHIP DETECTORS. THE ENGINE WAS REMOVED AND REPLACED IAW CONTINENTAL EXPRESS TASK CARD 6271001, OPERATIONAL AND LEAK CHECKS WERE SATISFACTORY. THE AIRCRAFT WAS RELEASED FOR SERVICE.
IAH- FLIGHT 3912, DURING POST FLIGHT INSPECTION, THE CREW REPORTED THAT THE AFT SECTION OF THE AISLE PATH EMERGENCY EXIT LIGHTS WAS INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE AFFECTED SECTION OF LIGHT STRIP, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH - FLT 4203, CREW REPROTED THAT DURING TAXI THE NR 1 ENGINE EMMITTED A HUMMING SOUND AND VIBRATION AT LOW TO MEDIUM POWER SETTINGS. MAINTENANCE REMOVED AND REPLACED THE NR 1 ENIGNE IAW ROLLS ROYCE MAINTENANCE MANUAL 71-00-00. THE AIRCRAFT WAS THEN APPROVED FOR RETURN TO SERVICE.
ATL - FLT 4179 - THE CREW REPORTED THAT THE NR 1 ENGINE SHUTDOWN UNCOMMANDED IN FLIGHT. THE AIRCRAFT DIVERTED TO ATL WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED THE AIRCRAFT AND FOUND THAT THE NR 1B FADEC WAS INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE AFFECTED FADEC, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE. (M)
