DURING A ROUTINE FLUORESENT PENETRANT INSPECTION, (PART OF A SCHEDULED OVERHAUL, DOCUMENTED ON SWO-049059-19) NDT TECH. FOUND ONE CRACK ON THE SYNC SHAFT SUB-ASSEMBLY ADAPTER. DISC-203256-19 HAS BEEN CREATED TO CAPTURE FURTHER MAINTENANCE.
LONGITUDINAL RIGID LINK AFT BOLT ON FORWARD HEAD WAS MAKING WEAR DUST/GREASE FROM THE BOLT HEAD. PROCEEDED TO REMOVE THE BOLT, AND WHEN THE TECHNICIAN STARTED TO LOOSEN THE BOLT, THE HEAD SNAPPED OFF. REMOVED THE REST OF THE REMAINING HARDWARE AND REPLACED WITH NEW HARDWARE.
REMOVED THE NR 1 ENGINE TO CHANGE THE ENGINE MOUNTS. DURING THE OPPORTUNITY INSPECTION FOUND THERE WAS A CRACK ON THE FS 477 BULKHEAD. UPON FURTHER INSPECTION FOUND THERE WAS CHAFING ON THE WEB FROM BL 7R TO BL 7L. REMOVED THE DAMAGE, FABRICATED REPAIR WEB, LOCATED AND INSTALLED REPAIR.
WHILE PERFORMING A "C" INSP THE FIRE SHIELD ON THE NR 4 SYNC SHAFT WAS FOUND TO BE CUT. DURING REPLACEMENT OF THE FIRE SHIELD AT STATION 410 THE CREW FOUND A SCREW HAD RUBBED A 0.25" DEEP HOLE IN THE 410 CAP ANGLE. DAMAGE REPAIRED ON VP-000245-18. DETAILS ON FILE WITH THIS REPAIR STATION
DURING INSPECTION, A CRACK WAS FOUND ORIGINATING AT A 410 BOLT HOLE, AND EXTENDING .5 INCH DOWN FROM THE HOLE. DAMAGED AREA WAS REMOVED, AND THE CRACK WAS VISIBLE FOR APPROX. 2 INCHES ON THE BACK SIDE. DAMAGED SECTION WAS REPLACED WITH REPAIR.
DURING AN INSPECTION AFTER THE TRANSMISSION IS REMOVED, FOUND A CRACK IN THE DOVE TAIL AREA ON A FRAME. REMOVED THE CRACKED FRAME AND FOUND A TWO AND A HALF INCH CRACK ON THE CANTED BULKHEAD AT BS 527, WL +71, LBL 5.
DURING COAST DOWN AT THE END OF DAY, HEARD AN ABNORMAL CLICKING NOISE COMING FROM THE NR 1 HIGH SPEED SHAFT. THE PILOTS REPORTED THE NR 1 NF STAYED WITH THE NR DURING COAST DOWN WHILE NR 2 NF INDICATION SPLIT OFF. ON FURTHER INVESTIGATION, FOUND THE NR 1 CLUTCH FAILED TO DISENGAGE. REMOVED AND REPLACED THE XMSN AND RETURNED THE A/C BACK TO SERVICE. DURING INSPECTION OF IDLER HUBS, FOUND PINION SPURS AND IDLER HUBS WERE WORN AND WERE OUT OF TOLERANCE. CLUTCHES AND ASSOCIATED ARTICLES SCRAPPED DUE TO WEAR.
CRACK FOUND ON CAP ANGLE AT STA 497, WL +44, LT. SUPPLEMENTAL INFORMATION: REPAIRED CRACKED CAP ANGLE PIN 107S7322-15 AT STA. 497, WI +44, BL 26L BY REPLACEMENT OF PREVIOUS REPAIR ANGLE (REF. SL 107-0013) FABRICATED FROM REPLACEMENT P/N 107S7322-15 (P/0-002007-14). ALSO REPLACED FITTING PIN 107S105-1, STA 477, WL+44, LT. A COPY OF THESE REPAIRS IS ON FILE WITH THIS REPAIR STATION AND DOCUMENTED ON VP-000213-18.
