THE FLIGHT CREW REPORTED THE LEFT AND RIGHT REVERSER FAULT WHITE CAS MESSAGE ILLUMINATED AFTER TAKEOFF. THE LEFT ENGINE AMBER REV MESSAGE ILLUMINATED DURING POWER REDUCTION. THE CREW RAN THE CHECKLIST AND LEFT REVERSER DEPLOY CIRCUIT BREAKER WAS PULLED ACCORDING TO THE CHECKLIST. THE FAULT WAS DEFERRED BY MEL 78-30-01-02 AND OPERATIONS WERE CONTINUED. THE MAINTENANCE PROVIDER REMOVED THE NR 1 INTERMITTENT SQUAT SWITCH AND REPLACED IT WITH A NEW SWITCH. THE SWITCH WAS RIGGED OPERATIONS WERE VERIFIED AND THE AIRCRAFT WAS RETURNED TO SERVICE.
THE FLIGHT CREW REPORTED THAT THE ENTIRE OUTER GLASS PLY OF THE LEFT WINDSHIELD SHATTERED IN FLIGHT AT FLIGHT LEVEL 43,000 FEET, MACH 0.76, SAT -59 C, NEGATIVE ICING, CABIN 9.1 PSID, CABIN ALTITUDE 6,600 FEET. THE CREW ELECTED TO LAND FOR EVALUATIONS AT KPDK. THE LANDING WAS UNEVENTFUL. THE MAINTENANCE TEAM WAS UNABLE TO DEFINE A CAUSE FOR THE WINDSHIELDS FAIL, NO OBVIOUS DEFECTS WERE FOUND. THE REPLACEMENT WILL BE DELAYED BY MATERIALS SHORTAGE AND THE AVAILABILITY OF A NEW WINDSHIELD. NO PART OR SERIAL NUMBER AVAILABLE FROM THE FAILED WINDOW AT THIS TIME.
FLIGHT CREW REPORTED A PRI TRIM FAIL AMBER CAS INTERMITTENTLY WHEN DISCONNECTION AUTOPILOT AT FINAL APPROACH SPEEDS WITH FLAPS AT 40 DEGREES. THE MAINTENANCE PROVIDER TROUBLESHOT AND FOUND THE HORIZONTAL STAB ACTUATOR AT FAULT. THE STAB ACTUATOR WAS REPLACED WITH AN OVERHAULED UNIT. FUNCTIONAL CHECKS WERE SATISFACTORY. THE AIRCRAFT WAS RETURNED TO SERVICE.
THE FLIGHT CREW REPORTED THAT UPON BATTERY POWER UP IMMEDIATE CLICKING NOISE HEARD FROM TAIL SECTION WHILE IN PRIMARY TRIM MODE. THE MAINTENANCE VENDOR TROUBLESHOT AND DETERMINED THAT THE HORIZONTAL STAB PITCH TRIM ACTUATOR WAS DEFECTIVE. THE DEFECTIVE UNIT WAS REMOVED AND EXCHANGED FOR AN OVERHAULED ACTUATOR UNIT. OPS CHECKS GOOD. AIRCRAFT WAS RETURNED TO SERVICE. TEARDOWN REPORT REQUESTED.
THE FLIGHT CREW REPORTED THAT THE STANDBY ATTITUDE INDICATOR PRECESSING IN SMOOTH AIR LEVEL FLIGHT 20 DEGREES NOSE DOWN AND 19 DEGREES BANK AFTER 28 MINS IN FLIGHT. THE MAINTENANCE TEAM DETERMINED THAT THE STANDBY ATTITUDE INDICATOR WAS DEFECTIVE AND WAS REPLACED WITH A REPAIRED UNIT. OPS CHECKS WERE SATISFACTORY AND THE AIRCRAFT WAS THEN RETURNED TO SERVICE .
THE FLIGHT CREW REPORTED THE RIGHT THRUST REVERSER INTERMITTENTLY WILL NOT DEPLOY WHEN REVERSER LEVER WAS RAISED. OPERATIONS WERE CONTINUED PER MEL. ONCE INTO MAINTENANCE THE VENDOR TROUBLESHOT AND DETERMINED THAT THE THROTTLE CONTROL MODULE (TCM) WAS DEFECTIVE. THE DEFECTIVE UNIT WAS REMOVED AND AN REPAIRED UNIT WAS INSTALLED. OPERATIONS WERE SATISFACTORY AND THE AIRCRAFT WAS RETURNED TO SERVICE.
THE FLIGHT CREW REPORTED AFTER LANDING AND DEPLOYING THE TRUST REVERSERS THE RIGHT HAND AMBER REV ANNUNCIATOR ABOVE RIGHT N1 ILLUMINATOR ON LANDING. THE MAINTENANCE VENDOR TROUBLESHOT THE RIGHT T/R FOR HAVING AMBER REV UPON LANDING BY SWAPPING THE LH AND RH TIME DELAY RELAYS AND VERIFIED THAT THE LH TIME DELAY RELAY WAS DEFECTIVE. REMOVED THE DEFECTIVE LH (K247) TIME DELAY RELAY AND INSTALLED A NEW RELAY. PERFORMED SATISFACTORY OPERATIONAL CHECKS WITH NO DEFECTS. THE AIRCRAFT THEN WAS RETURNED TO SERVICE.
