CLIMBING THRU FL180, IN ICING CONDITIONS, THE ANTI ICE CAME ON THEN THE NR 2 OIL TEMP WENT TO 147C, BROUGHT THRUST LEVER BACK TO IDLE AND TEMP WENT TO 156 C. STARTED DECENT BACK TO DEPARTURE, LEFT ICING CONDITIONS. THE ANTI TURNED OFF AND THE RIGHT OIL TEMP WENT BACK TO NORMAL. DECLARED AN EMERGENCY AND LANDED WITHOUT FURTHER EVENT. MAINTENANCE PERFORMED FIM 79-37-00-810-806, AND PERFORMED ENGINE RUNS IAW AMM 72-00-00, NO FAULTS FOUND. ACFT OK FOR SERVICE. AIRCRAFT WAS THEN TAKEN UP ON A FUNCTIONAL CHECK FLIGHT TO FURTHER VERIFY OPERATION. NO DEFECTS WERE NOTED. AIRCRAFT WAS RETURNED TO SERVICE. AIRCRAFT HAS FLOWN 11 FLIGHTS SINCE THE EVENT WITH NO FURTHER OIL TEMPERATURE ISSUES.
PILOT REPORTED A STRONG FUEL ODOR AFTER LANDING. MX PERFORMED INSPECTION OF BOTH ENGINES AND APU WITH NO LEAKS FOUND. ENGINE GROUND RUNS PERFORMED AND NO FUEL ODOR NOTED. ACFT WAS APPROVED FOR RETURN TO SERVICE.
ACFT WAS FERRIED HERE, BECAUSE OF A SMOKING ENGINE AND ODORS IN CABIN. INSPECTION BOROSCOPE THE ENGINE AND FOUND OIL BUILDING UP BEHIND THE IGV'S. MAINTENANCE R & R THE ENGINE AND OPS CHECK; LEAKS CHECKS WERE GOOD AND WILL BE RELEASED AT THE END OF THESE CHECKS.
FLIGHT 4263, SPOILER FAIL INFLIGHT AT FL370, SPOILERS OPEN INDICATION DISPLAYED ON EICAS. MAINTENANCE FOUND DAMAGED HARNESS AND PINS AT CONNECTOR P1166 FOR THE RT SPEED BRAKE PROXIMITY SWITCH. REMOVED DAMAGE SECTION AND REPAIRED IAW EMB-145 SWPM 20-21-00. R & R CONNECTOR PLUG, P1166 IAW SWPM 20-50-01. SPOILER SYSTEM OPS CHECK GOOD IAW AMM 27-63-01. OPS CHECK GOOD. NO OTHER DEFECTS NOTED AT THIS TIME. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
THE CREW REPORTED YAW TRIM SELF ACTUATES BOTH RT AND LT AT RANDOM. MAINTENANCE REPLACED YAW TRIM ACTUATOR. OPS CHECKS WERE GOOD.
THE CREW REPORTED A STRONG BURNING ELECTRICAL ODOR IN THE VICINITY OF ROW 22. THE ACFT LANDED WITHOUT INCIDENT WHERE MAINTENANCE R & R A SHORTED LIGHT BULB AT ROW 22, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK, THE HORIZONTAL STABILIZER ACTUATOR UPPER MOUNT BRACKET WAS FOUND TO BE WORN AND WOULD NOT RETAIN THE HAT BUSHINGS. MAINTENANCE R & R THE BRACKET IAW SRM 51-40-02. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK, THE SERVICE DOOR CENTER SILL WAS FOUND TO BE CORRODED. MAINTENANCE R & R THE SILL IAW SRM 51-40-02. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK, THE LT AND RT HORIZONTAL STABILIZER HINGE PIN FITTINGS WERE FOUND TO BE WORN AND WOULD NOT RETAIN THE BUSHINGS. MAINTENANCE PERFORMED A REAMING REPAIR TO THE FIRST OVERSIZE IAW SRM 55-10-02. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK, THE REAR E/E BAY DOOR INNER SKIN WAS FOUND TO BE CRACKED. MAINTENANCE PERFORMED A DOUBLER REPAIR IAW EC5240-01082. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING MX INSPECTION, AFT CABIN FLASHLIGHT FOUND INOPERATIVE. MX R & R FLASHLIGHT BATTERY. OPS CHECKED GOOD. ACFT APPROVED FOR RETURN TO SERVICE.
