PACK 1 VALVE CLOSED IN FLIGHT. FOUND BUSTED SLEEVES AFT OF NR 1 DUAL TEMP CONTROLLER AND FWD OF PACK 1 VALVE. REPLACED SLEEVES AND OPERATED PACK CHECKED THE NR 1 BLEED/PACK SYSTEM GOOD DUE TO PACK 1 FAIL IN FLIGHT. CHANGED THE NR 1 PACK PUSH BUTTON OPS CHECK GOOD.
ON POST FLIGHT, FOUND MAIN CABIN DOOR DIFFICULT TO OPERATE. LUBRICATED MAIN CABIN DOOR SEALS. OPS CHECK GOOD.
PAX DOOR SWING EMERGENCY LIGHT NR 1 LENS CRACKED. R&R PAX DOOR SWING EMERGENCY LIGHT NR 1 LENS OPS CHECK GOOD.
LANDING GEAR AIR GROUND FAIL WITH ASSOCIATED E1 AND 2 REVERSER DISAGREE. PERFORMED NOSE LANDING GEAR SERVICING. NO LANDING GEAR AIR/GRD FAIL ON EICAS AT THIS TIME.

PASSENGER DOOR EMERGENCY FLOOD LIGHT NR 1 IS INOPERATIVE. MAINTENANCE REPLACED LIGHT, OPS CHECKED GOOD.
COCKPIT EMERGENCY OVERHEAD LIGHT IS INOPERATIVE. MAINTENANCED REPLACED LIGHT, OPS CHECK GOOD.
EMERGENCY LIGHTS, NR 19, LT OVERWING EXIT IDENTIFIER, NR 7, COCKPIT FLOODLIGHT AND NR 13 MAIN DOOR FLOOD LIGHT, ARE INOPERATIVE. MAINTENANCE REPLACED LIGHTS, OPS CHECK GOOD.
DURING C-CHECK, STRINGER 20R WAS FOUND TO BE CORRODED BETWEEN FRAMES 61-67. MAINTENANCE REMOVED THE CORROSION AND PERFORMED A SPLICE REPAIR IAW SRM 53-00-03. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK THE CARGO DOOR CUTOUT LOWER BEAM WAS FOUND TO BE CORRODED. MAINTENANCE R & R THE BEAM IAW SRM 51-40-02. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK THE FRAME 68 SEGMENT BETWEEN STRINGERS 15-16L WAS FOUND TO BE CORRODED. MAINTENANCE REMOVED THE CORROSION AND PERFORMED A SPLICE REPAIR IAW SRM 53-31-04. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK, FRAME 66 WAS FOUND TO DAMAGED BETWEEN STRINGERS 17-19L. MAINTENANCE REMOVED THE DAMAGED AREA AND PERFORMED A SPLICE REPAIR IAW SRM 53-00-04. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING PREFLIGHT, THE CREW REPORTED THAT THE EMERGENCY EXIT SIGN AT 18A WAS INOPERATIVE. MAINTENANCE RESET THE EMERGENCY LIGHTING SYS, OPERATIONALLY TESTED WITH NO DEFECTS NOTED, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
THE CREW REPORTED THAT THE NR 2 MFD WENT BLANK IN FLIGHT FOLLOWED BY AN ELECTRICAL BURNING ODOR. CIRCUIT BREAKER 12 WAS PULLED AND THE ODOR WENT AWAY. AIRCRAFT LANDED WITHOUT INCIDENT WHERE MAINTENANCE REPAIRED SPLICE SP2160, OPS, MFD NR 2 WITH NO DEFECTS, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING MX, INSPECTION THE RT INTERIOR WING EXIT MARKER WAS FOUND NOT TO ILLUMINATE WHEN TESTED. MX R & R THE RT INTERIOR EMERGENCY EXIT MARKER WITH OPS CHECK SUCCESSFUL. ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING MX INSPECTION THE RT OVERWING LOWER FWD EMERGENCY EXIT SIGN WAS FOUND NOT TO ILLUMINATE. MX R & R THE NR 3 INVERTER WITH OPS CHECKED SUCCESSFUL. ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING MX INSPECTION THE CEILING EMERGENCY LIGHT AT ROW 12 WAS FOUND INOPERATIVE. MX R & R THE BULB WITH OPS CHECKED SUCCESSFULLY. ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C CHECK SKIN SHOWS SIGN OF CRACKING BETWEEN LT DIRECT VISION WINDOW AND PILOTS WINDSHIELD (FR 10 STR 4L). MX PERFORMED REPAIR OF ACFT SKIN BETWEEN LT DIRECT VISION WINDOW AND PILOTS WINDSHIELD (FRM 10 STR 4L) IAW EC5310-01629, CURE CHECK GOOD. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING POWER UP THE PILOT REPORTED THAT THE COCKPIT FILLED WITH SMOKE & ODOR. MX INSPECTED THE ACFT, R & R THE FUEL PUMP CONTACTORS 2A & 2B, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING POWER UP THE PILOT REPORTED TAHT THE COCKPIT FILLED WITH SMOKE & ODOR-. MX INSPECTED THE ACFT AND R & R FUEL CONTACTORS 2A & 2B. OPERATIONALLY TESTED WITH NO DEFECTS, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK STRINGER 21R CORRODED (LOCAL LEVEL 2) FRMS 12-13. MX PERFORMED REPAIR ON STRINGER 21R BETWEEN FR 12-13 IAW SRM 53-00-03, FIG. 202. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK STRINGER 21L CORRODED (LOCAL LEVEL 2) FRMS 12-13. MX PERFORMED REPAIR OF STRINGER 21L BETWEEN FRM 12-13 IAW SRM 53-00-03 FIG 202. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
ORD FLT 6116 THE CREW REPORTED AT FL360 HAD A AIRSPEED MISCOMPARE OF 7 KNTS. INITIATED A SLOW DESCENT AND RECEIVED A HIGH SPEED BULLET WITH ASSOCIATED ROLL. DESCENDING THRU FL340 CAPTAINS AIRSPEED AND ALTITUDE WENT BLANK. ALL AIRSPEED AND ALTITUDE DATA RESTORED AS CREW CONTINUED THE SLOW DESCENT. THE AIRCRAFT DIVERTED INTO FSD AND LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE INSPECTED AND REPLACED THE AIR DATA PROCESSOR UNIT. OPERATIONAL CHECKS WERE GOOD.
SHV - DURING C-CHECK RT OMEGA BEAM AT FRAME 23 IS CORRODED. MAINTENANCE PERFORMED REPAIR ON RT OMEGA BEAM AT FRAME 23 IAW EMB-145XR EC5320-01443 REV C. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
SHV - DURING C-CHECK CORROSION ON RT OMEGA BEAM AT FRAME 22. MAINTENANCE PERFORMED REPAIR ON RT OMEGA BEAM REPAIR AT FRAME 22 IAW EMB-145 SRM 53-21-10. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
SHV - DURING C-CHECK LOWER DOOR CUTOUT INTERNAL SIDE CORRODED (CARGO DOOR). MAINTENANCE REMOVED AND REPLACED LOWER DOOR (CARGO) CUTOUT BEAM IAW EMB145XR SRM 51-40-02. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
SHV - DURING C-CHECK LT OMEGA BEAM IS CORRODED AT FRAME 20-23. MAINTENANCE PERFORMED REPAIR TO LT OMEGA BEAM FRAME 20-23 IAW EC5320-01579 REV ORIG. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
FLIGHT 2511 - EWR - FLIGHT CREW REPORTED DURING TAXI, AFTER LETTING GO OF THE TILLER, THE NOSE WHEEL TURNS HARD TO THE RIGHT. EWR MX REMOVED AND REPLACED FEEDBACK POTENTIOMETER AND PERFORMED OPERATIONAL CHECK SUCCESSFULLY. ACFT WAS APPROVED TO RETURN TO SERVICE.
EWR FLT 4923 - MAINTENANCE REPORTED THE NR 2 ENGINE WAS BLOWING SMOKE INTO THE CABIN. MAINTENANCE REPLACED THE NR 2 ENGINE AND OPERATIONAL CHECKS WERE GOOD. THE TEAR DOWN REPORT FROM MANUFACTURER STATED THE NR 4 TEFLON LIP SEAL WAS THE PRIMARY FAILED ITEM ON THIS ENGINE. THE ENGINE WAS STRIPPED AND RESTORED TO A SERVICEABLE CONDITION.
