THE CREW REPORTED THAT THE GASPER FAN HAD A BURNING ODOR WITH PACK NR 2 AND NOT RUNNING THE NR 2 RECIRCULATION FAN. ACFT WAS APPROVED FOR RETURN TO SERVICE. MAINTENANCE INSPECTED THE ACFT AND OPERATIONALLY TESTED BOTH RECIRCULATING FANS WITH NO DEFECTS OR ODORS NOTED, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.

DURING C-CHECK CARGO DOOR LOWER THRESHOLD CORRODED. MAINTENANCE REPAIRED THRESHOLD IAW SRM 51-40-02. RETURNED TO SERVICE.
DURING C-CHECK SHEAR TIE AT FRAME 75, S2L-2R CORRODED. MX R & R SHEAR TIE IAW EC5310-01317. RETURNED TO SERVICE.
DURING C-CHECK, CENTER SERVICE DOOR SILL CORRODED. MAINTENANCE R & R DOOR SILL IAW SRM 51-40-02. RETURNED TO SERVICE.
DURING ROUTINE MX THE EMERGENCY LIGHT ABOVE THE F/A SEAT FAILED TO ILLUMINATE WHEN TESTED. MX REPLACED THE EMERGENCY LIGHT INVERTER, OPERATIONALLY TESTED THE SYS AND CLEARED THE DISCREPANCY.
CREW REPORTED, SHORTLY AFTER TAKEOFF A SMOKE ODOR IN THE CABIN AND THE COCKPIT. A LAVATORY SMOKE INDICATION THEN APPEARED ON THE EICAS. REPORTED THAT AFTER APPROXIMATELY 30 SECONDS THE ODOR AND THE INDICATION BOTH WENT AWAY. THE AIRCRAFT LANDED WITHOUT INCIDENT. MAINTENANCE WAS ABLE TO DUPLICATE THE ODOR AND ISOLATED THE SMELL TO THE LEFT ENGINE BLEED SYSTEM. REPORTED THAT WITH THE ENGINE BLEED CLOSED, THE ODOR WAS NOT PRESENT. MAINTENANCE LOCKED THE LEFT ENGINE BLEED SYSTEM CLOSED AND DEFERRED THE ITEM. THE DEFERRAL WAS CLEARED IN KSDF OF 6/13/2011 WHEN AN OPS CHECK OF THE LEFT BLEED SYSTEM WAS PERFORMED WITH NO DEFECTS NOTED.
DURING INSPECTION, THE RIGHT AFT EMERGENCY EXIT LIGHT WAS FOUND TO BE INOPERATIVE. MAINTENANCE PERFORMED AN OPERATIONAL CHECK OF THE EMERGENCY LIGHTS WITH NO DEFECTS NOTED.
CREW REPORTED THAT UPON TAKEOFF, AT APPROXIMATELY 400 FEET, CREW BECAME AWARE OF SMOKE IN THE COCKPIT AND CABIN. AFTER LEVELING OFF AT 4000 FEET, THE SMOKE RAPIDLY DISSIPATED AND WENT AWAY WITH POWER REDUCTION AND THE RECIRCULATION FANS TURNED OFF. CREW DECLARED AN EMERGENCY AND RETURNED TO KSDF. THE AIRCRAFT LANDED WITHOUT INCIDENT. UPON LANDING, EMERGENCY PERSONNEL CHECKED THE AIRCRAFT BEFORE RETURNING IT TO THE GATE. MAINTENANCE REPLACED THE ENGINE ONE ZERO CARBON SEAL. OPERATIONAL CHECKS AND LEAK CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED.
CREW REPORTED THAT ON CLIMB-OUT FROM MKE, RECEIVED A RUDDER SYS 1-2 INOP MESSAGE ALONG WITH A YAW DAMP FAIL MESSAGE ON EICAS. THE QRH WAS FOLLOWED AND THE MESSAGE CLEARED, BUT RETURNED AFTER ABOUT 5 MINUTES. THE QRH WAS FOLLOWED AGAIN AND THE MESSAGE CLEARED, AND IT WAS DECIDED TO RETURN TO MKE WHERE THE AIRCRAFT LANDED WITHOUT INCIDENT. MAINTENANCE REPLACED THE NR 2 RUDDER PRESSURE SWITCH AND THE RUDDER SYSTEM OPERATED NORMALLY. THE FAULT RETURNED ON THE NEXT FLIGHT TO SDF AND THE NR 2 PRESSURE SWITCH WAS FOUND TO BE BAD FROM STOCK. THE NR 2 PRESSURE SWITCH WAS REPLACED AGAIN WITH NO FURTHER DIFFICULTIES.
AFTER LANDING THE CREW REPORTED THAT HIGH SPEED BRAKING WAS INEFFECTIVE. BOTH BRAKE PEDALS WENT TO THE FLOOR WITH LITTLE EFFECT. CREW ALSO REPORTED THAT BRAKING SEEMED TO IMPROVE AT LOWER SPEEDS. MAINTENANCE FOUND FAULTS ON THE CMC FOR BRAKE PPTAN 3 FAIL AND CONFIRMED THE FAULT THROUGH FIM 32-41-00-810-812-A. THE NR 3 PEDAL POSITION TRANSDUCER WAS REPLACED AND THE BRAKE SYSTEM WAS TAXI CHECKED WITH THE ANALYZER SOFTWARE AND NO FURTHER DEFECTS WERE NOTED.
