AFTER TAKE-OFF DURING CLIMB OUT MAIN GEAR BOX CHIP SUMP KEEP CAME ON AND OFF NONSTOP. RTB. FOUND OIL & WATER IN SUMP CHIP PLUG CONNECTOR. CLEANED CONNECTOR.
DURING TAKEOFF, BUT PRIOR TO DP, PIC NOTED LARGE TORQUE SPLIT AND#2 ENGINE OPERATING OUT OF LIMITS. ABORTED TAKEOFF.
ON A NORMAL APPROACH TO THE H AT THE NORTH RAMP, AT ABOUT 150 AGL, WE SUDDENLY FELT A STRONG VIBRATION. WE PROCEEDED TO LAND AND SHUT DOWN ON THE H (NO TAXI). POST FLIGHT REVEALED THAT ONE TAIL ROTOR DAMPER DEPARTED THE AIRCRAFT.
DURING BEFORE LANDING CHECK, MAIN GEAR FAILED TO LOCK DOWN. EMERGENCY PROCEDURE PERFORMED, GEAR LOCKED DOWN WITH BUTTON. NR 2 HYDRAULIC MODULE LEAKING INTERNALLY. MODULE TO BE REPLACED WITH SERVICEABLE.
ON SHORT FINAL, A WAFT OF SMOKE ENTERED THE COCKPIT ACCOMPANIED BY ELECTRICAL BURNING SMELL. LIGHT CONTROL PANEL HAS SMOKE SMELL. CONTROL PANEL WILL BE REPLACED.
SEPERATION INSIDE BOOM IN VERTICAL PYLON AREA BOOM STATION 11,951 TO 12,052. REPLACED WITH NEW TAIL STRUCTURE.
LEFT SIDE BONDED PANEL SEPERATED ON SIDE SKIN AT BOOM STATION FROM 9970 TO 10.149 ON INSIDE SKIN TWO PLACES BOOM STATION 9817 TO 9868 AND 9970 TO 10.274. REPLACED TAIL STRUCTURE.
AFTER REVIEW OF HUMS DOWNLOAD, FOUND HIGH TAIL ROTOR AND TAIL ROTOR GEARBOX READINGS. INSPECTED TAIL ROTOR AND FOUND RED TAIL ROTOR BLADE BONDING STRIP BROKEN OFF. REPLACED.
AFTER LANDING POST FLIGHT INSPECTION REVEALED DAMAGE TO UNDERSIDE NEAR TIP OF WHITE MAIN ROTOR BLADE. FOUND YELLOW TAIL ROTOR BLADE EROSION SHIELD EXTENSION BROKE OFF AT ATTACHMENT LOCATION.