IAH - FLIGHT 2318, DURING CLIMB THE CREW REPORTED THAT THE PRIMARY PITCH TRIM SWITCH WAS INOPERATIVE. THE AIRCRAFT RETURNED TO IAH WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE F/O'S YOKE PITCH TRIM SWITCH, OPERATIONALLY TESTED WITH NO DEFECTS. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
TYS - DURING C-CHECK SMALL PITS WERE FOUND AT 3 MOUNT POINT HOLES ON THE NR 2 ENGINE INTAKE. THE 3 HOLES WERE REPAIRED IAW EC 5420-01158. A/C STILL IN C-CHECK.
FLIGHT 2679 - RIC - FLIGHT CREW REPORTED FLUCTUATION IN CABIN RATE (0 TO MINUS 100 FPM) ON DESCENT. RIC MX REMOVED AND REPLACED LT ELECTRO PNEUMATIC OUTFLOW VALVE AND PERFORMED OPERATIONAL CHECK SUCCESSFULLY. ACFT WAS APPROVED TO RETURN TO SERVICE.
FLIGHT 2679 - RIC - FLIGHT CREW REPORTED FLUCTUATION IN CABIN RATE (0 TO MINUS 100 FPM) ON DESCENT. RIC MX REMOVED AND REPLACED RT PNEUMATIC OUTFLOW VALVE AND PERFORMED OPERATIONAL CHECK SUCCESSFULLY. ACFT WAS APPROVED TO RETURN TO SERVICE.
MCI - DURING MAINTENANCE INSPECTION THE LEFT FORWARD EXIT EMERGENCY LIGHT WAS FOUND TO BE INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE NUMBER ONE EMERGENCY LIGHT BATTERY POWER SUPPLY, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH - FLIGHT 2133, THE CREW REPORTED THAT THE ISIS ALTITUDE AND SPEED BLANKED OUT DURING CLIMB. THE AIRCRAFT RETURNED TO IAH WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE ISIS COMPUTER, OPERATIONALLY TESTED WITHOUT DEFECT, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
FLIGHT 4917, THE CREW REPORTED A LANDING GEAR LEVER DISAGREE WARNING DURING RETRACTION. THE AIRCRAFT RETURNED TO IAH WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE LANDING GEAR LEVER, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH FLIGHT 2063. THE CREW REPORTED A BLEED 1 OVERTEMP IN FLIGHT. THE CREW CLOSED THE BLEED VALVE IN ACCORDANCE WITH THE QRH AND THE ACFT RETURNED TO IAH WHERE IT LANDED WITHOUT INCIDENT. MX R&R THE ENGINE 1 FAN AIR VALVE. OPS CHECKED WITH NO DEFECTS NOTED AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.

EWR FLIGHT 2558 THE CREW REPORTED UNABLE TO SELECT CLIMB ON THE THRUST RATING PANEL. THE AIRCRAFT DIVERTED INTO CLE AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED AND FOUND THE CLIMB BUTTON TO BE STUCK. REPLACED THE THRUST RATING PANEL AND OPERATIONAL CHECKS WERE GOOD. THE AIRCRAFT WAS RELEASED FOR SERVICE.
IAH - FLIGHT 2953, THE CREW REPORTED THAT CABIN PRESSURE WAS LOST IN FLIGHT. THE AIRCRAFT LANDED AT IAH WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE PRESSURIZATION SYSTEM DIGITAL CONTROLLER, OPERATIONALLY CHECKED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
RIGHT AILERON BRACKET, O/B AND I/B BUSHING IS LOOSE. REMOVED AND REPLACED PCA BRACKET IAW EC 5750-01177, EMB 135/145 CPM 51-21-04 AND EMB 145XR SRM 51-40-02.
TUL - FLIGHT 2403, THE CREW REPORTED A BATTERY 2 OVERTEMP INDICATION ON EICAS. THE CREW DEACTIVATED THE NUMBER TWO BATTERY, AND THE AIRCRAFT LANDED WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE NUMBER TWO MAIN BATTERY, OPERATIONALLY CHECKED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
MFE - FLIGHT 2082, THE CREW REPORTED THAT ON TAKEOFF OUT OF MFE, WITH THE GEAR UP AND LOCKED RECEIVED CONTINUOUS LANDING GEAR AURAL WARNINGS. THE AIRCRAFT RETURNED TO MFE WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED, AND RELEASED THE AIRCRAFT ON MEL 34-41-00 CAT A CTL NUMBER 121205, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE. THE AIRCRAFT RETURNED TO IAH ON 05/14/07 WHERE MAINTENANCE REMOVED AND REPLACED THE LANDING GEAR ELECTRONICS UNIT (LGEU), OPERATIONALLY CHECKED WITH NO DEFECTS, REMOVED THE MEL, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
FLT 2916, DURING CLIMB THE LANDING FAILED TO RETRACT. MAINTENANCE REMOVED AND REPLACED THE NOSE LANDING GEAR PROXIMITY SENSOR, OPERATIONALLY TESTED THE GEAR AND CLEARED THE DISCREPANCY.