DURING MX INSPECTION THE LT AND RT OVERWING EMERGENCY LIGHTS WERE FOUND INOPERATIVE. MX R & R NR 4 EMERGENCY LIGHT BATTERY CHARGER WITH OPERATIONAL CHECKS SUCCESSFUL. ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING PREFLIGHT INSPECTION, THE CREW REPORTED THAT THE FIRST AISLE PATH EMERGENCY LIGHT WOULD NOT ILLUMINATE. MX R & R THE NR 1 EMERGENCY LIGHT INVERTER OPS TESTED WITH NO DEFECTS, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK, CRACK AROUND RIVET FRAME 75, STRINGER 2L-3L VERIFIED WITH EDDY CURRENT. MX PERFORMED REPAIR BY INSTALLING FABRICATED REPAIR DOUBLER AND FILLER AT FRAME 75 BETWEEN STRINGER 2L-3L IAW EC5330-02313. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK CENTER OMEGA BEAM AT FRAME 44 LOWER CROSSBEAM ATTACHMENT POINT HAS CORROSION (LOCAL LEVEL 2). MX PERFORMED AND COMPLETED REPAIR OF CENTER OMEGA BEAM AT FRAME 44 LOWER CROSSBEAM ATTACHMENT POINT IAW EC5310-01466. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.

DURING C-CHECK CENTER OMEGA BEAM FRAME 59 HAS CORROSION (LOCAL LEVEL 2) ON SIDEWALL UNDERNEATH FASTENER AT CROSSBEAM ATTACHMENT POINT. MX PERFORMED REPAIR BY INSTALLING FABRICATED REPAIR DOUBLER AT CENTER OMEGA BEAM AT FRAME 59 CROSSBEAM ATTACHMENT IAW SRM 53-00-10. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK, RT MLG AFT UPPER TRUNNION OTBD MATING SURFACE CRACKED. MX COMPLETED REPAIR OF AFT UPPER MLG TRUNNION IAW EC5750-01483. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
FLIGHT 6124, CREW RPORTED THE GALLEY EMERGENCY LIGHT TO BE INOPERATIVE. MX R & R THE AFFECTED LIGHT INVERTER, OPS TESTED WITH NO DEFECTS, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
THE CREW REPORTED THE F/A'S EMERGENCY CALL SWITCH LIGHT WILL NOT EXTINGUISH. MX REPLACED THE FORWARD F/A'S HANDSET. OPS CHECKED GOOD.
IAH - DURING MAINTENANCE INSPECTION THE EMERGENCY EXIT LIGHTS AT SIDE DOORS WERE FOUND TO BE INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE NUMBER THREE EMERGENCY LIGHT BATTERY PACK, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
CLE FLT 2866 THE CREW REPORTED A COMM 1 STUCK MIC ON TRANSMIT. THE AIRCRAFT RETURNED TO CLE AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED AND REPLACED WIRE W101-3092-24 AND W101-3094-24 AND WIRE SPLICE 2020. OPERATIONAL CHECKS WERE GOOD.
CLE FLT 2864 THE CREW REPORTED DURING CRUISE (FL200) THE CABIN ALTITUDE WAS REACHING 8300FT. MAINTENANCE INSPECTED AND REPLACED THE CABIN PRESSURE ACQUISITION UNIT. OPERATIONAL CHECKS WERE GOOD.
IAH - FLIGHT 2435, THE CREW REPORTED THAT THE BAGGAGE SMOKE DETECTOR FAILED ON TEST. MAINTENANCE INSPECTED THE AIRCRAFT AND REMOVED AND REPLACED THE BAGGAGE COMPARTMENT FORWARD SMOKE DETECTOR, OPERATIONALLY TESTED WITH NO DEFECTS. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.

