PERFORMING PREFLIGHT INSPECTION & SAW A TRANSVERSE CRACK AT THE MAIN ROTOR BLADE FITTING ZONE NR 1 AREA. THIS IS ACCEPTABLE IN ZONE NR 1. AT APPROX THE CTR OF THE BLADE, THERE IS ANOTHER CRACK THAT GOES OUT FROM THIS TRANSVERSE CRACK, TOWARD THE END OF THE BLADE & APPROX 3/8 INCH INTO ZONE NR 2. THERE ARE NO CRACKS ALLOWED IN ZONE NR 2 AREA. SANDED THE FILLER DOWN TO THE FIBERGLASS & A CRACK IS STILL THERE. OUT OF SERVICE AT THIS TIME & ORDERED A REPLACEMENT BLADE.
ON APPROACH, THE TAILROTOR PEDALS KICKED HARD LT AND RT, YAW SAS TRIM ACTUATOR LIGHT ILLUMINATED FOR 3 SECONDS AND THEN EXTINGUISHED AND PEDALS RETURNED TO NORMAL. MX PERSONNEL PERFORMED CHECKS OF THE AUTO PILOT SYS, CLEANED CONNECTIONS AT THE YAW SEMA AND YAW TRIM ACTUATOR AND THE TAIL ROTOR CONTROL SYS. A FLIGHT CHECK WAS PERFORMED WITH 4 TAKEOFFS AND LANDINGS AND ALL SYSTEMS PERFORMED AS DESIGNED AND PROBLEM, COULD NOT BE DUPLICATED AT THIS TIME.
PILOT ON APPROACH TO LAND, MAST MOMENT INDICATOR PEGGED TO THE RT AND CONTINUED TO FLUCTUATE. VEMD SHOW OBVIOUS INDICATOR ERRORS OF EXCEEDANCE AT 64 PERCENT FOR 29 SECONDS 111.1 PERCENT, AT 77 PERCENT FOR 20.6 SECONDS, 111.1 PERCENT AND AT 90 PERCENT FOR 14 SECONDS 111.1 PERCENT. OPEN
PILOT REPORTED AFTER TAKEOFF, GOING THROUGH TRANSITIONAL LIFT, TRIM (YAW) TRIED TO MAKE NOSE YAW RIGHT, PILOT HAD TO EXCESSIVELY FIGHT TRIM. MECHANICS HAVE CHECKED THE TAIL ROTOR CONTROL SYS, INSPECTED AND CLEANED T/R BLADE GRIPS AND PERFORMED AUTOPILOT CHECKS FOR THE TAILROTOR. NO OBVIOUS PROBLEMS FOUND AND WE WILL BE PERFORMING A FLIGHT CHECK WHEN MAST MOMENT EXCEEDANCE DISCREPANCY IS CLEARED.
