ON TAKEOFF THE NR 1 PROPELLER WAS DAMAGED BY ROCKS DUE TO SOFT SPOT IN GRAVEL RUNWAY. MX REPLACED PROPELLER, AND ENGINE WAS TO BE INSPECTED PER LYCOMING SB 475C AND AD 2004-10-14 BEFORE BEING RETURNED TO SERVICE.
WHILE IN IFR CRUISE EXPERIENCED TOTAL ELECTRICAL FAILURE. BY SHUTING OFF ANTI-ICE EQUIPMENT WAS ABLE TO RESTORE POWER. RECEIVED PRIOITY ATC HANDLING LANDED IN DLG WITHOUT INCIDENT. MX FOUND A BROKEN WIRE ON BACK OF RIGHT HAND ALTERNATOR.
PILOT NOTICED ALTIMETER APPEARED TO BE STUCK. THIS WAS CONFIRMED BY CROSS REFERENCEING THE CO-PILOT AND GPS ALTIMETERS. REPLACED FAULTY ALTIMETER.
DURING CRUISE, PILOT SMELLED FUEL. AFTER LOOKING AROUND THE COCKPIT, PILOT NOTICED A SMALL FUEL STAIN IN THE COPILOT''S CARPET. THE AIRCRAFT IMMEDIATELY RETURNED TO THE AIRPORT. REPLACED FUEL LINE TO FUEL FLOW INDICATOR.
UPON GEAR RETRACTION UNSAFE LIGHT WOULD NOT EXTINGUISH AND SELECTOR WOULD NOT GO TO NEUTRAL. RETURNED TO AIRPORT FOR UNEVENTFUL LANDING. CLEANED AND LUBED ALL GEAR SWITCHES, OPS CHECK WAS FOUND NORMAL.
ON APPROACH TO MYU, RT ENGINE RPM INDICATION WENT TO ZERO, ENGINE RUNNING OK. RETURNED TO BET FOR UNEVENTFUL LANDING. REPLACED RPM INDICATOR, OPS CHECKED OK.
RT ALTERNATOR LIGHT CAME ON WHILE CRUISING IN PRECIP. RETURNED TO BASE. REPLACED ALTERNATOR PULLEY DUE TO WEAR.
WHILE IN CRUISE, THE PILOT NOTED A RAPID DROP IN FUEL QTY INDICATION. SHORTLY THEREAFTER THE LT ENG LOST POWER. FOLLOWED BY A RT ENG POWER LOSS 2 MIN LATER. A SUCCESSFUL LANDING WAS ACCOMPLISHED WITH NO DAMAGE TO THE ACFT. UPON FURTHER INVESTIGATION, IT WAS FOUND THAT 5 OF THE FUEL QTY SENDING UNITS IN THE ACFT DID NOT MEET THE CORRECT OHM VALUE. THE FAULTY SENDERS WERE REPLACED AND THE ACFT WAS RETURNED TO SERVCE. SUBSEQUENTLY A FLEET WIDE SURVEY (6 ACFT) WAS CONDUCTED. APPROX 40 PERCENT OF THE SENDERS IN SERVICE WERE FOUND OUT OF TOLERENCE. IN REPLACING THESE SENDERS IT WAS NOTED THAT 50 PERCENT OF THE NEW SENDERS ACQUIRED FROM PIPER WERE ALSO OUT OF TOLERENCE AND THEREFORE REJECTED.
WHILE IN CRUISE, THE PILOT NOTED A RAPID DROP IN FUEL QUANTITY INDICATION. SHORTLY THERE AFTER THE LEFT ENGINE LOST POWER. FOLLOWED BY A RT ENGINE POWER LOSS TWO MINUTES LATER. A SUCCESSFUL LANDING WAS ACCOMPLISHED WITH NO DAMAGE TO THE AIRCRAFT. CURRENTLY THIS INCIDENT IS UNDER INVESTIGATION.
LT ENGINE WOULD NOT DEVELOPE FULL POWER, ABORTED TAKEOFF. REPLACED TURBO CHARGER, OPERATIONAL CHECK OK.
NOSE GEAR WOULD NOT LOCK UP. CLEANED AND LUBED DIRTY NOSE GEAR UPLOCK ROLLER, OPS CHECK OK.