TYS DURING A 2C INSPECTION THE FLOOR CENTER AISLE BEAM IN THE GALLEY WAS FOUND CORRODED. MAINTENANCE REMOVED AND REPLACED THE CENTER OMEGA BEAM, TOP CAP AND RAMP ANGLE IN THE GALLEY IAW EMB 145XR 51-40-02. ALL WORK WAS ACCOMPLISHED AND A/C WAS RELEASED TO SEVICE AFTER THE 2C WAS COMPLIED WITH.
ON DESCENT, A LARGE RUMBLING TO BE FELT AND HEARD AS POWER WAS REDUCED CONTINUING UNTIL GEAR WAS FULLY EXTENDED. PERFORMED A FLY-BY OF TOWER, AND LANDED WITHOUT INCIDENT. MX INSPECTED AIRCRAFT AND FOUND A LOOSE BONDING JUMPER, AND AIRCRAFT WAS APPROVED FOR A TEST FLIGHT. THE TEST FLIGHT WAS UNSATISFACTORY, AND ANOTHER FLY-BY OF THE TOWER CONFIRMED THE NOSE LANDING GEAR DOORS REMAINED OPEN. THE AIRCRAFT LANDED AT RSW WITHOUT INCIDENT, WHERE THE AIRCRAFT WAS INSPECTED AND APPROVED FOR A FERRY FLIGHT TO IAH. THE AIRCRAFT LANDED AT IAH WITHOUT INCIDENT WHERE MAINTENANCE REMOVED AND REPLACED THE NLG DOOR SOLENOID VALVE, OPERATIONALLY CHECKED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
ON DESCENT, A LARGE RUMBLING FELT AND HEARD AS POWER WAS REDUCED CONTINUING UNTIL GEAR WAS FULLY EXTENDED. PERFORMED A FLY-BY OF THE TOWER, AND LANDED WITHOUT INCIDENT. MX INSPECTED AIRCRAFT AND FOUND A LOOSE BONDING JUMPER, AND AIRCRAFT WAS APPROVED FOR A TEST FLIGHT. TEST FLIGHT WAS UNSATISFACTORY, AND ANOTHER FLY-BY OF THE TOWER CONFIRMED, NOSE LANDING GEAR DOORS REMAINED OPEN. THE AIRCRAFT LANDED AT RSW WITHOUT INCIDENT, WHERE THE AIRCRAFT WAS INSPECTED AND APPROVED FOR A FERRY FLIGHT TO IAH. THE AIRCRAFT LANDED AT IAH WITHOUT INCIDENT WHERE MAINTENANCE REMOVED AND REPLACED THE NLG DOOR SEQUENCING PROX SWITCH, OPERATIONALLY CHECKED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
CLE FLIGHT NR 3239, THE CREW REPORTED THE GEAR HANDLE WOULD NOT MOVE TO THE UP POSITION WITHOUT DEPRESSING THE RELEASE BUTTON ON THE GEAR HANDLE CONTROL BOX. MAINTENANCE INSPECTED AND REPLACED AIR/GROUND RELAY KOO24. OPERATIONAL CHECKS WERE GOOD.
MAINTENANCE FOUND DAMAGE SKIN ON THE LT FORWARD FUSELAGE BETWEEN FR 1 AND FR 3 BETWEEN STR 13L AND 15L. MAINTENANCE REMOVED THE DAMAGE SECTION OF STR 13L AND SPLICED IN A NEW SECTION. RECONTOURED FR 3. REMOVED AND REPLACED THE SKIN ON THE LT FWD FUSELAGE IAW EC 2003-145/2122; SRM 51-40-02;03;06. ALL WORK WAS ACCOMPLISHED AND WAS RELEASED BACK TO SERVICE.
EWR---CO3547, CREW REPORTED, LEFTSIDE EMERGENCY LIGHT INOPERATIVE ON SERVICE DOOR. MAINTENANCE REMOVED AND REPLACED EMERGENCY LIGHT INVERTER IAW EMB145 AMM #33-50-11. OPS.CHECKED GOOD. AIRCRAFT RETURNED TO SERVICE.
