DURING MAINTENANCE A CRACK WAS VISUALLY DISCOVERED ON THE RIGHT HAND CLOSEOUT RIB (P/N: 5122115-56) AT R.W.S. 106.79. THIS CRACK ORIGINATES FROM A HOLE IN THE FORWARD ROW OF FASTENERS COMMON TO THE AFT MAIN LANDING GEAR TRUNNION SUPPORT PLATE AND TRAVELS FORWARD AN APPROXIMATE LENGTH OF 1.400 INCHES. THE CRACK IS CONSISTENT WITH OTHERS VISUALLY IDENTIFIED IN THIS SAME AREA. THIS RIB WILL BE REPLACED.
PILOT REPORTED BURNING SMELL IN CABIN, THEY TURNED THE HEATER OFF AND SMELL DISSIPATED. MAINTENANCE INSPECTED AND FOUND THE COMBUSTION BLOWER INOPERATIVE. MAINTENANCE REPLACED THE COMBUSTION BLOWER WITH NO FURTHER DEFECTS NOTED.
PILOT NOTED FUEL SMELL IN FLIGHT, DEACTIVATED THE HEATER AND THE SMELL WENT AWAY. MAINTENANCE INSPECTED AND FOUND FAILED SHAFT SEAL ON THE HEATER FUEL PUMP. FUEL PUMP WAS REPLACED WITH NO FURTHER DEFECTS NOTED.
CYLINDER REMOVED DUE TO LOW COMPRESSION AT INSPECTION; EXHAUST VALVE DISPLAYS EVIDENCE OF HEAT DAMAGE.
PILOT REPORTED STRONG FUEL ODOR IN CABIN. MAINTENANCE INSPECTED HEATER AND HEATER FUEL INLET, FOUND HEATER FUEL PUMP LEAKING AND REPLACED THE HEATER FUEL PUMP, OPERATIONAL AND LEAK CHECKED GOOD. ADDITIONAL DETAILS WILL BE MADE AVAILABLE UPON TEARDOWN REPORT RECEIPT.
PILOT REPORTED GAS SMELL FROM HEATER. RAN THE HEATER & FOUND FUEL LEAK COMING FROM HEATER FUEL PUMP LINE ON THE OUTLET SIDE AT FITTING. R & R HEATER FUEL LINE. PERFORMED LEAK & OPERATIONS CHECK, EVERYTHING OK IAW MM.
DURING CLIMB OUT OF A FERRY FLIGHT, CREW ATTEMPTED TO RETRACT THE LANDING GEAR. GEAR LIGHTS REMAINED IN THE DOWN AND 3 GREEN POSITION. ELECTED TO RETURN TO DEPARTURE. CREW PERFORMED A FLY BY OF THE TOWER, WHO ADVISED IT APPEARED THE RT GEAR WAS NOT FULLY EXTENDED. PERFORMED THE MANUAL GEAR EXTENSION ITEMS IN THE ABNORMAL CHECKLIST, DECLARED AN EMERGENCY AND PERFORMED A SECOND FLY-BY, TOWER ADVISED ALL GEAR TO BE IN THE EXTENDED POSITION. ACFT LANDED WITH NO FURTHER INCIDENT AND SHUTDOWN ON THE NEAREST TAXIWAY. MX TROUBLESHOT ACFT AND FOUND BROKEN WIRE AT CIRCUIT BREAKER (CB107) ON THE LANDING GEAR PANEL. MX REPAIRED THE WIRE AND OPS GEAR SWINGS AND FUNCTIONAL CHECKS WERE GOOD. ACFT WAS RETURNED TO SERVICE.
AFT SECTION OF EMERGENCY FLOOR LIGHTING IS INOPERABLE. REPLACED EMERGENCY BATTERY.
SERVICE DOOR WARNING DURING TAKEOFF. FOUND FUEL SERVICE DOOR UNLATCHED. SECURED DOOR CLOSED AND AIRCRAFT RETURNED TO SERVICE.
AUTOFEATHER SYSTEM INOPERABLE DURING TAKEOFF. CLEANED THE AUTOFEATHER ARMING SWITCHES IN POWER LEVER QUADRANT.
EXIT SIGN LOOSE AT ROW 5A. REPLACED EXIT SIGN.
'TRIM' AURAL WARNING DURING TAKEOFF. REPLACED THE AUDIO WARNING UNIT.
OUTBOARD FLAP INDICATION IS FLUCTUATING BETWEEN 2 AND 7 UNITS. COULD NOT DUPLICATE DISCREPANCY. REPLACED RIGHT HAND OUTBOARD FLAP ACTUATOR FOR TROUBLESHOOTING.

ON 1C TASK 35-09, THE OXYGEN MASKS AT ROW 9 FAILED TO DEPLOY. INVESTIGATION FOUND THE OXYGEN MASK DOOR SOLENOID TO BE SHORTED. THE SOLENOID WAS REPLACED WITH A NEW PART, OK FOR SERVICE.
SMOKE IN COCKPIT AND CABIN AFTER PUSH BACK. FOUND GROUND COOLING FANS NOT OPERATING. REPLACED THE LANDING GEAR RELAY BOX.
PCU ASSEMBLY REMOVED FROM AIRCRAFT FOR AD 95-22-12 COMPLIANCE. UPON INITIAL RECEIVING INSPECTION SEVERAL OF THE FORWARD PORTION OF THE SPLINES WERE FOUND TO BE GROUND DOWN. PCU WAS REMOVED FROM SERVICE AND RETURNED TO MANUFACTURER FOR FURTHER DISPOSITON.
WEATHER RADAR DOES NOT WORK. REPLACED THE ANTENNA.
FLAP CONTROL FAULT WARNING WITH FLAPS RETRACTED. REPLACED THE LT OUTBOARD FLAP ACTUATOR.
BIRD STRIKE ON LOWER FUSELAGE. INSPECTED EXTERIOR OF AIRCRAFT AND FOUND NO EVIDENCE OF BIRD STRIKE.
FLAP CONTROL FAULT WARNING DURING TAKEOFF. REPLACED THE FLAP CONTROL UNIT.
LT AND RT WING DEICE BOOTS AND LT AND RT ENGINE INLET DEICE BOOTS FAILED DURING FLIGHT. REPLACED WING AND ENGINE INLET DEICE BOOTS.

RT HYDRAULIC RESERVOIR LOW PRESSURE INDICATOR ILLUMINATED. REPAIRED CHAFED WIRE AT PRESSURE SWITCH.
LANDING GEAR INTRANSIT LIGHTS REMAINED ON WITH GEAR RETRACTED. REPLACED THE LANDING GEAR CONTROL BOX.
FLAP DISAGREEMENT WARNING WHEN FLAPS RETRACTED AFTER TAKEOFF. COULD NOT DUPLICATE DISCREPANCY. CLEANED AND INSPECTED FLAP TRACKS AND OPERATIONAL CHECK NORMAL.

THE PILOTS VSI IS COMPLETELY UNUSABLE IN ALL PHASES OF FLIGHT. REPLACED THE PILOTS TCASIVSI.
LEFT TORQUE INDICATION ERRATIC. LEFT PROPELLER AUTOFEATHERED. REPLACED THE TORQUE SIGNAL CONDITIONER.

LEFT ENGINE FAILED ABRUPTLY IN FLIGHT. REMOVED AND REPLACED LEFT ENGINE.

NLG A SYSTEM INTRANSIT LIGHT IS ILLUMINATING INTERMITTENTLY. REPLACED THE NLG DOWNLOCK PROXIMITY SENSOR.
