DURING CRUISE AT 6000 FT FROM SJU TO STT THE LEFT FUEL PRESSURE WARNING ILLUMINATED. RAN MEMORY ITEMS AND ENGINE SHUTDOWN CHECKLIST FROM QRH AND DECLARED AN EMERGENCY. LANDED SAFELY AND NO DAMAGE TO AIRCRAFT. SUBSEQUENT MAINTENANCE TROUBLESHOOTING DETERMINED THE LEFT FORWARD TANK FUEL INDICATION AMPLIFIER HAD FAILED. MAINTENANCE REPLACED THE AMPLIFIER AND RETURNED THE AIRCRAFT TO SERVICE.
DURING CRUISE THE CREW NOTICED A STRONG SMOKE SMELL IN THE COCKPIT FROM THE LEFT ENGINE HYDRAULIC INDICATOR. CREW CLOSED VALVE AND FOLLOWED QRH, PROCEEDED TO DESTINATION AND LANDED WITHOUT INCIDENT. MAINTENANCE REPLACED FAULTY LEFT-HAND HYDRAULIC MAGNETIC INDICATOR, OPERATIONAL CHECK GOOD.
AFTER TAKEOFF, UPON CLIMB OUT THROUGH 4200 FT IT WAS NOTICED THAT LT FIRE FAULT LIGHT ON CWP ILLUMINATED. CREW CONSULTED QRH & RESET CB WHICH HAD POPPED. IT WAS THEN NOTICED THAT BOTH LT ENG FIRE EXTINGUISHER INDICATORS INDICATED DISCHARGED CONDITION, WHITE. CB WAS ATTEMPTED TO RESET AGAIN 3 TIMES IAW QRH, AT WHICH TIME CREW DECIDED TO RETURN TO BASE FOR SAFETY REASONS. UPON MX INVESTIGATION IT WAS DETERMINED THAT LT ENG INBD BOTTLE, CONTENTS HAVE BEEN EXHAUSTED. BOTTLE WAS REPLACED WITH SERVICEABLE BOTTLE. SYS OPERATIONAL & FUNCTION CHECK WERE COMPLETED SATISFACTORILY.
ON ROTATION FIRST OFFICER HAD TO USE FULL LT RUDDER PEDAL & WAS UNABLE TO MAKE RUDDER TRIM ADJUSTMENTS. LT ENGINE POWER WAS RETARDED TO COMPENSATE FOR LOSS OF TRIM CONTROL. ACFT RETURNED TO DEPARTURE FIELD WITHOUT ANY FURTHER INCIDENCE. FOUND RUDDER TRIM CABLE & CHAIN ASSEMBLY ON ACFT LT SIDE RUDDER BROKEN. APPEARS THAT CABLE BROKE APPROX 10 INCHES FROM CHAIN END OF ASSEMBLY IN A HARD TO REACH LOW VISIBILITY AREA. AFFECTED PART WAS REPLACED IAW MAINTENANCE INSTRUCTIONS & FUNCTIONAL CHECKS WERE SATISFACTORILY ACCOMPLISHED.
DURING CRUISE FLIGHT, CREW EXPERIENCED A DROP IN TORQUE ON RT ENGINE, FOLLOWED BY FLUCTUATING TORQUE AND RISING ITT. SHUTDOWN ENGINE, DECLARED EMERGENCY, AND PERFORMED SUCCESSFUL SINGLE ENGINE APPROACH AND LANDING.
RIGHT MLG UNLOCKED INDICATION ON APPROACH. DECLARED EMERGENCY AND PERFORMED FLY-BY TO VERIFY GEAR DOWN. LANDED SAFELY. DETERMINED TO BE AN INDICATION PROBLEM. GEAR WAS DEFERRED PER MEL AND PINS INSTALLED TO LOCK GEAR DOWN. RIGHT DOWNLOCK MICROSWITCH WAS DETERMINED TO BE AT FAULT AND WAS ADJUSTED TO CLEAR THE MEL.
