CORRECTIVE ACTION: COMMERCIAL JET IN KDHN REMOVED DAMAGED AREA AND PERFORMED A HFEC INSPECTION OF THE AREA. THEY FABRICATED A NEW REPAIR DOUBLER USING .050 THICK CLAD 2024-T3 ALUMINUM. DRILLED, TREATED, PRIMED AND INSTALLED -003 DOUBLER, IAW EO 2020-1118 AND 8110-3.

THE LANDING GEAR EXTENDED SUCCESSFULLY AND LANDED SAFELY. THE ACFT WAS FLOWN UNDER SPECIAL FLIGHT PERMIT, WITH GEAR DOWN AND LOCKED, TO MAIN BASE. MAINTENANCE COULD NOT VERIFY PROBLEM. R & R RT DOOR ACTUATOR. RIG, OPS, LEAK CHECK GOOD IAW MM 32-34-4. ADJUSTED RT MLG INBD DOOR POSITION SWITCH IAW MM 32-60-3. OPS CHECK WAS GOOD. PERFORMED TEST OF GROUND BY-PASS LEVER. OPS CHECKED GOOD IAW MM 32-31-3. PERFORMED TEST ON GEAR DOOR LATCH MECHANISM. OPS CHECKED GOOD IAW MM 32-31-4. AIRCRAFT WAS TEST FLOWN FOR OPS CHECK OF GEAR INDICATION. THE TEST FLIGHT WAS CONSIDERED TO BE UN-SATISFACTORY. SUBSEQUENT TO THE TEST FLIGHT MAINTENANCE CHECKED POSITION OF MLG DOOR SWITCH ADJUSTMENT BOLTS IAW MM 32-60-3. OPS CHECK WAS GOOD. R & R DIMMER BOX AND OPS CHECKED GOOD. R & R THE LT MLG ACTUATOR IAW MM 32-32-2. THE OPS AND LEAK CHECK WAS GOOD. THE RT GEAR DOOR SWITCH AND STIFFENER WAS REMOVED. FABRICATED AND INSTALLED THE NEW STIFFENER IAW SRM 51-60-0. REINSTALLED THE SAME RT GEAR DOOR SWITCH IAW MM 32-60-1 AND RIGGED. THE LANDING GEAR WAS CYCLED SEVERAL TIMES AND OPERATIONALLY CHECKED GOOD IAW MM 32-30-0. THE ACFT WAS SUBSEQUENTLY TEST FLOWN. THE TEST FLIGHT WAS SATISFACTORY AND THE ACFT WAS RETURNED TO SERVICE. THE STIFFENER REPLACEMENT IS SUSPECTED TO HAVE CORRECTED THE PROBLEM.
CREW REPORTED AFTER GEAR UP LATCH CHECK, THE GEAR DOOR OPEN LIGHT ILLUMINATED. REMAINED ILLUMINATED AFTER COM 5.32.15. CONTACTED FLIGHT OPS TO INFORM OF SITUATION AND DIVERTED. CREW DECLARED EMERGENCY. AFTER GEAR EXTENSION 3 GREEN DOWN AND LOCKED LIGHTS WITH EXTINGUISHED GEAR DOOR OPEN LIGHT. LANDED WITHOUT INCIDENT. THE AIRCRAFT WAS FLOWN UNDER SPECIAL FLIGHT PERMIT, WITH GEAR DOWN AND LOCKED, TO MAIN BASE. MAINTENANCE VERIFIED AND FOUND RT GEAR DOOR POSITION SWITCH MALFUNCTIONING. R & R SWITCH IAW MM 32-60-3. NO ADJUSTMENT REQUIRED. SYSTEM CHECKS GOOD IAW MM 32-60-3. ACFT RETURNED TO SERVICE.

LEFT DOWNLOCK GEAR INDICATOR, GREEN, STAYS ILLUMINATED AFTER GEAR RETRACTION. CREW REPORTED THAT AFTER GEAR HANDLE PLACED UP, LT MAIN GEAR GREEN LIGHT REMAINED ILLUMINATED. GEAR APPEARED TO HAVE RETRACTED, ABSENCE OF WIND NOISE AND NO YAWING MOMENT. CREW REFERRED TO THE COM, DECLARED EMERGENCY, PERFORMED LOW APPROACH, ATC ADVISED GEAR APPEARED NORMAL. UNEVENTFUL APPROACH ND LANDING ACCOMPLISHED. MX TROUBLESHOT THE PROBLEM. MX R & R LT MLG SAFE LIGHT SOCKET IAW MM 20-00. THE SYSTEM THEN OPERATIONALLY CHECKED GOOD IAW MM 32-60-00. NO FURTHER DEFECTS NOTED.