AFTER TAKEOFF WHEN A POSITIVE RATE OF CLIMB WAS ESTABLISHED, THE GEAR HANDLE WAS SELECTED TO THE UP POSITION, AND AFTER A FEW SECONDS THE CREW RECEIVED A RED GEAR DOWN INDICATION AND MASTER WARNING ON EICAS OF LG/DISAGREE. THE QRH WAS FOLLOWED AND THE AIRCRAFT RETURNED TO MKE AND LANDED WITHOUT INCIDENT. MAINTENANCE REPLACED THE LANDING GEAR SOLENOID VALVE AND PERFORMED GEAR SWINGS WITH NO FURTHER DEFECTS NOTED.
FLIGHT 1206, IND TO MKE. AFTER TAKEOFF, THE CREW SELECTED THE GEAR UP AND RECEIVED A 3 RED INDICATION SHOWING DOWN IN ADDITION TO LG LEVER DISAGREE ON EICAS. THE QRH WAS FOLLOWED AND THE FAULT DID NOT CLEAR. THE AIRCRAFT RETURNED TO IND AND LANDED WITHOUT INCIDENT. MAINTENANCE REPLACED THE LGEU AND PERFORMED OPS CHECKS WITH NO FURTHER DEFECTS NOTED.
FLIGHT 1119, MKE TO SDF. CREW REPORTED THAT AFTER DEPARTURE THE LANDING GEAR FAILED TO RETRACT ALONG WITH A LANDING GEAR LEVER DISAGREE MESSAGE. THE CREW RAN THE QRH PROCEDURE WITH NO SUCCESS. THE AIRCRAFT THEN RETURNED TO MKE AND LANDED WITHOUT INCIDENT. MAINTENANCE PERFORMED MULTIPLE GEAR SWING OPERATIONAL CHECKS AND FOLLOWED FIM 32-32-00-810-803 WITH NO DEFECTS NOTED AND THE AIRCRAFT WAS RETURNED TO SERVICE.
IAH - FLIGHT 2206, THE CREW REPORTED LANDING GEAR DOES NOT RETRACT PROPERLY, LANDING GEAR HANDLE SELECTED UP GEAR INDICATION 3 DOWN FOR 5 - 10 SEC THEN IN TRANSIT AND THEN GEAR UP. THE AIRCRAFT LANDED WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED LANDING GEAR HANDLE, OPS CHECK GOOD REF EMB 145 MM 32-32-02, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH - FLIGHT 2922 AFTER TAKEOFF, THE CREW SELECTED GEAR LEVER TO UP POSTION BUT GEAR DID NOT RETRACT, CYCLED LEVER BACK DOWN THEN UP RETRACTED NORMAL. THE AIRCRAFT CONTINUED TO IAH WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE PERFORMED EXTENSION AND RETRACTION CHECKS WITH NO DEFECTS NOTED, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH - FLIGHT 3109, THE CREW REPORTED AFTER T/O THE GEAR STARTED TO RETRACT, GOT AMBER THEN RED INDICATIONS WITH LANDING GEAR LEVER DISAGREE MESSAGE. THE GEAR STOWED AT 1300 FT. THE AIRCRAFT RETURNED TO IAH WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED THE AIRCRAFT OPERATIONALLY CHECKED THE EXTENSION & RETRACTION SYSTEMS WITH NO DEFECTS NOTED, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
LRD - FLIGHT 2359, DURING PREFLIGHT INSPECTION, THE CREW REPORTED THAT BOTH SIDE PANEL EXIT LIGHTS AT THE OVERWING EXITS WERE INOPERATIVE. THE CREW RESET THE EMERGENCY LIGHT SYSTEM, OPERATIONALLY CHECK WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
SHV - DURING C-CHECK APU FIRE WALL CRACK AROUND NUT PLATE FASTENERS AT 3 O`CLOCK POSITION. MAINTENANCE REPAIRED APU FIREWALL AT 3 O`CLOCK POSITION IAW EC 5330-01483. THE AIRCRAFT WAS APPROVED TO CONTINUED FOR SERVICE.
SHV - DURING C-CHECK CORROSION ON RT OMEGA BEAM AT FR 18. MAINTENANCE REPAIRED RT OMEGA BEAM AT FR 18 IAW EMB 145 SRM 53-21-10. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
DURING A SCHEDULED INSPECTION CORROSION WAS FOUND ON THE PASSENGER ENTRY DOOR THRESHOLD PLATE. MAINTENANCE REMOVED AND REPLACED THE THRESHOLD PLATE IAW EMB SRM 51-21-00. THE AIRCRAFT WILL BE RELEASED UPON COMPLETION OF INSPECTION.