MAINTENANCE REMOVED AND REPLACED THE NUMBER 3 & 17 THERMOCOUPLES, AND REMOVED AND REPLACED THE COMPRESSOR ACCELERATION CONTROL VALVE, OPERATIONALLY AND LEAK CHECKED WITH NO DEFECTS NOTED, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.

FLIGHT 2710 - EWR - FLIGHT CREW REPORTED THAT DURING CLIMBING TAT NR 1 FAIL MESSAGE IN EICAS. CREW PERFORMED AIR RETURN AND LANDED ACFT UNEVENTFULLY. EWR MX REMOVED AND REPLACED TAT NR 1 AND PERFORMED OPERATIONAL CHECK SUCCESSFULLY. ACFT WAS APPROVED TO RETURN TO SERVICE.

IAH - DURING MAINTENANCE INSPECTION THE NUMBER THREE EMERGENCY LIGHT BATTERY PACK WAS FOUND TO NOT HOLD A CHARGE. MAINTENANCE REMOVED AND REPLACED THE AFFECTED BATTERY PACK, OPERATIONALLY TESTED WITH NO DEFECTS. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.

DURING ROUTINE MX, MX FOUND RT EMERGENCY EXIT MARKER INOPERATIVE. MX REMOVED AND REPLACED NR 5 EMERGENCY INVERTER AND PERFORMED OPS CHECK SUCCESSFULLY. ACFT WAS APPROVED TO RETURN TO SERVICE.
FLIGHT 3009 - EWR - FLIGHT CREW REPORTED NR 1 ENG ANTI-ICE OPEN INDICATION INOPERATIVE IN ICE CONDITION. EWR MX REMOVED AND REPLACED NR 1 ENGINE ANTI-ICE VALVE AND PERFORMED OPERATIONAL CHECK SUCCESSFULLY. AIRCRAFT WAS APPROVED TO RETURN TO SERVICE.
IAH - FLIGHT 2312, THE CREW REPORTED THAT THE NUMBER TWO MAIN BATTERY WAS OVERHEATING IN FLIGHT. THE CREW DEACTIVATED THE NUMBER TWO BATTERY, AND THE TEMPERATURE REMAINED CONSTANT UNTIL DESCENT. THE AIRCRAFT LANDED AT IAH WITHOUT INCIDENT WHERE MAINTENANCE REMOVED AND REPLACED THE NUMBER TWO MAIN BATTERY, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
CLE FLIGHT 2150 - THE CREW REPORTED THE AIRCRAFT WOULD NOT PRESSURIZE ON CLIMBOUT. THE AIRCRAFT RETURNED TO CLE AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED AND REPLACED THE DIGITAL CONTROLLER. OPERATIONAL CHECKS WERE GOOD.
EWR - FLIGHT 2400 - THE CREW REPORTED A ENGINE 1/2 EXTBTL A INOP MESSAGE ON THE EICAS INFLIGHT. THE AIRCRAFT DIVERTED INTO CLE AND LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE INSPECTED AND REPLACED THE ENGINE NR 1 FIRE BOTTLE A. OPERATIONAL CHECKS WERE GOOD.
FLT 3077 - EWR - FLT CREW REPORTED EMERGENCY LIGHTS INOPERATIVE. EWR MX RESET EMERGENCY LIGHT SYSTEM AND PERFORMED OPS CHECK SUCCESSFULLY. AC WAS RETURNED TO SERVICE.
CLE - FLIGHT 2552 THE CREW REPORTED, UNABLE TO OPEN THE MAIN CABIN DOOR FROM INSIDE. MAINTENANCE INSPECTED AND REPLACED THE FORWARD BUSHINGS ON THE LINK SUPPORT OF THE MAIN CABIN DOOR. THE MAIN DOOR CABLES WERE ALSO ADJUSTED AND OPERATIONAL CHECKS WERE GOOD. (S)

SHV - DURING C-CHECK RT AILERON OUTBOARD CONTROL ROD ATTACHING BRACKET ON AILERON, HAS MIGRATED HAT BUSHING. MX REMOVED AND REPLACED RT AILERON OUTBOARD CONTROL ROD ATTACHING BRACKET IAW EC5750-01177. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE. (A)

TYS - DURING A OVERNIGHT A-CHECK THE AFT CEILING EMERGENCY LIGHT WAS FOUND INOP. MAINTENANCE REMOVED AND REPLACED THE EMERGENCY LAMP IAW AMM 33-50-08. OPS CHECKS GOOD.
