THE FLIGHT CREW REPORTED THAT DURING FLIGHT OF A "STAB ANTI-ICE INOP/FAIL" ANNUNCIATOR ILLUMINATING. OPERATIONS WERE CONTINUED PER MEL 30-10-02. THE MAINTENANCE TEAM TROUBLESHOT AND VERIFIED THE RIGHT ELEVATOR HORN HEATER WAS INOP. THE DEFECTIVE HEATED HORN WAS REMOVED AND A REPAIRED UNIT WAS INSTALLED. OPS CHECKS WERE SATISFACTORY AND THE AIRCRAFT WAS RETURNED TO SERVICE
THE FLIGHT CREW REPORTED THAT THE LEFT ENGINE ANTI-ICE "LO TEMP" ANNUNCIATOR CONTINUOUSLY FLICKERS WHILE IN USE. THE MAINTENANCE TEAM TROUBLESHOT AND FOUND THE LEFT ENGINE ANTI-ICING VALVE DEFECTIVE. THE VALVE WAS REMOVED AND AN OVERHAULED VALVE WAS INSTALLED. GROUND RUN, LEAK CHECKS AND OPERATIONAL CHECKS WERE SATISFACTORY.
THE FLIGHT CREW FOUND DURING PREFLIGHT CHECKS THAT THE RIGHT FUEL SHUT OFF VALVE FAIL PREFLIGHT TESTS. THE MAINTENANCE TEAM REMOVED THE FAILED FUEL SHUTOFF VALVE AND REPLACED IT WITH AN OVERHAULED VALVE. OPERATIONAL AND LEAK CHECKS WERE SATISFACTORY. THE AIRCRAFT WAS THEN RETURNED TO SERVICE.
THE FLIGHT CREW REPORTED THAT DURING PREFLIGHT CHECKS THE RIGHT FUEL SHUT OFF VALVE FAILED TESTING. THIS CAUSED THE CANCELLATION OF FLIGHT. THE MAINTENANCE TEAM DETERMINED THE RIGHT FUEL SHUTOFF VALVE WAS DEFECTIVE. THE VALVE ASSY WAS REMOVED AND AN OVERHAULED VALVE WAS INSTALLED. OPS AND LEAK CHECKS WERE SATISFACTORY. THE AIRCRAFT WAS THEN RETURNED TO SERVICE.
THE FLIGHT CREW REPORTED THAT DURING FLIGHT THE ANGLE OF ATTACH INDICATOR (AOA) WAS NOT FUNCTIONING PROPERLY. THE MAINTENANCE PROVIDER TROUBLESHOT AND DETERMINED THE LEFT PRESSURE SENSOR HAD FAILED. THE FAULTY SENSOR WAS REMOVED AND REPLACED WITH A NEW PRESSURE SENSOR. THE MX VENDOR PERFORMED SENSOR AND AOA TESTS AND FOUND BOTH TO BE SATISFACTORY. THE AIRCRAFT WAS RETURNED TO SERVICE.
THE FLIGHT CREW REPORTED THAT DURING PREFLIGHT CHECKS THAT THE LEFT ENGINE FUEL FLOW INDICATOR WAS READING ZERO WITH THE ENGINE RUNNING. THIS DISCREPANCY CAUSED THE FLIGHT TO BE CANCELLED. THE MAINTENANCE VENDOR TROUBLESHOT AND DETERMINED THAT THE LEFT ENGINE FUEL FLOW TRANSMITTER WAS DEFECTIVE. THE MAINTENANCE VENDOR REPLACED THE TRANSMITTER WITH AN OVERHAULED UNIT. OPS AND LEAK CHECKS WERE SATISFACTORY. THE AIRCRAFT WAS RETURNED TO SERVICE.
A FLIGHT WAS CANCELLED DUE TO THE CREW NOT BEING ABLE TO START THE NR 1 ENGINE. THE MAINTENANCE TEAM TROUBLESHOT, BUT WAS UNABLE TO DUPLICATE THE DISCREPANCY. AS A PRECAUTIONARY MEASURE THE STARTER DISENGAGE SWITCH WAS CLEANED. OPERATIONAL CHECKS WERE SATISFACTORY AND THE AIRCRAFT WAS RETURNED TO SERVICE.
WHILE PERFORMING REPETITIVE INSPECTION REQUIRED BY AD 2001-03-06 FOUND BROKEN WIRE BRAIDING ON RT LOWER OUTBOARD BLEED AIR DUCT BELLOWS ASSEMBLY. THIS IS THE COMPONENT THAT IS REQUIRED INSPECTION AS PER THE AD.
DURING THE POST FLIGHT INSPECTIONS THE PILOTS OXYGEN MASK WAS LEAKING OXYGEN FROM THE SUPPLY HOSE AT THE FLOW INDICATOR. THE DEFECTIVE MASK ASSEMBLY PN 174262-12, SN 15072 WAS REMOVED & THE MASK WAS REPLACED WITH A LOANER MASK.
CREW REPORTED THAT THE STANDBY ATTITUDE INDICATOR IS PROCESSING IN FLIGHT. THE DEFECTIVE STANDBY ATTITUDE INDICATOR WAS REPLACED WITH AN OVERHAULED UNIT.
CREW REPORTED THAT THE AUTOPILOT TRIM FAILED IN FLIGHT. THE MAINTENANCE TEAM POWERED UP THE ACFT & ENGAGED AUTOPILOT, THE LT AUTOPILOT PITCH TRIM FAILED. REMOVED THE FCC CARDS FROM THE IAPS RACK, CLEANED & REINSTALLED CARDS. FAULT CLEARED, NO DEFECTS NOTED. GND CHECKS OK, ACFT RETURNED TO SERVICE.