MX DISCOVERED .5" SKIN DAMAGE ON THE L1 DOOR CUT OUT UPPER EDGE OF FUSELAGE. MX REPAIRED IAW IAW DWG 53-13164-1 AND 8110-3.
MX DISCOVERED A 1" CRACK ON THE L1 DOOR TOP FLANGE 12" FWD OF AFT EDGE. FERRIED ACFT TO WHERE THEY REPAIRED THE DAMAGE IAW SRM 51-30-02 PGS, 1-4, 51-30-5 PGS, 1-22 AND 52-05, FIG 67.
CREW REPORTED AFTER TAKEOFF UPON CLEANUP AND ACCELERATING IN CLIMB THEY NOTED A VIBRATION CHANGED THROTTLE SETTINGS/NO CHANGE. NO CONTROL ISSUES. CREW DECLARED AN EMERGENCY WITH APPROACH TO RETURN TO DEPARTURE. ACFT RETURNED TO POINT OF DEPARTURE AND LANDED WITHOUT INCIDENT. MX FOUND THE RAM AIR COOLING DUCT FOR LT & RT HEAT EXCHANGERS WITH LOOSE CLAMPS (SLIPPED OFF) ON RUBBER COUPLINGS. LT HEAT EXCHANGER DUCT CRACKED AT RAM AIR INLET FLEX COUPLING MOUNTS. THE LT HEAT EXCHANGER DUCT CRACK WAS WELDED IAW PROCESS STANDARDS 10.225. THE LT & RT HEAT EXCHANGER CLAMPS WERE REINSTALLED AND TIGHTENED. OPS CHECK WAS GOOD. ALL WORK PERFORMED IAW MM 21-20-06. AS A PRECAUTION A VIBE SCAN SURVEY WAS PERFORMED ON BOTH ENGINES IAW MM 72-00-00. NO DEFECTS WERE NOTED AND BOTH ENGINES WITHIN LIMITS. THE ACFT WAS SUBSEQUENTLY TEST FLOWN TO VERIFY THAT VIBRATION WAS NO LONGER PRESENT. TEST FLIGHT WAS SATISFACTORY. ACFT WAS RETURNED TO SERVICE.
AT CRUISE ALTITUDE (FL 340), AT NIGHT, CAPTAIN NOTED A REFLECTION OF THE MOON ABOVE HIS HEAD IN THE COCKPIT. UPON INVESTIGATION THE PILOT DISCOVERED THE LT AFT COCKPIT WINDOW HAD A SEMI-CIRCULAR CRACK APPROX 8" LONG BY APPROX ,5" WIDE. THE CREW DECLARED AN EMERGENCY AND REQUESTED AN EMERGENCY DESCENT. THE DESCENT WAS UNEVENTFUL AND THE ACFT MADE AN UNSCHEDULED LANDING. THE ACFT REMAINED PRESSURIZED DURING THE DESCENT AND LANDING. MX REPLACED THE DEFECTIVE WINDOW AND THE ACFT WAS RETURNED TO SERVICE. A CAUSE FOR THE FAILURE OF THE WINDOW COULD NOT BE DETERMINED.
RUDDER TRAVEL UNRESTRICTED LIGHT CAME ON IN CRUISE FLIGHT. FOUND VERTICAL STABILIZER PITOT STATIC LINE CHAFED THROUGH AND LEAKING AT BULKHEAD. WHEN REMOVING SAME LINE FROM ANOTHER AIRCRAFT IN HEAVY CHECK, FOUND LINE CHAFED IN THE SAME PLACE. (N NR 196US). SEE IPC 34-11-00 FIG 63 ITEM 35. LINES ARE CHAFING WHERE THEY GO THOUGH THE BULKHEAD AT ITEM 100. ITEM 100 IS A RUBBER GROMMET WHICH FAILS ALLOWING LINE TO CHAFE. REPAIRED BY REPLACING LINE AND GROMMET. ALSO ISSUED FLEET CAMPAIGN TO CHECK THESE LINES ON ALL OUR DC9 AIRCRAFT. NO SUBSEQUENT FINDINGS.
YIP - LANDING GEAR HANDLE FROZEN IN THE 'UP' POSITION. AFTER MISSED APPROACH, GEAR HANDLE OPERATED NORMALLY. INSPECTED AND FOUND ICE AND STANDING WATER IN BOTH MAIN GEAR WHEEL WELLS AND GEAR CONTROL VALVE AREA. REMOVED WATER AND ICE FROM BOTH WHEEL WELL AREAS. (M)
YIP - UNSCHEDULED LANDING MADE, OIL QTY WENT TO ZERO AND LOW OIL PRESSURE LIGHT FLICKERED. THE OIL PRESSURE REMAINED NORMAL ACCORDING TO THE PRESSURE INDICATOR. REPLACED THE OIL QTY TRANSMITTER AND REPLACED THE OIL PRESSURE SWITCH. ENGINE OPERATED AND CHECKED FOR PROPER OPERATION AND FOR LEAKS. NO DEFECTS NOTED. REFERENCE Y2PA96060.
