SLIDE PRESSURE DOOR 4R NO TEST. *S/D* THE REASON FOR SLIDE PRESSURE FAILURE AT DOOR 4 WAS INDICATION AND NOT SLIDE PRESSURE. THIS CONDITION WAS CORRECTED BY REPLACING THE TEST INDICATOR BATTERY PACK.
ON TAKEOFF (90 KTS) NR 2 GEN LIGHTS FLASHED AND AC BUS TRIPPED OFF. *S/D* REPLACED NR 2 CSD. OPS AND LEAK CHECKED OK. SHOP FINDINGS - NO DESCREPANCY FOUND; REPLACED HYDRAULIC LOG ASSEMBLY, SEALS AND FILTER ELEMENT, PER UAL SPECIFICATIONS.
FUMES IN COCKPIT AND CABIN USING NR 2 PACK THRU 1-2 PNEU ISOL VALVE. *S/D* NR 2 BLEED WAS DEFERRED INOP DUE TO A PRESSURE INDICATION PROBLEM. THE NR 2 PACK WAS RUN OFF THE NR 1 BLEED VIA THE 1-2 ISOLATION VALVE. THERE IS NO HISTORY OF A FLUID LEAK FROM THE NR 1 ENGINE PRIOR TO THE INCIDENT, ALTHOUGH THERE IS SOME HISTORY OF AN OIL LEAK FROM THE APU. SOURCE OF FUMES MAY BE FROM INGESTION OF OIL BY THE APU, CONTAMINATING THE SYSTEM. THE NR 2 PACK SYSTEM WAS BURNED OUT. THERE HAS BEEN NO RECURRANCE.
NR 1 ENGINE HAD A LOUD NOISE AT T/O. ABOUT 90 PERCENT POWER SUSPECT COMPRESSOR STALL. TOWER REMARKED ON LARGE FLAME EXITING ENGINE. FOUND METAL PIECES IN TAILPIPE AFTER RETURN TO GATE (LARGER THAN FLAKES, SMALLER THAN BB''S). *S/D* INITIAL ON-WING BORESCOPE REVEALED HPC STG 16 BLADE DAMAGE, APPROXIMATELY 80 PERCENT BEYOND LIMITS, CRACKED, BENT, TORN. ENGINE WAS REPLACED. POST ENGINE REMOVAL BORESCOPE OF THE HPC SHOWED IMPACT DAMAGE STARTING AT THE HPC STG 1 AND PROGRESSING DOWNSTREAM. THE 5TH STG BLDS SHOWING THE MOST DAMAGE. IMPACT IMPRESSIONS IN THE BLADES INDICATE FOD, PROBABLY A WASHER OR OTHER ROUND HARDWARE.
STRONG SMELL REPORTED IN REAR CABIN FOR THE FIRST 10 MINUTES FOLLOWING TAKEOFF. DEBRIEF CABIN CREW FOR ADDITIONAL DETAILS. FLIGHT ATTENDANT REPORTED APPROXIMATELY 6-8 ATTENDANTS AND PASSENGER BECAME ILL DURING FLIGHT WITH HEADACHES AND FEELING NAUSEA. *S/D* BURNED OUT ALL PACKS. NO EVIDENCE OF APU LEAKS OR FLUID INGESTION.

SLIDE/RAFT FAILED AIR RETENTION TEST DUE TO SEAM SEPARATION IN TUBE ASSEMBLY. *S/D* SEAM SEPARATION IN INFLATABLE TUBE ASSEMBLY. UNIT IS BEYOND ECONOMICAL REPAIR AND WAS SCRAPPED.
RUDDERS LOCKED ON FINAL. CREW FIRST OPERATED RUDDER BELOW 10,000 FEET. RUDDER FELT STIFF. AFTER TOUCHDOWN RUDDER PEDALS WERE STUCK IN NEUTRAL POSITION. *S/D* STANDING WATER FOUND IN FORWARD DIVERTER COMPARTMENT BELOW NR 2 ENGINE. AT ALTITUDE, WATER FROZE IN VICINITY OF RUDDER CABLE THROUGH SEALS INTO STAB COMPARTMENT PREVENTING NORMAL MOVEMENT. COMPARTMENT DRAIN HOLES FOUND OBSTRUCTED AND CLEARED. REMAINING WATER REMOVED AND PANELS CHECKED FOR PROPER SEALING AND CORRECTED AS REQUIRED.
EMERGENCY FLOOR LIGHTS WERE ILLUMINATED BETWEEN SEATS 17 AND 30. REPLACED BATTERY PACK FOR EEL LIGHTS. LIGHTS CHECKED OKAY.
LEFT AISLE AND FORWARD FLOOR EMERGENCY LIGHTING IN ZONE B REMAINS ON WHILE SWITCH IN COCKPIT WAS SELECTED TO ANY POSITION. REPLACED EEL BATTERY. OP CHECK OF EMERGENCY LIGHTS ARE NORMAL.
EEL STRIP IN RIGHT FORWARD CABIN IS ILLUMINATED WHILE SWITCH IN OFF POSITION. REPLACED THE FORWARD CABIN EEL BATTERY POWER SUPPLY.
EEL LIGHT STRIP AT DOOR 2L TO 3L COMES ON WITH POWER OFF (DISCONNECTED STRIP AT DOOR 2L). REPLACED BATTERY, OP CHECKS OK PER MM 33-50-00.