MKE FLIGHT 2266, THE CREW REPORTED THE NR 2 HIGH STAGE BLEED VALVE C/B POPPED IN FLIGHT AND NOTICED A SMOKE SMELL IN THE CABIN. MAINTENANCE INSPECTED AND REPLACED THE NR 2 HIGH STAGE BLEED VALVE. ALSO REPLACED THE NR 2 HIGH STAGE VALVE PRESSURE SWITCH. OPERATIONAL CHECKS WERE GOOD.
IAH - FLIGHT 2653, THE CREW REPORTED AN UNCOMMANED ENGINE 2 SHUTDOWN WITH FADEC 2A &2B CIRCUIT BREAKERS POPPING. THE AIRCRAFT RETURNED TO IAH WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED THE AIRCRAFT AND REMOVED AND REPLACED THE NR 2 ENGINE PMA STATOR, AND THE 2A & 2B FADECS AS A PRECAUTION. OPERATIONAL CHECKS PERFORMED WITHOUT DEFECT, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH - FLIGHT 2287, THE CREW REPORTED THAT THE MAIN CABIN DOOR WAS GETTING JAMMED ON THE TOP AND BOTTOM. MAINTENANCE INSPECTED AND RIGGED THE DOOR, OPERATIONAL CHECKS WERE PERFORMED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
CLE FLIGHT 3203 THE CREW REPORTED NR 1 ENGINE SURGING ON FADEC 1B. MAINTENANCE INSPECTED AND REPLACED FADEC 1B. OPERATIONAL CHECKS WERE GOOD.
FLIGHT 2334 EWR TO SDF, CREW REPORTED HYDRAULIC SYSTEM NR 1 FAILED IN FLIGHT. FLIGHT DIVERTED TO RIC. RIC MX FOUND THE CAUSE OF FLUID LOSS TO BE AT THE NR 1 HYD MANIFOLD, EDHP PRESSURE SWITCH. MX REMOVED AND REPLACED THE PRESSURE SWITCH O-RING AND DUE TO THE SYSTEM BEING RUN OUT OF FLUID ALSO REPLACED NR 1 EDHP AND NR 1 EMDP ALONG WITH DECONTAMINATING THE FLUID SYSTEM AND CHANGING NR 1 SYSTEM HYD FLUID FILTERS.
IAH - FLIGHT 2989, THE CREW REPORTED THAT THE NLG DOORS WOULD NOT CLOSE. THE AIRCRAFT LANDED AT IAH WITHOUT INCIDENT WHERE MAINTENANCE REMOVED AND REPLACED THE NLG DOOR SEQUENCE PROXIMITY SWITCH, OPERATIONALLY CHECKED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.

IAD - FLIGHT 3293 CREW REPORTED LAV SMOKE WARNING IN FLIGHT, NO SMOKE VISIBLE. CREW FOLLOWED QRH PROCEDURE. IAD MAINTENANCE CLEANED SMOKE DETECTOR SENSOR AND PERFORMED LAV SMOKE FIM TASK 26-14-00. ALL OPERATIONAL CHECKS WERE SATISFACTORY. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
DURING INSPECTION FOUND CENTER AISLE EMERGENCY LIGHT AT ROW 12 INOPERATIVE. REMOVED THE CENTER AISLE EMERGENCY LIGHT ASSY. OPS CHECK OK.
SHV-DURING C-CHECK THE CENTER FLOORBEAM WAS FOUND TO BE CORRODED OUT OF LIMITS IAW SRM 53-21-10. MAINTENANCE REMOVED AND REPLACED THE AFFECTED BEAM IAW SRM 51-40-02. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
NON-ROUTINE WORK CARD WRITE UP, NR 2 OVERWING EMERGENCY LIGHT INOP. REMOVED AND REPLACED NR 2 OVERWING EMERGENCY LIGHT IAW EMB 145 MM 33-50-07. OPS CHECK GOOD.