YIP - UNSCHEDULED LANDING MADE, OIL QTY WENT TO ZERO AND LOW OIL PRESSURE LIGHT FLICKERED. THE OIL PRESSURE REMAINED NORMAL ACCORDING TO THE PRESSURE INDICATOR. REPLACED THE OIL QTY TRANSMITTER AND REPLACED THE OIL PRESSURE SWITCH. ENGINE OPERATED AND CHECKED FOR PROPER OPERATION AND FOR LEAKS. NO DEFECTS NOTED. (M)
YIP - FLT 17790 - NR 2 ENGINE TAKES 5 SECONDS LONGER THAN NR 1 ENGINE TO SPOOL UP AND RUNS 50 DEGREES HOTTER THAN NR 1 ENGINE. ALL READINGS WITHIN LIMITS; HOWEVER, ENGINE CONDITION MONITORING SHOW ERRATIC EGT INDICATIONS. PERFORMED BORESCOPE INSPECTION OF NR 2 ENGINE. FOUND NR 6 AND NR 7 BURNER CANS SHIFTED APPROXIMATELY .0625 INCH. FOUND NR 8 BURNER CAN SHIFTED APPROXIMATELY .0625 OF AN INCH, AND DISCOVERED A 7 INCH CRACK AROUND THE CIRCUMFERENCE WELD, ADJACENT TO THE FUEL NOZZLE AREA. ENGINE REMOVED FOR REPLACEMENT OF BURNER CANS, AND DETAILED INSPECTION OF HOT SECTION. PART TOTAL CYCLES: 1,115.(X)
ORD - IN FLIGHT AT HIGH ALTITUDE AND HIGH SPEEDS HORIZ STABILIZER RUNS FULL FORWARD (NOSE DOWN TRIM). CREW MUST PLACE FORWARD PRESSURE ON YOKE TO MAINTAIN LEVEL FLIGHT. FOUND PITOT STATIC LEAK IN DRAIN BEHIND CADC COMPUTER TEST FLEW AIRCRAFT. MACH TRIM COMPENSATOR APPEARS TO OPERATE EARLY. ADJUSTED AND TESTED HORIZONTAL STABILIZER TRIM INDICATING AND SHUT OFF CONTROL CABLES FOUND LOW CABLE TENSION ON SHUT OFF CABLES RIGGED PER MM 27-40-0. MACH TRIM STILL ACTUATES EARLY. DEFERRED MACH TRIM, DMI NR 0047. REMOVED AND REPLACED AIR DATA COMPUTER. SYSTEM OPERATIONAL CHECKED GOOD PER DC9MM 22-10-0 AND 22-10-1. DMI CLEARED. (M)
ELP - ON TAKEOFF, N1 INDICATION WAS 85 PERCENT ON BOTH ENGINES WHEN EPR INDICATION WAS 1.93. INSPECTED AND FOUND PT-2 PROBES ON 'BOTH' ENGINES PLUGGED WITH 'MUD' FROM WASP'S NESTS. REMOVED OBSTRUCTIONS, AND RAN ENGINES. OPERATIONAL CHECKED GOOD. (X)
DTW/CLE - FLT AVR6023 - NOSE GEAR FAILED TO EXTEND ON APPROACH TO 'CLE'. RECYCLED THE GEAR AND RECEIVED A DOWN AND LOCKED INDICATION. LANDED WITHOUT FURTHER INCIDENT. FERRIED AIRCRAFT TO DTW FOR REPAIR (REF: USA JET FERRY FLIGHT AUTHORIZATION NR 95-002). PERFORMED RETRACTION TEST OF NOSE GEAR RETRACTION/EXTENSION SYSTEM, COULD NOT DUPLICATE MALFUNCTION. REPLACED NOSE GEAR UPLOCK RELEASE BUNGEE AS A PRECAUTION, SYSTEM OPERATIONALLY CHECKED GOOD. (X)
MMX - AIRCRAFT WOULD NOT PRESSURIZE AFTER TAKEOFF. RETURNED TO MMX. FOUND 'VENT DOOR' IN MAIN CABIN TO BE IN THE OPEN PSOITION. CLOSED THE 'VENT DOOR'. (M)