RT MLG UNLOCKED INDICATION ON APPROACH. DECLARED EMERGENCY AND PERFORMED FLY-BY TO VERIFY GEAR DOWN. LANDED SAFELY. DETERMINED TO BE INDICATION PROBLEM. GEAR DEFERRED IAW MEL AND PINS INSTALLED TO LOCK GEAR.
WHILE ON THE RETURN FLIGHT, CREW EXPERIENCED RT ENGINE ABNORMALITIES. THE TORQUE INDICATION EXCEEDED THE RED LINE LIMITATION AND BECAME UNCONTROLLABLE. AT THE SAME TIME, A DRASTIC DECLINE IN PROP RPM (BELOW 900 RPM) WAS NOTED. BOTH INDICATIONS WERE NOT ADJUSTABLE TO A STABLE CONDITION, THUS THE NECESSITY TO EXECUTE PRECAUTIONARY RT ENGINE SHUTDOWN. AN EMERGENCY WAS ALSO DECLARED. THE AIRCRAFT LANDED WITHOUT INCIDENT. MECHANIC REPLACED PROP GOVERNOR WITH A SERVICEABLE ONE IAW MM. OPS CHECK GOOD AND THE AIRCRAFT WAS RETURNED TO SERVICE. (K)
RETURNED AFTER TAKEOFF DUE TO LOW PROP RPM ON LT ENGINE. MECHANIC DETERMINED A BAD PROP GAUGE. REMOVED AND REPLACED WITH SERVICEABLE UNIT. OPS CHECK GOOD.
HEARD AN ENGINE NOISE, SOUND CHANGE FROM RT ENGINE & CKD STATUS OF ENGINE & PRIMARY INSTURMENTS. RT ENGINE TORQUE GOING PAST REDLINE & INCREASING. ITT INCREASED SLIGHTLY THEN DROPPED TO 450 & DECREASING. CAPTAIN DECREASED RT ENGINE TORQUE 200 NR'S AT FIRST & TORQUE CONTINUED TO INCREASE EVEN AFTER TORQUE REDUCTION OF 600 NR WHILE ITT STILL DECREASING. NG & PROP RPM STABLE & FUEL FLOW & OIL TEMPS NORMAL. PERFORMED VITAL ACTIONS ENGINE CHECKLIST & PERFORMED SUBSEQUENT ACTIONS CHECKLIST. A/C LANDING WITH 1 ENGINE SHUTDOWN, BUT DIDN'T DECLARE AN EMERGENCY. EMERGENCY VEHICLES ON STANDBY. A/C LANDED WITHOUT INCIDENT. MECHANIC R & R PROP GOVERNOR IAW MM. OPS CK GOOD & A/C RETURNED TO SERVICE. (M)
LT ENGINE STARTED TO SHAKE AFTER START UP. THE CREW SHUTDOWN THE ENGINE AND EXITED THE AIRCRAFT. MAINTENANCE INSPECTED THE LT ENGINE AND FOUND 2 BROKEN PT BLADES WHICH CAUSED BINDING OF THE POWER SECTION. THE POWER SECTION IS BEING REMOVED AND REPLACED.
ON FLT 42310, ON THE DESCENT INTO CWA THE FIRST OFFICER PULLED THE POWER BACK AND THE LEFT ENGINE WOULD NOT GO BELOW 2300 LBS OF TORQUE. THE CREW DECIDED TO PRECAUTIONARY SHUTDOWN THE ENGINE ON THE APPROACH INTO CWA. THE CWA ATC WAS NOTIFIED AND AN EMERGENCY WAS NOT DECLARED. THE EMERGENCY VEHICLES WERE DISPATCHED ANYWAY. THE AIRCRAFT WAS LANDED WITHOUT INCIDENT AND WAS TAXIED TO THE RAMP. THE MECHANICS WERE DISPATCHED FROM MKE TO CWA. THE LEFT ENGINE WAS INSPECTED AND FOUND THE CAMBOX TO BE DIRTY. THE CAMBOX WAS CLEANED AND LUBRICATED. OPS CHECK GOOD AND THE AIRCRAFT WAS RETURNED BACK TO SERVICE. (M)