DURING CLIMB THE NOSE GEAR DOORS DID NOT RETRACT AFTER THE NOSE LANDING GEAR WAS UP AND LOCKED. THE AIRCRAFT RETURNED TO XNA AND LANDED WITHOUT INCIDENT. THE AIRCRAFT WAS FERRIED TO IAH WHERE MAINTENANCE REMOVED AND REPLACED THE NOSE LANDING GEAR UPLOCK PROXIMITY SWITCH. THE LANDING GEAR SYSTEM WAS OPERATIONALLY TESTED AND THE DISCREPANCY CLEARED.
CREW REPORTED, AUTO PRESSURIZATION FAILED DURING DESCENT. AIRCRAFT WAS RELEASED IAW MEL 21-31-03/1 CONTROL NUMBER 037305. AIRCRAFT RETURNED TO SERVICE. MAINTENANCE REMOVED AND REPLACED DIGITAL PRESSURIZATION CONTROLLER IAW EMB135 AMM 21-31-01. OPS CHECKED GOOD. REMOVED FROM MEL. AIRCRAFT RETURNED TO SERVICE.
CLE FLIGHT 2051 THE CREW REPORTED THE RIGHT OVERWING EMERGENCY LIGHT WAS INOPERATIVE. MAINTENANCE INSPECTED AND REPLACED THE RIGHT WING AFT EMERGENCY LIGHT. OPERATIONAL CHECKS WERE GOOD.
DURING ROUTINE MAINTENANCE THE EMERGENCY AISLE PATH LIGHTS WERE DIM. MAINTENANCE REPLACED THE NR 2 EMERGENCY BATTERY CHARGER. OPERATIONAL CHECKS WERE GOOD.
TYS- DURING A SCHEDULED 1-C INSPECTION THE FIRE HANDLE NR 2 WOULD NOT LIGHT UP DURING THE FIRE DETECTION TEST. MAINTENANCE REMOVED AND REPLACED NR 2 ENGINE FIRE SHUTOFF AND EXTINGUISHING HANDLE. OPS CHECK WAS GOOD. A/C WILL BE RELEASED UPON COMPLETION OF ALL WORK.
TYS, DURING A SCHEDULED INSPECTION A TEMPORARY REPAIR BETWEEN FRAMES 24 AND 25 AT STRINGER 12R, WAS MADE PERMANENT. REMOVED THE OLD REPAIR STRINGER 12R, CUT OUT A SECTION OF FR 24 AND INSTALLED A FRAME SPLICE, DOUBLER AND STRINGER IAW SRM 53-00-04, 53-22-01, 51-21-04 AND 05. THE REPAIR WAS SATISFACTORY AND AIRCRAFT WILL BE RELEASED UPON COMPLETION OF INSPECTION.
TYS, DURING A SCHEDULED INSPECTION A DENT WAS FOUND ON THE LEFT FORWARD FUSELAGE BETWEEN FRAMES 10 AND 11 AND BETWEEN STRINGERS 13L AND 15L. MAINTENANCE REMOVED THE DAMAGED SHEAR CLIP, RESTORED SKIN AND INSTALLED A NEW SHEAR CLIP IAW EC5330-01246. THE REPAIR WAS SATISFACTORY AND THE AIRCRAFT WILL BE RELEASED UPON COMPLETION OF INSPECTION.
TYS, DURING A SCHEDULED INSPECTION THE FORWARD DIGITAL PLATE WAS FOUND CORRODED AFT OF THE COCKPIT STEP IN THE GALLEY. MAINTENANCE REMOVED AND REPLACED THE CENTER FORWARD DIGITAL IAW EMB-145 SRM 53-21-10, 51-40-02, 51-21-04/05 AND 06. THE REPAIR WAS SATISFACTORY AND THE AIRCRAFT WILL BE RELEASED UPON COMPLETION OF INSPECTION.
CLE FLIGHT#3141 THE CREW REPORTED A LANDING GEAR AIR/GROUND FAIL MESSAGE DURING FLIGHT ASSOCIATED WITH "THUMP" NOISE UNDER NOSE.THE AIRCRAFT LANDED WITHOUT FURTHER INCIDENT.MAINTENANCE REPLACED THE LEFT MAIN LANDING GEAR WOW PROXIMITY SWITCH WIRE HARNESS W510.OPERATIONAL CHECKS WERE GOOD.THE AIRCRAFT WAS RELEASED FOR SERVICE.
SLP - FLIGHT 4153, THE CREW REPORTED A STRONG SMELL OF BURNING RUBBER FROM THE NR 2 COFFEE JUG. THE AIRCRAFT WAS RELEASED IAW 25-60-02/3D. MAINTENANCE REPLACED THE COFFEE POT RECEPTACLE IN THE GALLEY. OPERATIONAL CHECKS WERE GOOD. THE AIRCRAFT WAS RELEASED FOR SERVICE.
DTW FLIGHT 3318 THE CREW REPORTED A NR 2 FIRE DETECT FAIL MESSAGE ON TAKEOFF. THE TAKEOFF WAS ABORTED AND AIRCRAFT RETURNED TO THE GATE. MAINTENANCE INSPECTED AND REPLACED THE NR 2 ENGINE FIRE LOOP DETECTOR AND CLEANED THE NR 2 ENGINE FIRE BOTTLE CANNON PLUG.
