Part: CONTROL CABLE — FRAYED (TE FLAPS) · airframe 9,046 hrs · 8,258 cycles

DURING A PHASE 1, THROUGH 5 INSP, FOUND BROKEN STRANDS IN A FLAP CABLE, PN 5565150-3 FOR THE RT FLAP. AFTER INSTALLING THE NEW FLAP CABLE IT WAS DISCOVERED THAT THE CHOICE OF LOCKING PINS FOR THE TURNBUCKLE WERE TOO SHORT OR TOO LONG. FURTHER INVESTIGATION DISCOVERED THE TURNBUCKLE BARREL, PN MS21251B6S HAD BEEN MACHINED SHORTER AND LOCKING PINS CUT TO FIT. WE ARE CURRENTLY WORKING WITH MFG TO RESOLVE THIS ISSUE.

Part: CYLINDER — STUDS BROKE (NR 2 BASE)

ON TWO DIFFERENT SCHWEIZER HELICOPTERS EQUIPPED WITH HIO360D1A ENGINES, NR 2 CYLINDER BROKE 6 BASE STUDS. THE TWO THROUGH-STUDS REMAINED INTACT IN BOTH CASES. ONE ENGINE HAD 932 HOURS AND THE OTHER 1,424 HOURS SINCE MAJOR OVERHAUL. BREAKS ARE CLEAN WITH NO SIGNS OF WORKING OR LOOSENESS AND OCCURS JUST BELOW NUT.
