IN CLIMB AT 3500 FEET WHEN PILOTS REDUCED THROTTLE THE RIGHT PROPELLER WENT INTO FEATHER. QRH PROCEDURES FOLLOWED, EMERGENCY DECLARED AND AIRCRAFT RETURNED TO BASE (SJU). SUBSEQUENT MAINTENANCE INVESTIGATION FOUND AUTOFEATHER MICRO-SWITCHES OUT OF ADJUSTMENT. ADJUSTED SWITCHES AND RETURNED AIRCRAFT TO SERVICE.
ON DESCENT HYD RT CWP LIGHT ILLUMINATED. HYD PANEL SHOWED RESERVOIR CONTENTS, UPPER RED. LT & RT FLUID TEMP 70 DEG, MAIN SYS 3000 PSI. SHORTLY THEREAFTER BOTH HYD LT & HYD RT CWP WARNING LIGHTS ILLUMINATE. HYD PANEL SHOWED RESERVOIR CONTENTS LOWER RED. FLUID TEMP 80 - 90 DEG, MAIN SYS PSI DECREASING. CREW COMPLETED EMER CHECKLIST, MAINTAINED FULL CONTROL OF ACFT. DECLARED EMER & LANDED WITHOUT INCIDENT. ACFT WAS SHUT DOWN ON TAXI WAY M & TOWED TO MX LOCATION AT RAMP. FOUND LEAK ON HYD PUMP. PUMP WAS REPLACED & OPERATIONS CHECKED SATISFACTORILY. ACFT RETURNED TO SERVICE WITHOUT FURTHER INCIDENT.
AFTER TAKEOFF CREW EXPERIENCED LT ENG SURGE. RETURNED SAFELY TO POINT OF ORIGIN. FOUND TO BE CAUSED BY ENG P2.5 BLEED AIR LEAK. REPLACED P2.5 FLEXIBLE HOSE.
IN CRUISE, CREW OBSERVED A RT LOW FUEL PRESSURE ANNUNCIATION FOLLOWED BY RT ENGINE FLAMEOUT. DECLARED AN EMERGENCY AND COMPLETED AN UNEVENTFUL SINGLE ENGINE APPROACH AND LANDING. DISCOVERED AFT FUEL TANK QTY SYS WAS INDICATING 400 LBS WHEN EMPTY AND ENGINE FLAMED OUT DUE TO FUEL STARVATION. FOUND THAT THE CAPACITY OF BOTH FUEL PROBES IN FUEL CELL NR 3 WERE OUT OF TOLERANCE. BOTH PROBES WERE REPLACED AND THE SYS WAS RECALIBRATED.
ON APPROACH, THE CREW INITIATED A GO-AROUND. UPON INCREASING THRUST, THE CREW EXPERIENCED SURGING AND TORQUE FLUCTUATION ON THE RIGHT ENGINE. FOLLOWING QRH PROCEDURES THE CREW DECLARED AN EMERGENCY, SHUTDOWN THE AFFECTED ENGINE, AND COMPLETED AN UNEVENTFUL SINGLE ENGINE APPROACH AND LANDING. IT WAS DETERMINED THAT THE DIFFICULTY WAS DUE TO A FAULTY ENGINE BLEED VALVE. THE FAULTY ASSEMBLY WAS REPLACED AND ENGINE OPS CHECKED NORMAL.
N151CA DEPARTED MKE AT 00:00 ON A REPOSITION FLIGHT TO DNV. UPON REACHING CRUISE (5000FT) ALTITUDE, CRUISE POWER WAS SET BY THE FIRST OFFICER AS THE CAPTAIN WAS THE PILOT FLYING. SHORTLY AFTER CRUISE POWER WAS SET THE RIGHT ENGINE FAILED. BOTH TORQUE AND ITT DROPPED TO APPROXIMATELY ZERO SHORTLY FOLLOWED BY A DROP IN PROP RPM. THE AIRCRAFT ENGINE WAS SECURED BY THE FOLLOWING APPROPRIATE CHECKLIST. AN EMERGENCY WAS DECLARED TO ATC AND FLIGHT RETURNED TO MKE WITH NO FURTHER INCIDENTS.
THE CREW WAS DEPARTING CHARLESTON, SC, THE PILOT SMOOTHLY ADVANCED THE THROTTLES TO ALLOW THE PROPS TO SPIN UP TO MAX RPM, BEFORE SETTING TAKEOFF POWER. THE LEFT ENGINE STARTED SURGING AND BACKFIRING WHILE THE PILOT ADVANCED THE THROTTLES. THE COPILOT CALLED TO ABORT THE TAKEOFF. PILOT PROMPTLY REACTED AND BROUGHT THE AIRPLANE TO A COMPLETE STOP. THE AIRCRAFT WAS TAXIED TO THE FOB AND MAINTENANCE AND DISPATCH WERE CALLED.
A/C DEPARTED CHEYENNE, WY, AFTER FUEL TOP OFF AND CLIMBING OUT THE LT ENGINE TORQUE DROPPED TO 2500 NR AND NG DROPPED 3 PERCENT. THE PILOT INCREASED THE POWER LEVERS, THE TORQUE ROSE AND THEN SETTLED BACK TO 2500 NR. THE CREW MADE A PRECAUTIONARY ENGINE SHUT DOWN AND MADE AN UNEVENTFUL LANDING AT LORAMY, WY. NO EMERGENCY WAS DECLARED. TOTAL FLIGHT TIME WAS .4 HRS. THE MECHANIC INSPECTED THE ENGINE FOR BLEED AIR, PY, P3 FOR LEAKS AND NO DEFECTS WERE NOTED. THE MECHANIC REMOVED AND REPLACED THE BLEED VALVE AND PERFORMED RUN CHECKS ON THE ENGINE IAW THE MM AND DECLARED THE ENGINE OK FOR RETURN TO SERVICE. THE AIRCRAFT CONTINUED THE FOLLOWING DAY WITH NO FURTHER OCCURRENCES. (M)
UPON LANDING IN MLI, THE LEFT MAIN WHEEL BLEW OUT. THE MLI MECHANIC REMOVED AND REPLACED THE LT MAIN WHEEL ASSY. THERE WAS NO DAMAGE TO THE BRAKE AND STRUT. OPS CHECKED GOOD. (X)
AT CVG, CAPTAIN REPORTED THAT THE LEFT GENERATOR WOULD NOT COME ON LINE. AT THE REQUEST OF MAINTENANCE, ANOTHER ATTEMPT WAS MADE, AT THIS TIME THE GENERATOR CAME ON LINE AND REMAINED ON LINE. PILOT ALSO REPORTED THAT THIS HAD BEEN A PRIOR PROBLEM. AIRCRAFT WAS BROUGHT TO MKE FOR TROUBLESHOOTING. MX FOUND THAT THE STARTER/GENERATOR LINE CONTACTOR WAS INTERMITTENT. THE CONTACTOR WAS REPLACED AND AIRCRAFT RETURNED TO SERVICE. THE CONTACTOR OPEN INVESTIGATION FOUND THAT THE MICROSWITCH ACTUATOR WAS OUT OF ADJUSTMENT, WOULD NOT MAKE SAME TIME WHEN PLACED INTO GENERATOR POSITION.
