NR 1 INS SCREEN WENT BLANK, NR 1 INU FAILED. NR 2 INS WARNING FLAG FLASHING, NR 2 INU FAILED. INFORMATION REPORTED FROM FLIGHT CREW IS AS FOLLOWS. CREW DECLARED EMERGENCY. ON DESCENT FROM FL 260 TO 6000 WITHIN 75 MILES OF DESTINATION. THE NR 1 INS CDU WENT BLANK ALONG WITH NR 1 EADI AND NR 1 EHSI. THE LOSS OF 1 INS CHECKLIST WAS PERFORMED. THE ISSP WAS PUSHED SELECTING THE NR 2 SIDE INFO BE SHOWN ON NR 1. EHSI WAS RESTORED. CONTINUED VISUAL APPROACH TO LAND, RUNWAY 17L. 17L-17R-35L AT DESTINATION WERE CLOSED AND ILS FOR 17R WAS OUT OF SERVICE. RUNWAY 14-32 WAS DEEMED NOT LONG ENOUGH FOR SAFE LANDING. WHILE ON MISSED APPROACH THE NR 2 INS STARTED FLASHING WARNING. THE INS MESSAGE WAS "DELAYED MX" DURING WHICH THE NR 2 EADI WAS LOST. THE NR 2 EHSI APPEARED TO FUNCTION BUT ALSO HAD RED FLAG. AT THIS TIME AN EMERGENCY WAS DECLARED. ATC NOTIFIED. CONTINUED ON STAND BY ATTITUDE GYRO, PITOT STATIC INSTRUMENTS. ATC SUGGESTED LANDING, DUE TO WEATHER AND RUNWAY LENGTH. PROCEEDED TO AIRPORT AND LANDED.
IN CRUISE NR 4 ENGINE LOW OIL PRESSURE LIGHT ON STEADY, ENGINE OIL PRESSURE INDICATOR 50 PSI . PERFORMED INFLIGHT ENGINE SHUTDOWN. WINDMILLING OIL PRESSURE 10 PSI.
NR1, IMMEDIATELY AFTER ROTATION, NR 1 INU FAILED, CAPT INSTRUMENTS FLAGGED FAIL. NR 2, IMMEDIATELY AFTER ROTATION GPWS WENT INOP. NR 3, IMMEDIATELY AFTER ROTATION YAW DAMPER KICKED OFF AND A/P OR YAW DAMPER WOULD NOT ENGAGE FROM EITHER PILOTS A/P TRANSFER SWITCH POSITION. NR1, REPLACED NR 1 INU IAW AMM 34-44-03-4, PERFORMED ADJ & TEST IAW AMM 34-44-08-05. OP'S CKS GOOD. NR 2, PERFORMED ADJ & TEST OF GPWS IAW AMM 34-56-06 AFTER NR 1 INU REPLACEMENT, OP'S CKS GOOD. NR3, REPLACED NR1 INU IAW AMM 34-44-03-4, PERFORMED ADJ & TEST OF AUTOPILOT SYS IAW AMM 22-10-00-05, OPS CKS GOOD.
FOUND BULKHEAD (NR 45346-000) LOCATED AT STA 46.0 TO HAVE CRACKS IN RADIUS OF VERTICAL FLANGE, EXTENDING APPROXIMATELY .5 INCH FROM THE TOP. (K)
FOUND CRACKS EMINATING FROM LAST RIVET ON INBOARD UPPER ENGINE SUPPORT BRACKET. IT APPEARS THAT THIS RIVET WAS SMASHED HARDER THAT NECESSARY DURING MANUFACTURE.
GEAR UNSAFE LIGHT STAYED ON AFTER GEAR RETRACTION. FOUND FORWARD SUPPORT BRACKET FOR RT MLG UPLOCK FRACTURED AND UPLOCK ASSY BENT. BOLT HOLE ACROSS FRACTURE SHOWS EVIDENCE OF PRE-EXISTING CRACK. NOT VISIBLE WITHOUT REMOVAL OF UPLOCK ASSY.
ENGINE SHUT DOWN SHORTLY AFTER TAKEOFF. FOUND ENGINE FUEL PUMP DRIVE COUPLING SHEARED (COUPLING WORN OUT). REPLACED FUEL PUMP AND ENGINE FUNCTIONING CHECK OK. PART TOTAL TIME: 2,009.0 HOURS SMOH.
RIGHT THROTTLE CABLE BROKE DURING ENGINE POWER UP. PART TOTAL TIME: 7,358.0 HOURS.
ON APPROACH TO SVE WITH FLAPS FULLY EXTENDED, THE RT FLAP SUDDENTLY RETRACTED CAUSING SEVERE YAW. LT FLAP WAS RETRACTED AND REGAINED CONTROL. ABORTED LANDING AND RETURNED TO RNO. TRANSMISSION FAILED INTERNALLY. DRIVEN GEAR DISINTEGRATED. NOTE: WEAR CHECKS PERFORMED PER AD 82-27-13R2 EACH 100 HOURS. SUBMITTER RECOMMENDS REPLACING OLDER DUKES TRANSMISSIONS AND DECREASING THE INTERNAL INSPECTION INTERVAL.
