FLT 1496 - CLT - ELECTRICAL SMELL - OVEN IN FORWARD GALLEY GAVE OFF AN ELECTRICAL BURNING SMELL AFTER TAKEOFF. TURNED OVEN OFF AND ODOR DISSIPATED. MAINTENANCE REPLACED FORWARD GALLEY OVEN.
THE FLIGHT ABORTED TAKEOFF DUE TO NR 2 ENGINE FAILURE. CREW REPORTED FLUCTUATION AT 80 PERCENT N1, THEN DROPPED OFF POWER. MAINTENANCE FOUND THE J8 HARNESS SWELLED DUE TO AN EARLIER LEAK ON THE HMU. REPLACED THE J8 HARNESS.
FLT 789 LGA-CLT - RETURN TO FIELD - LGA ATC TOWER REPORTED BLACK SMOKE FROM NR 2 ENGINE ON TAKEOFF. CREW HAD NO COCKPIT INDICATIONS OF ANY PROBLEM. NO EMERGENCY WAS DECLARED. AIRCRAFT RETURNED TO FIELD. MAINTENANCE FOUND LEAKING GASKET AND REPLACED PER AIRBUS MAINTENANCE MANUAL 73-21-10-000-002.
FLT 904 CLT-ORD - RETURN TO FIELD - APPROXIMATELY 10 MINUTES INTO FLIGHT, FUMES WERE REPORTED IN THE REAR OF THE AIRCRAFT. THE CREW BEGAN A RETURN TO CLT AND WHEN AIRCRAFT STOPPED CLIMBING, THE FUMES DISSIPATED. IT WAS DECIDED TO CONTINUE TO ORD, BUT WHEN AIRCRAFT BEGAN TO CLIMB AGAIN, THE FUMES RETURNED. THE CREW DECLARED AN EMERGENCY AND RETURNED TO CLT. THE AIRCRAFT LANDED AND TAXIED TO THE GATE WITHOUT INCIDENT. MAINTENANCE TROUBLESHOT PER THE TSM (TROUBLESHOOTING MANUAL) 71-00-00-810-802 WITH NO FAULTS NOTED. RAN BOTH ENGINES AND APU BLEEDS AND PACKS TO TAKEOFF POWER. NO SMOKE OR SMELL NOTED.
FRAME 23, RIGHT OUTBOARD SEAT TRACK -Y760 HAS CORROSION. SPLICED IN NEW SECTION IAW SRM 53-00-15 FIG 201, DEFECT SIZE: 1 INCH DIA.
FLOORBEAM Y254 HAS CORROSION. REMOVED & REPLACED BEAM PER SRM 51-72-11; CORR TASK JA-220-53500, LEVEL 1, DEFECT SIZE: 2.5 X 3.5 INCHES.
CORRODED RIVET TAILS ON FUSELAGE FORWARD CARGO COMPARTMENT BETWEEN FRM 28-35, STRINGER 38L. APPROXIMATELY 900 RIVETS WERE REPLACED. A RESIDUE OF SODIUM CHLORIDE ATTACKED THE RIVETS. NUMEROUS OTHER RIVETS HAD PAINT FLAKING OFF THE RIVET TAILS THAT WERE CLEANED AND HAD PRIMER AND PAINT REAPPLIED. AC MFG 6/20/2001. (K)
DTW FLT 902 EN ROUTE CLT TO DTW, DURING DESCENT, BELOW FL 290, ENGINE 1 BEGAN TO RUMBLE AND VIBRATE. THE AUTOTHROTTLE WAS DISCONNECTED AND THE NUMBER 1 ENGINE WAS BROUGHT BACK IDLE AND THE VIBRATIONS STOPPED. AT 7,000 FEET AND 210 KNOTS, THE AUTOTHROTTLES WERE RE-ENGAGED AND ALL OPERATIONS WERE NORMAL. AFTER LANDING AND ARRIVAL AT THE GATE, THE GROUND CREW NOTICED A TRAIL OF FUEL THAT FOLLOWED THE NUMBER ENGINE, ENDING IN A PUDDLE UNDERNEATH IT. ALSO, A 1,000 POUND DIFFERENCE IN FUEL BURN BETWEEN THE LEFT AND RIGHT FUEL TANKS WAS NOTED. MAINTENANCE FOUND THE NUMBER 1 ENGINE HMU LEAKING. THE HMU WAS CHANGED WITH THE ENGINE. THE ENGINE WAS CHANGED DUE TO OTHER PERFORMANCE ISSUES NOT RELATED TO THE FUEL LEA
EMERGENCY EXIT LIGHTS WILL NOT TEST PROPERLY. MAINTENANCE RESET THE LEFT AND RIGHT EPSU'S. NO FURTHER DIFFICULTIES.
DURING EMERGENCY LIGHT TEST, GREEN LIGHT WOULD NOT ILLUMINATE ON PROGRAMMING AND TEST PANEL. MAINTENANCE REPLACED THE LEFT HAND, AFT EMERGENCY POWER SUPPLY UNIT. OPERATIONALLY CHECKS OK.
MCI - FLT 201 - ENROUTE PHL - SFO, DURING CRUISE, ECAM MESSAGE F/CTL ELAC 2 PITCH FAULT GENERATED, FOLLOWED BY AN A/P 2 DISC MESSAGE. AFTERWARDS, CREW FURTHER RECEIVED ECAM MESSAGES F/CTL STABILIZER JAM AND F/CTL ALTN LAW. DURING THIS TIME, STATUS ITEMS THAT BECAME INOPERATIVE WERE: F/CTL ROT, STAB, ELAC PITCH, AND A/P 1 AND 2. AT THIS TIME, THE FLIGHT CREW REPORTED DIFFICULTY IN CONTROLLING PITCH. MANUAL TRIM WAS USED TO CONTROL PITCH. FLIGHT CREW OBSERVED THAT CONTROLLABILTY IMPROVED AS THE AIRCRAFT DESCENDED. MAINTENANCE REPLACED ELAC 2. OPERATIONALLY CHECKS OK.
PHL - DURING INSPECTION, THE R3 EMERGENCY EXIT DOOR FAILED THE BELLCRANK DISTANCE CHECK WHEN THE BELLCRANK FAILED TO MOVE. WITHOUT BELLCRANK MOVEMENT, THE SLIDE WOULD NOT HAVE DEPLOYED. MAINTENANCE FOUND THE OVER-RIDE CLUTCH TRIPPED. THE CLUTCH WAS RESET AND THE BELLCRANK DISTANCE CHECK PASSED THE OPERATIONAL TEST. (M)
