CRACKS FOUND AT STA 160, BL 21R AND 24R.
DURING RAMP CHECK, DISCOVERED THAT NR 1 ENGINE AFT LATERAL ENGINE MOUNT LINK ASSEMBLY HAD MOVEMENT ON THE OUTBOARD END RELATIVE TO THE AIRFRAME STRUCTURE. DETERMINED THAT THE AFT LATERAL ENGINE MOUNT TOP AND BOTTOM AIRFRAME ANGLES HAD ENGINE MOUNTING HOLES WHICH WERE ELONGATED. REMOVED AND REPLACED L/H AFT LATERAL UPPER AND LOWER MOUNT ANGLES.
DURING AN "A" INSPECTION, TWO METAL PARTICLES WERE DISCOVERED IN THE AFT TRANSMISSION OIL FILTER. THESE METAL PARTICLES ARE IDENTIFIABLE AS BROKEN SEAL TANGS. TRANSMISSION WAS REPLACED AND RETURNED TO CHIR823C. AFT TRANSMISSION WAS REPAIRED. ALL ASSOCIATED DISCREPANCIES WERE CORRECTED IAW CHI C07-5 CR/OM SEC 62-10-04 REV 4 DATED MAY 09, 2017.
NR 2 ENGINE FIRE LIGHT ILLUMINATED FOR A BRIEF AMOUNT OF TIME. PILOTS CONFIRMED THAT THERE WAS NO FIRE AND DETERMINED THAT IT WAS A FALSE FIRE INDICATION. SUPPLEMENTAL INFORMATION; THE DETECTORS IN THIS A/C HAVE BEEN REPLACED WITH P/N 477223 FLAME DETECTORS IAW COLUMBIA HELICOPTERS, INC. SERVICE BULLETIN NUMBER 107-26-S0004. DETAILS ARE ON FILE WITH OPERATOR CHIA823C. THIS SUPPLEMENTAL INFORMATION WILL BE REPEATED ON THE FOLLOWING SDRâS; CHIR20160608001, CHIR20161003001, CHIR20140217001, CHIR20140214001, CHIR20161006001. SHORTLY AFTER DEPARTURE, THE NR 2 ENGINE FIRE LIGHT ILLUMINATED FOR A BRIEF MOMENT. PILOTS CONFIRMED THAT THERE WAS NO FIRE AND DETERMINED THAT IT WAS A FALSE FIRE INDICATION. AFTER APPROXIMATELY 1 MINUTE OF THE FIRE INDICATION LIGHT ILLUMINATION, THE PILOT PULLED AND RESET THE CIRCUIT BREAKER AND THEN THE FALSE FIRE INDICATION ILLUMINATED AGAIN AND CONTINUED FOR APPROXIMATELY AN ADDITIONAL 15 SECONDS. FIRE DETECTOR BREAKOUT PANEL C07T2953-9 WAS INSTALLED AT THE TIME IAW SL 107-0171 AND IT INDICATED THAT THE NR 2 AFT UPPER INBOARD FIRE DETECTOR TRIGGERED THE FIRE INDICATION SYSTEM. LANDED AND THE AIRCRAFT WAS SHUT DOWN. PERFORMED A VISUAL INSPECTION OF THE ASSOCIATED AIRCRAFT WIRING AND COULD NOT FIND ANY DEFECTS. REMOVED AND REPLACED THE NR 2 ENGINE AFT UPPER INBOARD FIRE DETECTOR.
CRACK DISCOVERED IN THE MIX BOX COVER, PN 107D25535. THE CRACK RUNS FROM JUST UNDER THE WEB ON THE TOP OF THE MIX BOX LT SIDE, INTO THE O-RING GROOVE IN THE FRONT OF THE COVER. CRACK IS APPROX 6 INCHES OVERALL IN LENGTH. DAMAGED AREA OF THE COVER HAS BEEN SENT TO LABORATORY FOR ANALYSIS. (K)
NR 1 CLUTCH HAD OVERRUN DURING TOPPING CHECK AND WOULD NOT RE-ENGAGE. NO PROBLEM FOUND WITH CLUTCHES, HUB OR PINION. (X)
NR 2 CLUTCH DISENGAGED DURING FLIGHT. FOUND CLUTCH SPRAGS MAY BE UNDERSIZED. ALL OTHER PARTS CHECKED GOOD ON DIMENSIONAL INSPECTION.