DURING ENGINE REPLACEMENT, CRACK WAS FOUND IN AFT ENGINE SUPPORT ANGLE, NR 2 ENGINE.
APPROXIMATELY 10 MINUTES AFTER DEPARTURE, THE FLIGHT CREW SMELLED SMOKE. THE SOURCE OF THE SMOKE WAS MOT APPARENT, IT WAS REPORTED BY BOTH PILOTS AND THE CABIN CREWMEMBER. THE PILOTS SUSPECTED ELECTRICAL FIRE AND DETERMINED TO RETURN TO THE POINT OF DEPARTURE AS THE NEAREST SUITABLE LANDING SITE AND SIMULTANEOUSLY COMPLETED THE EMERGENCY PROCEDURE FOR ELECTRICAL FIRE. THE SOURCE OF THE SMOKE WAS THE FRESH AIR VENTS AND IAW THE EMERGENCY ABNORMAL CHECKLIST, THE SYSTEM WAS SWITCHED OFF. THE CABIN IMMEDIATELY BEGAN TO CLEAR OF SMOKE. A LANDING WAS MADE WITHOUT FURTHER INCIDENT. AFTER LANDING MAINTENANCE PERSONNEL BEGAN TROUBLESHOOTING AND DETERMINED THE BLOWER FAN ASSEMBLY WAS FAULTY.AFTER TROUBLESHOOTING FOUND THE HEATER BLOWER TO BE FAULTY, REMOVED AND REPLACED THE HEATER BLOWER AND RETURNED THE ACFT BACK TO SERVICE
DURING TAKEOFF, NR 1 ENGINE FIRE WARNING LIGHT ILLUMINATED WITH NO SECONDARY INDICATIONS. ACFT LANDED IMMEDIATELY BACK AT MAINTENANCE RAMP. THE NR 1 AFT, UPPER, FIRE DETECTOR WAS REPLACED AND DOCUMENTED ON LC-000594-88-16, DISC-000542-88-16. SUPPLEMENTAL INFORMATION; THE DETECTORS IN THIS A/C HAVE BEEN REPLACED WITH P/N 477223 FLAME DETECTORS IAW COLUMBIA HELICOPTERS, INC. SERVICE BULLETIN NUMBER 107-26-S0004. DETAILS ARE ON FILE WITH OPERATOR CHIA823C.
DURING AN INSPECTION MECHANIC REMOVED AFT XMSN LUBE FILTER & NOTICED LARGE METALLIC CHIPS IN THE FILTER HOUSING & ON THE FILTER PACKS. DISASSEMBLED FILTER & FOUND MORE CHIPS. DUE TO A PREVIOUS EXPERIENCE ON N6682D, CREW CHIEF DIRECTED MECHANIC TO REMOVE BLOWER DRIVE SHAFT & REMOVE BLOWER INPUT COVER ON THE XMSN TO DETERMINE WHETHER THE CHIPS CAME FROM THE BLOWER INPUT SEAL ON THE XMSN. IT WAS DETERMINED THE RING INSIDE THE SEAL THAT'S CONNECTED TO METAL TABS TO KEEP THE SEAL TIGHT AROUND THE BLOWER INPUT WAS MISSING. MX NOTIFIED THE DOM AS WELL AS THE LEAD IN THE XMSN SHOP ON HOW TO PROCEED. DECIDED TO SEND A REPLACEMENT AFT XMSN ASSEMBLY TO THE WORK SITE.
INDICATION FOUND AROUND THE BASE OF THE BEARING JOURNAL, CLOSEST TO THE GEAR TEETH THAT ARE DRIVEN BY THE ACCESSORY DRIVE GEAR DURING MPI REQUIRED FOR O/H OF COMPONENT. PART HAS BEEN SENT OUT FOR FURTHER TESTING SINCE INDICATION SEEMS ISOLATED TO THE OUTER SURFACE.
UPON INSPECTION, DISCOVERED CRACKED ANTENNA MOUNT DOUBLER & SKIN.