THE CREW REPORTED A NUMBER TWO BATTERY OVERTEMP IN FLIGHT. THE AIRCRAFT DIVERTED TO EFD WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED BOTH AIRCRAFT BATTERIES, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
DURING ROUTINE MAINTENANCE THE EMERGENCY LIGHT AT ROW 12B/C WAS INOPERATIVE. MAINTENANCE REPLACED THE EMERGENCY LIGHT BULB AT ROW 12 B/C. OPERATIONAL CHECKS WERE GOOD.
CLE FLIGHT 2058 THE CREW REPORTED DURING PREFLIGHT THE EMERGENCY EXIT LIGHT WAS INOPERATIVE AT ROW 12. MAINTENANCE REPLACED THE NR 6 INVERTER. OPERATIONAL CHECKS WERE GOOD.
DURING ROUTINE MAINTENANCE NR 2 AND NR 3 EMERGENCY AISLE PATH LIGHTS WERE FOUND INOPERATIVE. MAINTENANCE REPLACED THE EMERGENCY AISLE PATH LIGHT STRIP. OPERATIONAL CHECKS WERE GOOD.
EWR---CO2196, CREW REPORTED, EMERGENCY EXIT LIGHT STRIP INOPERATIVE.....MAINTENANCE REMOVED AND REPLACED FUSES IN GALLEY EMERGENCY LIGHT BATTERY IAW EMB145 CMM #25-60-03 AND 33-50-00, OPS. CHECKED EMERGENCY LIGHT STRIP GOOD.
TYS, During a scheduled inspection the first aisle emergency light was found inoperative. Maintenance removed and replaced the bulb. operiational checked good, aircraft released to service.
CLE FLIGHT#2316 THE CREW REPORTED AFTER TAKEOFF A BLEED LEAK INDICATION OCCURED.THE CREW TURNED OFF THE #2ENGINE BLEED AND THE MESSAGE CLEARED.SHORTLY AFTER THE MESSAGE RETURNED AND THE CREW SHUTDOWN THE #2 ENGINE.THE AIRCRAFT LANDED WITHOUT FURTHER INCIDENT.MAINTENANCE INSPECTED AND REPLACED THE BLEED AIR THERMAL SWITCH SO220.OPERATIONAL CHECKS WERE GOOD.
SHV- DURING C CHECK BUTT STRAP BETWEEN FRAMES 15 AND 16 CENTERLINE WAS FOUND TO HAVE CORROSION. MAINTENANCE REMOVED DAMAGE AND PEFORMED A SPLICE REPAIR IAW EMB 145 SRM 53-21-10. THE AIRCRAFT WAS APPROVED TO RETURN TO SERVICE.
DURING C CHECK, A DENT WAS FOUND JUST AFT OF FRAME 12A BY STRINGER 18 LT. MAITENTANCE REPAIRED DENT IAW CONTINENTAL EXPRESS EC 5330-01206. THE AIRCRAFT WAS APPROVED TO RETURN TO SERVICE.
IAH- FLIGHT 2908 NR 1 BLEED OVERTEMP WHILE CLIMBING THROUGH FL200 WHEN ENTERING ICING CONDITIONS QRH COMPLETED. MAINTENANCE REMOVED AND REPLACED NR 1 FAN AIR BYPASS VALVE, PERFORMED A OPERATIONAL CHECK, NO DEFECTS NOTED, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
GSP---CO4088, CREW REPORTED LEFT HAND COFFEE RECEPTACLE GIVES OFF BURN SMELL. MAINTENANCE PLACED BOTH COFFEEMAKERS ON MEL AND RETURNED THE AIRCRAFT TO SERVICE.
IAH-DURING A-CHECK, THE MECHANIC NOTED THE AISLE PATH LIGHTING AT SEAT 9A WAS INOPERATIVE. IAH-MAINTENANCE REMOVED AND REPLACED THE AISLE EMERGENCY LIGHT AT SEAT 9A AND THE OPERATIONAL CHECK OF THE EMERGENCY LIGHTING SYSTEM WAS SATISFACTORY. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
