MAST POLE MFG INCORRECTLY. SCALLOP FOR DRIVE LINK THROUGH BOLT MACHINED IN WRONG LOCATION.
ON TAKEOFF, RPM ROSE TO 104 PERCENT, THE PILOT WAS UNABLE TO BEEP DOWN TO NORMAL RANGE, RETURNED TO DEPARTURE. THE MECHANIC FOUND THE LINEAR ACTUATOR RANGE WAS SET TOO HIGH, HE ADJUSTED THE BEEP RANGE TO WITHIN THE LIMITS AND THE AC WAS RETURNED TO SERVICE. (K)
TRANSMISSION ASSY MAKING METAL, DISASSEMBLED AND FOUND DUPLEX BEARING BREAKING DOWN. REMOVED AND REPLACED DUPLEX BEARING.
TRANSMISSION CHIP LIGHT ILLUMINATED IN HOVER. MAINTENANCE REMOVED TRANSMISSION, MAST, AND OIL COOLER. A SERVICEABLE TRANSMISSION, MAST AND OIL COOLER WERE INSTALLED. FLUSHED TRANSMISSION OIL SYSTEM. IAW MM. GROUND AND LEAK CHECK, OK. (K)
LOWER SURFACE OB GRIP FINGER DOUBLER DEBONDED. FOUND CORROSION BETWEEN SKIN AND SPAR. REPLACED UNIT W/NEW BLADE.
MAIN ROTOR BLADE TRAILING EDGE EXTRUSION EXFOLIATED AT STA. 98 AND 130. DAMAGE IS CONSIDERED A MATERIAL DEFECT.
DURING ROUTINE INSPECTION, FOUND T/R GEARBOX FAIRING HINGE WORN, FILLER DOOR MISSING. THE FILLER DOOR HAD CONTACTED ONE TAIL ROTOR BLADE. THIS BLADE WAS FOUND UNSERVICEABLE. TAIL ROTOR HUB BLADE ASSY WAS REPLACED. AIRCRAFT WAS VISUALLY INSPECTED AND RETURNED TO SERVICE.

DURING DAILY INSPECTION, FOUND THE PITCH LINK STUD ON THE PITCH HORN WAS LOOSE. SUSPECT CAUSE IS THE STUD WAS NOT PERMANENTLY SECURED AT THE FACTORY. ALSO, THE PITCH HORN WAS NOT PROPERLY TORQUED AT THE FACTORY CAUSING THE PITCH HORN TO BECOME LOOSE ON THE TAIL ROTOR BLADE.
